I bought two apps through my iTunes in Ireland and gave the email address of my brother, who lives in Italy. He got an email to download the apps, but when he tried, it told him he couldn't because he's not in Ireland. There was absolutely nothing during the purchasing process that told me I could not buy apps for someone who is in another country. I thought iTunes was all one system until this happened.
I cannot find a way to request a refund or to place the order through Italian iTunes, so my brother can access the apps for my niece. What am I supposed to do?

You shouldn't be charged until its sent. Could happen, but I haven't ran across it. It will charge your card then the email. If the email however isn't "redeemed" you still are charged. So keep checking for that email. I'd check tomorrow on your online banking if you have it. If not wait until business day Monday. If nothing was charged try again.

If you bought an Apple gift card, then you cannot use it in the iTunes store. You can only use it in the Apple store for hardware.
You would need an iTunes gift card to buy anything in iTunes

No. You can redeem Gift cards directly in the Store Apps by tapping the "Redeem" button next to your Apple ID. Usually found at the bottom of the different tabs in the Store Apps.
You can set your payment method to none.
In fact that's what I do. I have no payment option set, and stick exclusively to gift cards.

iTunes and App Store gift cards are basically the same thing, both are redeemed onto your iTunes account and can be used to buy from the iTunes store, app store, Mac app store on Mac computers and the iBookstore - the cards that you shouldn't buy are Apple Store gift cards, they cannot be used with an iTunes account.
When you've redeemed it onto your account then your balance should show in both the iTunes and App Store apps on your iPad, and your computer's iTunes. On my iPad it shows at the bottom of the Featured tab in the App Store app under my account id and at the bottom of the Music tab in the iTunes store app (or you can view your account via Settings > iTunes & App Store and it should show on the screen that you are taken to). On my computer's iTunes it shows at the top left of the store's pages next to my id, and by logging in to view my account via the Store > View Account menu option.
If you are trying to download a paid-for item and you don't currently have a balance or other payment method on your account then you will be taken to the payment details screen so that you can either redeem a gift card or enter one of the other payment methods.
To redeem a card onto your account you can use the Redeem button at the bottom of the Featured tab in the App Store app, or the Music tab in the iTunes store app, on your iPad. Or you can do it via your computer's iTunes, either via the right-hand side of the iTunes store homepage

I bought a $20 App Store Gift Card from Australia and i can't do in-app purchases because my apple id was created in Sri Lanka.

This is what happened: I bought a $20 App store gift card so i can get some gems on my Clash of Clans Account, once i entered the code i went into Clash of Clans and tried purchasing $10 worth of gems. But it says to change into the Sri Lankan Store but when i went to enter the code it said to change the store to Australia. I tried doing it by creating a new Apple ID. Once i went to the billing information i didn't enter my credit card details but i entered my Gift Card Code, although it said it was invalid. I did see i had $20 Credit Balance in my Apple ID. PLS HELP! Reply as soon as possible.

All iTunes gift cards are country-specific, so an Australian iTunes gift card can only be redeemed and used in Australia on an Australian iTunes account. And whilst you are in Australia you can only use the Australian store, the store's terms of use include having to be in a country.
Also all apps (and only other content from the stores) are tied to the account that bought/downloaded it, so only the account that bought an app can do in-app purchases in it - so the new account that you've created won't be able to do in-app purchases in that game as you downloaded by a different account. Though you are being asked for card details it doesn't necessarily mean that the card will be charged for the purchase (unless you want to gift content). Have you tried entering your card's details to confirm the account ? You should then be able to remove it.

then you will need to enter card details (debit cards are no longer accepted in all countries) before the account can be used to download any item from the store.
