I'm trying to do a mail merge but cannot customize the merge target

Question

I'm trying to merge a fields from a Numbers document into a Pages document to create a customized form letter.  I have been able to customize the merge field name so that I can create a Company merge field in Inspector Link's merge table, and while I can do that, I cannot customize the target field.  Target fields seem to be limited to First Name, Last Name, Address, City, Prov, PC (as they are shown in my Numbers document), even though I do have a column entitled Company.  For some reason, when I customize Company as a merge field, what shows up in the target field in the merge link table is "First Name", and there is no option to select anything else from the drop down menu other that what I have noted above.
Help ... please.  Desperate!!! And slowly going crazy!!!

Hello Peter,
I'm not sure what you mean by "Have you ensured your Numbers table has this exactly (U&lc) in the header?", but this is what I have done.
This is how my column titles look in Numbers, i.e., Company is one column title, First Name is another, etc.:
Company
First Name
Last Name
Address
City
Province
PC
This is how my merge fields look in my Pages document when trying to set up the merge fields:
First Name Last Name
Company
Address
City, Province   PC
Here's a screen shot of Inspector Link - Merge looks like:
I cannot for the life of me figure out what the heck I'm doing wrong ... if in fact I am doing something wrong.  Oh yes, when I click on the arrows in the target name column, the only choices available in the drop down menu are:
First name
Last Name
Street
City
Province
PC
Company
Now, if you, or anyone else, can figure that out, I'd really appreciate it.
