Blue screen on start up, even in safe mode, common fixes not working

I did quite a bit of searching for the problem I'm having, and it seems some of the symptoms are common, but none of the various fixes I've come across have worked. Any help pointing to other things to try would be appreciated, as it is getting harder to search for things without finding repeats of what I've already tried. Also, I was going to post this on the OSX 10.4 board, but changes in symptoms (mentioned at the end of this post) are starting to make me think this could be a hardware issue.
The main problem is that my computer boots normally until the blue screen just before the login window should appear, then hangs.
My system: iBook G4, 800 MHz, running 10.4.11, has an extra GB of RAM installed (for a total of 1.1 GB), new hard drive that is less than a year old.
Things I've tried:
Booting in safe mode: No change, still hangs at blue screen.
Removing airport card, and extra ram has no effect.
I've done a PRAM reset
I've done a PSU reset
Checked for file system and permissions errors with Disk Utility on OS CD
Booting into single user mode works fine, where I tried:
fsck -fy, no errors reported
running /etc/rc worked fine for a while
No unexpected errors or problems seen in /Library/Logs/Console/501/console.log, or in /var/log/system.log
I've temporarily moved /Library/Preferences, /Library/Caches, and /System/Library/Caches, /User/Library/Caches, /User/Library/Preferences, /System/Extensions.kextcache and /var/db/netinfo/local.nidb to make sure none of those were corrupted (plus checked preferences with plutil), nothing fixed.
I've double checked the permissions of the root directory.
This problem did not follow an update or installation of any kind. The computer just completely locked up in the middle of using Safari (and just typing in an edit box, nothing with much of a workload) and the blue screen hang happened upon rebooting after shutting down by holding down the power button.
Single user mode appeared to work fine, although I did get single user mode to hang when trying to back up a few GB of files onto a thumbdrive after copying about 90% of the files. And then a new problem came up where afterward, now the blue screen pops up and hangs about 30 seconds after running /etc/rc returns to the prompt (and about 20 seconds after last of it finishes outputting). I've tried clearing caches again but /etc/rc still causes this.
I was going to try to run AppleJack (recommended for checking permissions and clearing caches) after copying the files to the thumbdrive, but of course didn't think to install that until after I had a problem. And now that /etc/rc causes a problem, I don't know how to attach the AppleJack .dmg file other than hdiutil, which needs the /etc/rc to have been run, and also can't remount the thumbdrive to copy any more files over.
After this recent problem, I noticed that now booting from the OS CD goes to the blue screen too and no further. Has anyone else seen such a problem? I've heard of some issues with loose GPUs in the iBook G4, but they seem to mention scrambled or no/black video (video looks fine in single user mode and in open firmware, and up to the blue screen which is clear and bright). So now I don't seem to have an option to try an Archive & Install of the OS (which I was hoping to avoid, due to effort needed to reinstall other software).

Thank you, I had forgotten about target mode. I was able to back up all of my files. I also copied some additional 50 GB of files of the disk as a stress test and the hard drive worked fine and quick (and because it was quicker than retrieving the same files from a network drive to my desktop).
I was hoping to also unpack the Applejack files on my desktop and put them back so I could run them in single user mode without running /etc/rc. However I haven't been able to get my Linux desktop to mount the iBook in Read-Write instead of Read-Only yet (I don't have access to another Mac at the moment).

    Ive repaired 1000s of laptops over the years (translate: diagnose and replace parts).
    Its not that old, Ive owned 100s of laptops (most were free) ....some I still have are nearly 13 years old and still perfect, ....some extremely expensive ones died in under a year.
    other than becoming obsolete at some point, there isnt really any "will spoil on" data like Milk for solid state computer parts, ....alas.
    You can have a $5000 laptop die in 4 months, and a junker one keep kicking after 10 years.... Ive seen that firsthand countless times.
    Common logic dictates that "very expensive = long life" in computers,..... but all the repair people will tell you just the opposite.
    I know you have a gripe, I get that all too well honestly .

  • Firefox crashes on start-up, even in safe mode

    Back in March/April 2012, Firefox started crashing for me on start up, possibly after the automatic/remote installation of MS security patches. After that time, I've not been able to start Firefox without it going directly to the Mozilla Crash Reporter. I've tried everything I could think of including safe-mode, uninstall/re-install, uninstall/back-up profiles directory/wipe profiles and all registry entries/re-install. This continues to fail since version 11(?) through 13.0.1. Even Sea Monkey crashes same manner. BTW, clicking on the "Details" button in the Mozilla Crash Reporter has no effect for me. I was trying it to help fill in the Troubleshooting Information box below as I can't get to "Help > Troubleshooting Information" as Firefox doesn't start.

    That one does not seem to give any useful info at present.
    Malware is one common cause of crashes, and that may produce variable crash reports, it is probably worth doing full scans with multiple tools/software to try to rule out the possibility of malware.

    Besides trying in safe mode did you also try disabling all plugins whilst in safe mode. (If it crashes before then, are you able to create a new profile and try that ? ) Also if crashing before about:crashes may be entered have you seen http://kb.mozillazine.org/Breakpad#Location_of_crash_reports

    I had the same problem after an upgrade to 3.6.3.
    I was using password manager for a number of web sites before. Not using cookies but password manager.
    After 3.6.3 auto upgrade it did not work.
    I could not enter private browsing mode.
    When I looked in tools>options>security at my saved passwords the list was blank. The button in the saved passwords dialog that normally says "Show saved passwords" was actually blank (the button was there but there was no text on it).
    I did have my old bookmarks and was able to create new ones but oddly the bookmark all tabs menu function wouldn't work.
    I decided to reinstall firefox 3.6.3. There is no way something with that many bugs got out!
    I was able to get the full firefox executable from http://mozilla.com
    I closed all firefox windows and ran the installation.
    After re-running the install and starting firefox everything was fine. It must have been a bad download or the original install got messed up somehow.
    I hope you have the same good fortune I had!

  • Will not load sites linked from another website--have cleared cache, cookies, history, restarted, checked in Safe Mode--still does not work.

    Some sites load fine. But some sites will not load when linked from another web site, or, occasionally, even from my bookmarks. I went through troubleshooting routine--cleared cache and cookies, history, checked in Safe Mode for extensions, etc., but still will not work in Safe Mode. No other problems with computer or browser.

    What happens when you click the link that does not open up the new page? Is it a blank page? Does this also happen if you Shift Click the link?
    Sometimes a problem with Firefox may be a result of malware installed on your computer, that you may not be aware of.

  • After upgrade to 8.0 FF crashes every time I try to start it, even in safe mode

    I guess the automatic upgrade happened last night (i hate that you auto upgrade btw) and now FF at work crashes every time I try to open it. I've had problems with FF ever since 7.0 came out. Please fix these problems because FF is almost unusable now :( So sad it is such a great program that has been ruined now

    It seems to be caused by roboform 7.5.1. I upgraded to the latest version of robo and it's working now. Sorry mozilla, not your fault.. very frustrated.

  • Blue Screen on start up (NOT a new install)

    I am using a MacBook Pro (Intel Duo Core 2.4GHz - 4GB RAM). I am running OS X v10.5.7. No recent updates around the time of the problem.
    For three days I am unable to boot up my system, I hold at the blue screen. I cannot boot in safe mode either. I have done the PRAM reset, run disk utilities from the Mac disks. No errors were found on the HDD. When I try to reapir permissions i get the following "error: The underlying task reported failure on exit." and then "permissions verification complete"
    Eventually I decided to do an Archive and Install. Everything "loaded" from Disk 1, but then a notice came up saying that the installation failed.
    I am at a loss and our IT guy is not Mac savvy.
    Any suggestions? I don't have a recent backup, though I think I can get my data off with target disk mode. I really prefer not to have to do a wipe and reinstall, as I have quite a bit of software on the machine.

    The inability to repair permissions might mean something, but I (unfortunately) haven't a clue what that could be.
    Not being able to complete an Archive & Install--despite the hard drive supposedly being "OK" is also worrisome.
    Unfortunately, the only other thing I can think of is to save your important data, find the installation discs for your software and try an Erase and Install.

  • My computer won't boot up even in safe mode

    I have a 2010 Imac that has been a great computer. Yesterday, I tried to turn it back on and it wouldn't boot up. The white screen with the apple came on, the little wheel spun and then it went directly to a dark gray screen that I could see the mouse present. 
    I looked on line the directions to start it up in safe mode, but that didnt work either.
    Help.  I hope it is fixable.

    The likely problem is use of certain Oracle etc software, the developer Oracle being aware of the problem and working on a fix. Meanwhile there is a registry hack as a potential workaround, or the simpler workaround of downgrading temporarily to firefox 3.6.23.<br>

  • Windows 8.1 safe mode password doesn't work

    Using HP Pavilion 23 all in one model 23-b037c, 2012 windows 8.1.
    During testing for internet connectivity was instructed to initiate msconfig and reboot in Safe Mode.
    After rebooting the sign in security screen asked for my password.  I entered my password that has worked for long time., but system said password incorrect reenter.  tried numerous other passwords non accepted.  changed MS account PW no good. Worked with HP support several hours for 2 days, trying all advanced trouble shooting solutions they recommended. Ran restore, backup and recovery  etc. all failed.  After one backup attempt error message said must get out of SAFE Mode.
    How do you do this without correct password?
    Ran Hard disk test  passed OK. Now having to clean and remove all programs and personal files per HP Tech.. Will take several hours per HP sTech.  Then  will try again to run System recovery to original manufactures settings.  Huge mess.
    Big problem. Hope this works. 
    Q. Anyone ever have this problem?
     How do you bypass password security screen to access Windows 8.1 without going through this extensive process? tried several password recovery options non worked.
      Tried all Advanced troubleshooting options offered by HP Tech.

    I have read your post on how the Windows 8.1 safe mode password does not work on your desktop computer, and I would be happy to assist you in this matter!
    To disable the built-in administrator account for Windows 8.1, I recommend following the steps in this document on How to Enable or Disable Built-in Elevated "Administrator" Account Windows 8 and 8.1. This should help to enter your password when booting into Safe Mode.

    I got the old stuck on blue screen at start up ,
    would only boot in safe mode , after two afternoons of RAM test , repairing disk permissions , hrdware tests , zapping PRam and the likes to no avail , I finally resolved the problem thanks to a tip from this forum:
    bad video card drivers ( ATIRadeon 8500 ) , after removing them from the extension folder , it boots up! ( I tried to reset the pram , it won't work though , one chime only then nothing).
    so I'm wondering what's next , can i reinstall the video card drivers only ?
    is it the card itself ? (everything looks normal) or should I go for an archive and install ?
    any input would be great,

    Jean Baptiste mentioned that he removed extensions for the drivers.
    Removing a conflicting driver is fine as long as the driver specific for the card installed remains.
    In this case, I think it may have been a corrupt driver, rather than a conflicting driver.
    Radeon drivers in OS X have a good track record of compatibility, and such driver conflict is not a historically common occurrence.
    Hard drive maintenance with Disk Warrior is the best way to correct corrupt files, unless bad RAM is corrupting the files.....
    In your case, it shouldn't hurt anything to remove the Radeon 8500 drivers.
    Could it be the driver extensions that are preventing a proper boot. The monitor displays perfectly in safe mode so I don't understand why it would be the video card.
    Booting to Safe mode unloads drivers. Only very basic core graphics rendering drivers are loaded.
    No Quartz/Quartz Extreme, no Core Image, no 3D acceleration, etc.
    This doesn't tax a graphics card.
    Failure to boot, or failure of graphics when drivers are loaded are due to one of two things- corrupt drivers or failing components on the graphics card.
    A reinstall of the OS can rule out the first.
    Installing a different graphics card is the only way to rule out the second.
    Before buying a new card, I would try a fresh OS.
    Installing a new OS on a separate drive, and booting to that is an easy way to test.
