Creating a book in 5.5" x 8.5"

Question

I'd like to create a book in CS3 that uses regular size paper, in landscape format, folded, so that the finished book is 5.5"wide x 8.5 tall."
I made all my new documents (chapters) and then put them in a book. The problem is getting it to print right. Apparently this is a much different process then MSPublisher which is what I am weaning myself from!
The pages are set up in 4.25"x 8.5" format.  When I print double sided, the printer gives me two of the small pages per sheet, but they aren't back to back.
Should I have set up the document as a full size sheet with connected text boxes sitting side by side? Am I just not setting up the printer correctly?
Right now the book is small, about 40 pages. It's an instruction manual so it will grow as the directions are written, I'd like to fix whatever formatting I might have done wrong before it gets much bigger.

I don't think Dave's script is going to help with a book since it imposes a single document, and only for saddle stitch, but here's a link: http://pdsassoc.com/downloads/Buildbooklet.zip
My usual advise for this situation is to export the book to PDF, then plaace the PDF ina new single ID file, and use Print Booklet.
