Ask her for the passcode  If she does not have or remember it restore it as new phone.  All data on it will be lost

    Buying an off-contract phone does not guarantee that it will be an unlocked iPhone.  You need to explicitly request it to be an unlocked iPhone when you purchase it.
    As KiltedTim suggests, try returning it to the store where you purchased it in exchange for an unlocked iPhone.  If that is not possible, you can try contactign T-Mobile to see what their policy for unlocking iPhones is, they may do that for you (but they may charge a fee too).

         2. Turn off the device: Press and hold the Sleep/Wake button for a few seconds until the
             red slider appears, then slide the slider. Wait for the device to turn off.
              If you cannot turn off the device using the slider, press and hold the Sleep/Wake
              and Home buttons at the same time. When the device turns off, release the Sleep/Wake
              and Home buttons.
         3. While pressing and holding the Home button, reconnect the USB cable to the device.
             The device should turn on. Note: If you see the screen pictured below, let the device
             charge for at least ten minutes to ensure that the battery has some charge, and then
             start with step 2 again.
         4. Continue holding the Home button until you see the "Connect to iTunes" screen.
             When this screen appears you can release the Home button.
         5. If necessary, open iTunes. You should see the following "recovery mode" alert:

    She would have to ask her carrier if they unlock iPhones and if she qualifies for this service.

When I try to sync the ipod by connecting it to my computer the ipod does not show up in the itunes drop down menu. Is thid because the ipod was named by my daughter? Does she need to deauthorize it from her Mac computer?

    I'm not sure what you mean by "drop-down window", but your daughter doesn't need to do anything for you to claim and use her iPod. If you have iTunes 11, the iPod should appear as a button in the upper-right of the iTunes window, e.g.:
    If you have an earlier version of iTunes, you may need to update, but normally the iPod would appear in the sidebar under the "Devices" header.  If it does not appear in either place, this support article may help:

    I did change to my own cell number (with AT&T).
    I am not sure whether I restored and activated under MY name/Apple ID. (Out of convenience I have probably maintained HER Apple ID since I know the password.)
    When I text people her name shows up and people don't realize it is me. (I don't know what appears with phone use.)

    Use different Apple IDs for iMessage (and also FaceTime) to avoid getting each others text messages (and FaceTime calls).  One of you should go to Settings>Messages>Send & Receive (and Settings>FaceTime), tap the ID, sign out, then sign back in with a different ID.  You can continue to shared the same ID for the iTunes store and other services.
