I'm looking for assistance with a form created; the end user will complete the form and then press the submit button to email it. Next I need to import that data to either a database or excel spreadsheet.

After creating your table, Tap the Tab marked "+" and select "Form" - you will be asked which table to use. (You should get into the habit of naming your tables - if the table name isn't visible, select the table and tap the Styles (Brush) Menu > Table > Turn ON table Name, then double tap on the Table to edit it.)
Select the table you wish to to fill with a form and you will see a new form based on the header data.
The form will allow you to add one row at a time and when you get to the last row, there is an option at the bottom of the form to add a new row using the "+" button. At the top of the form is the row header which you can rename by double tapping.
You can also rename the form on the Tab by double tapping on the name.
Try it out.

thanks, a HUGE suggestion for you: Please add that to the restrictions of the trade-in details online, it mentions that nowhere and is VERY deceptive. Enter parameters and then click on the option to fill out data

I have a form for entering a start date and end date then click Open report button. The use wants to add a default option on this form to pull out data based on a condition (i.e., only class 1, 2, and 3 to be pulled out
by default). If uncheck the option, the report will show ALL data (i.e., all classes). How can I do that? I think I should change the user requirement so that when he or she clicks on the button, class 1, 2, or 3 to be pulled (by default). And I
will create an option button to pull out all students on that report. Can you please show me which way is better and how to do it? Thank you in advance.

Thanks, Karl for your idea. But currently the form has nothing except the Start Date and End Date parameters and a button "open report". Once click, it will open the report with ALL classes.
Now the user wants to add an option control on the form and by default it will pull out records that meet the condition of class =1, class = 2 and class =3. Then after the user enters the dates parameters, and click
on "open report" button, since it's default, the report shows only mentioned data.
If the user uncheck the box, and click on the "open report" button, all classes will show up. How can I deal with this one? Thank you

First thing to try is to reset your device. Press and hold the Home and Sleep buttons simultaneously until the Apple logo appears. Let go of the buttons and let the device restart. See if that fixes your problem.
Personal Hotspot should be at Settings > Cellular Data > Personal Hotspot

Adobe has made this process much simpler in recent years.
No need to re-install any version earlier than the current version (CS6). All installers (upgrade or full) contain the full program code. The difference between full and upgrade is in the serial number.
All you need do is download (link below) and install CS6, enter the CS6 upgrade serial number then enter the CS5 serial number (even if that too is an upgrade).

If version 4 is an upgrade only, you will have to have the serial number of the 3.5 full version.
If you can't find it try contacting Apple with your proff of purchase, they may be able to help.

Is your iMac running okay? Where are you getting this update from? Especially if it is an e-mail, I would not do what the 'update' is suggesting.
Your profile shows you are running Mavericks. If everything is okay, then it would appear to me you don't need to download something to run Mavericks.

Yes, that's for network backups only.

You can tell if the File System (the various directories, catalogs etc.) is ok by repairing your backup drive, per #A5 in Time Machine - Troubleshooting.
You can "spot check" whether the Time Machine "Star Wars" display seems correct.
You can do a full system restore to another disk per #14 in Time Machine - Frequently Asked Questions, then start up from it and see if things are there and seem to work.
You can keep "secondary" backups (always a good idea). See #27 in the FAQ post for some suggestions.

When in the itunes program I can be moving around in my library and itunes
when choosing a song or songs, choosing file, edit, etc,. whatever command I
choose it just suddenly stops. I can issue five commands and watch each one
respond in 30 second to two minute intervals. I can issue several commands,
even shutting down, and it will do all but takes several minutes to do so.
My itunes program was working fine when this problem suddenly arose.

Click here and follow the instructions. You may need to completely remove and reinstall iTunes and all related components, or run the process multiple times; this won't normally affect its library, but that should be backed up anyway.

It does not sound like a battery problem.
Try a couple of things:
1. Reset the SMC

2. Reset (if you can) the PRAM
http://support.apple.com/kb/PH4405
If that does not help, delete the mouse in system preferences, turn it off, restart, and turn the mouse back on.

Welcome to the Apple community.
So long as you are logged into the iTunes Store, using the correct ID and password (settings >store), then you will always have access to your apps, books, music, TV shows etc etc bought via the iTunes Store on that account.
