I see the "Connect to iTunes" screen and the device appears in iTunes. Every time I connect to iTunes, iTunes says my device is in recovery mode and has to be recovered so I do so. The same recovery message shows in iTunes after iTunes is complete. I have a iPod touch 64gb. I have no idea what OS is on it now that iTunes has tried to recover it and erased all info.

Try these steps to resolve the issue:

Put the device in DFU mode.
Restore as new, once the restore completes, then try restoring from backup.

I did the restore and it cleared and reset the iPod but after it finished the same error came up. I tried this 3 times. Also it no longer shows up as a drine in My Computer.
if it's relentlessly doing that to you, it's plausible that the message is being caused by a Windows drive letter confusion. (that's consistent with the disappearance for My Computer, too.)

Check your USB connections
Related errors: 13, 14, 1600, 1601, 1602, 1603, 1604, 1611, 1643-1650, 2000, 2001, 2002, 2005, 2006, 2009, 4005, 4013, 4014, or “invalid response."
If the USB connection between your device and computer is interrupted, you may be unable to update or restore.
To narrow down the issue, you can also change up your hardware:
Use the USB cable that came with your device, or a different Apple USB cable.
Plug your cable into a different USB port directly on your computer. Don't plug it into your keyboard.
Try a different computer.
If necessary, resolve any further issues with the USB connection, then with your security software.
If you're still seeing the error message, check for hardware issues by following the next section.

Once the Device is asking to be Restored with iTunes... it is too late to save anything... and you must continue with the Recovery...

I downloaded 5 to my itunes. When I tried to update my iPad 2 it extracted the informaion and then when it tried to restore the iPad, I got an error message saying that my iPad malfunctioned, it went into recovery mode, and it would not restore.

When I downloaded (on PC) the new 5 it crashed my iPad 2. I worked for two days to get it to stop the malfuction, went to the apple store for four hours, iPad crashed again, got a new one, new iPad crashed when I tried to restore my back up.
