Is it possible to create a form with multiple form fields on a single line?

Is it possible to create a form with multiple form fields on a single line? I can't find anything in the documentation or a template that does this.
I am trying to create a "documents received" checklist with a check box on the left margin, a date-received field to the right of the check box and and a description of the document (Formatted Text) on the far right.
In the past I have entered the Fixed Text with a word processor, published it to a PDF file, then added the check box and date fields with the Acrobat Forms editor. I would prefer to use FormsCentral if it is possible.

We now support multiple fields on one line. This post provides a brief overview.

Our school has a Forms Central account which works great for our registration forms but I need to find a solution for creating an online purchase form with multiple calculation fields - I know that forms central does not support calculation fields (too bad) but I know that Acrobat Pro does... soooo...
Can you create the forms in Acrobat and then somehow integrate the advanced features into forms central? Do they talk to each other? Is this easy to do? .... I guess another way to putting it is can you create the forms in Acrobat including all of the advanced features for payment calculation and then host it online using Forms Central to manage and collect the data? (I guess that really is my question)

The naming convention was the consistant up until a point when I read that you need a '.' syntax (?!) - does anyone know if this is true?
Attached is a version with Bernd Alheit's suggestion and with all the naming of the fields being consistant. It's still not working for me though after doing this and I'm stuck as to why, because I think it should work. I've also tried writing the calcualting line of code in the same manner that Bernd Alheit suggests before I came on here, and it wouldn't work then.
As with any coding, it must be something to which I have done, but I can't see it anywhere

Using CR XI. Using CSV input from ODBC text driver. No problems here.
There are 3 fields that I want the same group by action. Is this possible?)
If any one of these 3 fields change I want some header information and column fieldnames to display. The header information I only want displayed on the first page (on the change) and the column fieldnames to display on every page. I have played around with things like "InRepeatedGroupHeader" and "report group header on each page". I am not getting all the results that I am looking for though.
I want report to look like:
Page header stuff (to display on every page) This data consists of some fields that will have static data and will display on every page. And it consists of 3 non-static fields that I want to group on.
Group header stuff that I only want displayed on the first page on a change from one of the 3 fields mentioned above.
Column headings that I want displayed on every page.
Details data
Group footer stuff (details not important for this)
I get the expected output (column headings displayed on every page, and group header stuff display on the first page on each group change) with one group field. But I want the group change to happen for all 3 fields.
It seems simple but I can't find a way to create a "group" so if "field1 or field2 or field3" change, I get the same "group by" action. I just need to know how to get the expected action.
I expect that I might have some terms mixed up and/or some important information that would aide you in helping me. If that is the case please re-post and I will add any missing or mis-stated information.

Then group on the formula. If this is not what you want, could you post some sample data, with the desired results.

im using oravle application express. APEX
i would like to build a page with multiple forms linked to one table (orders) , the page has 4 from each one with different order_id number (depending on filtering), and if the order is prepared click yes for each order and this 'YES' should be UPDATED AND SAVED to each order number in the same table with the press of one button.
i created all the form as (sql query)
and create one update process

i dont know really if i can do that, but it appear hat it actually saving according to order_id number , but not all the time some time it saved the value as "null".

I am no Apex expert, but with a situation as "complex" as yours, have you thought about creating a VIEW that joins these 7/8 tables, placing an INSTEAD OF trigger on that view to do all the business logic in the database, and base your application on the view?

I'm fairly new to this, but I think it has to do with the way you have the drop downs named. Did you copy one then keep pasting it in each field? If so, that is the problem. You should rename each one with a different number: Dropdown1, Dropdown2, etc. I think that might solve the issue.

There are two scenarios where the purchase order is created with zero value.
1. Standard Purchase order with Free tick indicator.
Whenever we expect a material with free of cost from vendor, we use to put the free indicator in item overview. This means the material is valuated at zero price irrespective of price control in material master. Quanitiy is updated while receipts.
Conditions tab in item detail doesnt appear.
2. Consignment PO.
While creating a purchase order for consignment, the system doesnt ask us the price and the condition tab doesnt appear in item detail.
An info record for consignment must be maintained before making the GR.

The question came to mind is that if I am the father company of two child/sub companies and both these function under my company and the users of all the companies will have different SAP log in . But, I can I create a Main PRD client say 'PRD L1 and under this client I create PRD L2 and PRD L3.
Is that possible and seems logical?

Do you mean a form connected to a MasterData Object with multiple childs?
If so:
1. Create a new ScreenPainter form;
2. Add a Folder item for each of the childs;
3. Add a new matrix to each of the childs and change their PaneLevel to from 1 to N (N = number of childs you have);
If your code you then need to catch item pressed event for the Folder Items and change the form's Pane Level accordingly.

How to create list items with multiple attachment files using rest api javascript

In one of user form I am using javascript rest api to create a list item with multiple attachment files. So far I am able to create list item and once created uploading an attachment file. But this is two step process first create an item and then upload
a file.
It create an additional version of the item which is not desired. Also I am not able find a way to attach multiple files in a go. Following is the code I am using.

But this does not solve the core of the issue. You are uploading attachments after creation of item and multiple files are being attached in loop. This is kind of iterative update to an existing item with attachments. This will end up creating multiple versions.
What I am trying to achieve is to create an item along with multiple attachments in a go. No item updates further to attach a file.
Please suggest how this can be done in one go. SharePoint does it when one creates an item with multiple attachment.
