Firefox has always opened with an extra tab that says "server not found." How can I fix this?

Several years ago, I downloaded Firefox for the first time on my Xp machine. The browser opens to the home page, but up in the tab area, there is always, and I mean through EVERY version and upgrade since that first one, including the most recent version 4, there is a second tab just to the right of the home page tab that says, "server not found." Now, since the home page loaded, it is obvious to me that the server WAS indeed "found." what I don't understand is why this second tab is there. Any ideas? I have read through the FAQ about not finding the server, and nothing there is relevant to this, as far as I can see.

How many pages do you have set as your "homepage"? <br />
Firefox allows for multiple homepages in separate tabs, separated by the pipe | symbol.
Try resetting your homepage. <br />

Although possibly not related to your problem, I have to remind you that the version of Firefox you are using at the moment has been discontinued and is no longer supported. On top of this, it has known unpatched bugs and security problems. I urge you to update to the latest version of Firefox, for maximum security, stability, performance and usability. When a new album is created it will fill with whatever is in the right hand window, If you're in the Events or Photos mode it will contain all photos in the library. Yes, strange behavior.

Clear the cache and the cookies from websites that cause problems.

You do NOT need to restore your iPad.
I believe that the problem is that there is a disconnect with your iTunes account and your iPad. What you are experiencing is a somewhat common issue after you sync your iPad, update your device - it could be a number of reasons that are causing it. the recommended fix is to download another app in order to "reconnect" your ID and iTunes account to the iPad.
You can read more about it here. You will need a WiFi internet connection in order to fix it - or you have to be able to sync with your own iTunes library.

If FaceTime ever worked properly for you, restarting your modem (and separate router/wireless if you have such) may help. If the problem continues, try connecting at a different location to see if the problem is within your wi-fi.
The black dot problem may mean you need to reset your iPod. If that does not help, it may need service by an Apple Authorized Service Provider.

Following up: I went with Option 2 above, and it seems to have worked quite well. I didn't even need to quit iTunes. After the files were moved, the exclamation marks remained at first, until I played the file, or restarted iTunes - now all seems well, until the next crash anyway.

You're ignoring context. On the old Apple Discussion Groups I never posted replies like that, instead giving people relatively detailed answers. The new Apple Support Communities made things worse by introducing certain inefficiencies. Then came Lion. The flood of messages that came with Lion required a painful choice for any of the people who had been helping here: (1) Give quality responses to a few questions and ignore the rest. What does "online documentation" mean exactly? Are you using the Notes app to try to create notes in iCloud, for example?
If so, that should still work in iOS 7.1.
If not, please tell us specific symptoms of what is (or is not) happening.
