Apple does not fix, they replace the phone. Cost for an iPhone 4 is US $149. Make an appointment at any Apple store.

    Apple does not repair the iPhone 4 screens. It will be a complete iPhone 4 replacement. But if you make a Genius Appointment at Apple Store and just tell the Genius the truth, you never know, but you could get a one time free replacement.

    The glass is part number 922-9795 and dvwarehouse sells it.  So that might give you an estimate of the minimum cost for replacement (unless you do it yourself).
