Have a form sent to an email address after completion without the use of Mailto:

This project is for a dentist office.
We need to have the customers go to the office website and fill out a form before their appointment. I need the completed form to the sent by email to the receptionist.
Below are some of the issues to be resolved;
Mailto: will not work with web based emails.
Some users will have trouble with download, complete, and attaching the form to the web email client.
Do not need to analyze data I only need to print the form it can be filed in the office records.
In essence I need a web form that can be filled out on the web and when submitted show up in a mailbox.
I was hoping this could be done through Form Central, however if coding is require I have some experience with ASP.net and the office website is sitting on an ASP server. Also if this project needs coding to accomplish please leave sample(s), or links to point me in the right direction.

Could I use FormsCentral to collect the data than have one of the office personnel login and retrieve the data into a PDF format and/or download data and merge into PDF form in the office for printing. Also if the previous is doable, can the FormCentral collection form sent an email that a form has been fill out.

The problem is that the Form Distribution Wizard only accepts a single email address for the return data. Unfortunately this cannot be changed because the Wizard also applies Reader-enabling to the form so it can't be changed later.
As a work around you can try using the Wizard to create the collection DataSet, but not send out the form. Instead send out your original form with the multiple return addresses. Be sure to Reader Enabled the form for "Form" and "Save" rights, and make sure the submit is returning the entire PDF. When the files come back from the users you'll have to manually add them into the data collection.

You will need to find a tutorial for sending email via AS3 (normally requires a PHP or other server-side processing file). Search Google using terms like "AS3 send email tutorial" and develop things from there.

Was just wondering - is there a way to show the form submitters email address as the "From" email address when receiving a notification from the contact form?
There's a setting that allows the form details to be sent to an email address, but that email is from the user or the site itself, so if you "reply" from within your email client, you're effectively emailing yourself...
Do I have to create a new workflow as in this thread?

Hi Dave,
The proper way to do it is when you get a workflow or submission notification it automatically creates a 'Case' in the site admin.
It is also listed in your live feed on your dashboard.
So the next steps would be to go to that case in the admin and reply there. It will keep a record of your reply within the admin and it will send to the user.

Yes, when you set up the Submit Form action, specify a mailto type URL. Make sure mailto is lower case and there are no spaces, something like: mailto:[email protected]
If you want Reader users to be able to send the entire PDF, it must be Reader-enabled: Advanced > Extend Features in Adobe Reader

As with the Mail app on a Mac, the iPhone's Mail app stores all email recipients in a list of previous recipients which is separate from the address book or contacts. The email address autofill feature when addressing an message pulls from Contacts and from the list of previous recipients.
Different from the Mail app on a Mac, there is no access to the list of previous recipients with iOS on an iPhone, iPod Touch, or iPad to remove a previous recipient from the list, or to add a previous recipient to the address book or contacts that is not already entered - not at the present time anyway.
The only way to purge the list of previous recipients at the present time is by restoring your iPhone with iTunes as a new iPhone or not from your iPhone's backup, which will start being re-populated again with every sent message.

When clicking on name in address book -the email address appears different in the email address bar and can't be sent -why?

I have been using thunderbird for some years - in last month when I have been trying to send some emails I have a problem with some of them in that part of the email address has changed when I insert it ino the address bar and has inverted commas around the first part of the address before the @. I have tried deleting the addresses and re-inserting them but the same thing happens. If I go into the address bar and delete the commas the email is sent - can I overcome this problem without having to go into each email? ok, I've spent some time doing various scenarios.
I have managed to replicate one scenario which does as you say.
If my contact in the address book:
With name details as Peter Foo
email address: [email protected]
where there is an inserted space before the name peter, of course this is not instantly noticeable, but it is as if you hit the space bar just before typing the email address.
Then when I use the Contacts Sidebar in a Write message, it will add the email address like this:
Peter Foo <" peter"@somewhere.com>
which of course in invalid.
but in the address book, if highlight the contact, in the section below the email address in blue -if I click this to send an email the email address is printed exacly with the space and it will send as if nothing is wrong.
So i'm asking you to check those email addresses which insert incorrectly via the Contacts sidebar. They only do this for me if there is a hidden inserted space at the start of the email address.

You may change your Apple ID to any email address that is not already used as another Apple ID, or that is not already associated with another Apple ID.
In other words, if I have an Apple ID [email protected], and have alternate email addresses of [email protected] and [email protected], none of them may be used by me or anyone else for an Apple ID, Rescue Email, alternate email, or anything Apple-related.
If the Apple ID link above indicates your email is already in use then it is. Use https://iforgot.apple.com, select Forgot your Apple ID? and provide the information requested. If the email address is one you created and can still access, you may be able to retrieve it. If it is one someone else created, you will not.

It sounds like you have two Email account preferences: one for your .mac the other for the aol. Depending on which account you have selected will determine which outgoing email server is used. Check that the selected mac account prefs is using .mac as its outgoing mail server.

Emailing a PDF form w/o an email address submit button

I have created a form; but my company does not want it to go to specific email address. So I took out the submit button. What they want is for people to be able to fill in the PDF form and send it to whomever they choose via email. BUT in reader, they cannot save the PDF. What do I need to do to get this to work correctly?

I had the same request.
All i did was leave the email address blank. On the "Design View", it will show an error but the employee will not see this. They can then just enter an email address.

Do you get a drop down list with suggestions if you clear that "from" field and click elsewhere on that page to move the focus off that field and then double-click again in that field or start typing the first letters of that unwanted email address?
