Quorum/Witness Disk Offline every 15 minutes error - Event ID 1069 - Event ID 1558

2 node active passive, 2008 failover cluster.  I keep getting the following error events:
The cluster service detected a problem with the witness resource. The witness resource will be failed over to another node within the cluster in an attempt to reestablish access to cluster configuration data. Event ID 1558
Cluster resource 'Cluster Disk 1' in clustered service or application 'Cluster Group' failed. Event ID 1069
However my quorum disk is online from what I can see.   One thing I did notice is, the Quorum disk no longer has any files in it. It is just a empty drive.
I also noticed these errors are being entered every 15 minutes exactly.
I monitored the Quorum disk and it is going offline momentarily every 15 minutes.
Experts Exchange Posts indicate this is known issue and waiting for microsoft to deliver SP2?  Really?

support.microsoft.com/kb/2750820 - This does not resolved your issue because it applies only to the Quorum model Node and File share majority.
The issue is because of the cluster configuration corruption and cluster I checking after every 15 minute and is seeing the cluster quorum disk in failed state (actually it is not failed) but is not able to update the current state of the quorum disk.
Check for below logs in the cluster logs:

Try reforming the cluster, it will resolve the issue.
Change the quorum model to Node majority and back to node and disk majority.
Stop cluster service on all the nodes and then start them back when it is stopped on all the nodes.

  • 2012 R2 - First cluster, having issues creating Quorum witness disk

    Its my first Server 2012 R2 Cluster and its based on a Dell VRTX server with shared storage.
    I've created the cluster and I'm now trying to setup the Quorum witness disk.
    In FCM Storage/Disks a 600mb disk is shown as online and I've called it Quorum.
    In Win Disk Manager this disk shows as offline and reserved.
    When I run the 'Configure Cluster Quorum Settings' wizard, pick 'Select the quorum witness', pick ' configure a disk witness' I get the error 
    No suitable disks exist in Available storage.
    What am I missing?
    Any help/advice/suggestions grateful appreciated :)

    ok, figured it out:-
    On Cluster 1 selected the Disk and put it in maintenance in Storage/Pools.
    On Server 2 - selected Disk 1 and formatted it as NTFS and called it Quorum.
    Then took the Disk 1 offline in Disk Management.
    In Cluster 1 brought the Disk back on-line.
    Ran the Quorum wizard and it works :)

    Even the Authenticated Users Group was already in the Users Group for th BuiltIn Folder in AD I have restarted the srmsvc and srmreports ...
    But still the error 12317 is poping up...

  • All Windows VM - Virtual Disk Service start/stop every minute

    Dear all
    I recently discovered that all my Windows VM's (2003/2008/2008R2) start and stop the virtual disk service:
    System Log File:
    08:37:27 Virtual Disk Service --> Service started
    08:37:27 Service Control Manager --> The Virtual Disk service entered the running state.
    08:37:28 Virtual Disk Service --> Service stopped
    08:37:28 Service Control Manager --> The Virtual Disk service entered the stopped state.
    and i have this entry every minute.
    I removed the DVD Drive from the VM but no change. What could be the reason for this? All my physical servers with the same backup program haven't this behavior.

    Just as a follow up:
    Dell know that problem and it will be "fixed" in Version 4.7.
    "In releases of the Host Integration Tools previous to V4.6,  Auto Snapshot Manager would query the entire system state every time a refresh was performed.  In Host
    Integration Tools V4.6 Auto Snapshot Manager will query the entire system state only during the initial load.  After that ASM uses event notifications to keep itself up to date refreshing only these changed items.   However event notifications
    aren’t enough,  to pick up some application changes Engineering implemented a polling mechanism.   The Virtual Disk Service events are a side effect of that polling and are normal and expected.
    However while these messages are entirely benign,  Engineering understands these message could be a concern as  they do fill up the event log and drown out important
    events.  Changes have been added to the V4.7 release of Auto Snapshot Manager giving the user the ability to control how often these refreshes occur along with the option of disabling automatic refreshes and using just a manual refresh button in the GUI."

    We have an 11.2.0.2 Grid Infrastructure listener that is configured with Oracle Restart. At this time, the listener is listening for connections to a 10.2.0.4 database. The following events are being logged every minute in the listener.log file. In the past, we have occasionally seen similar events logged in the 10.2.0.4 log file, but not every minute.

    I am wondering if the LsnrAgt is part of the Oracle Restart configuration and is it really just informational when it is occurring every minute?

    This error seems to be directly tied to the auto restart configuration of the listener.
    If you remove the listener from the config, the problem goes away.
    ie.
    srvctl remove listener -l listener
    Also, if you want to leave the configuration, you can modify the check interval to alleviate the problems.
    Setting it to 0, prevents the service_died errors. Of course this has the impact of listener not automatically restarting in the event it crashes.
    crsctl modify resource ora.LISTENER.lsnr -attr CHECK_INTERVAL=0

  • Event ID 7000 and 7009 every minute in event log then system crashes

    I am getting this error any a lot of machines. What happens is it will get tons of EVENT ID 7009 and 7000. This will continue until the system crashes completely. Below are some of the event log errors that happen. The 7009 and 7000 happen every minute. 
    EVEN ID 10005 DCOM got error "1053" attempting to start the service VSS with arguments "" in order to run the server:

    EVENT ID 10010 The server {1F87137D-0E7C-44D5-8C73-4EFFB68962F2} did not register with DCOM within the required timeout.

  • Apple mail will not recieve emails without delay. Going offline is not working. Connection Doctor is successfull. Mail check set to every minute.

    I am using Apple mail on my Imac. My emails are coming in but always late...the delay varies from 15 minutes to over an hour. 
    Im using imap...email hosting is via Godaddy. (imap.secureserver.net)  I would normally blame Godaddy, but I use the same account on my Iphone... yet their is no problems whatsoever. I set them up using the exact same information.
    - Mail check is set to every minute.
    - Connection Doctor successfully connects right away for both incoming and outgoing.
    - Closing the program and reopening does not work... Nor does going offline and checking for mail.
    - Check mail button does nothing...literally nothing.
    Currently using Yosemite 10.10.2
    Any help would be greatly appreciated!:)

    Delete the GoDaddy account from Internet Accounts (make sure to select Remove All)
    Locate the Cached folder, after you find the com.apple.mail folder, delete everything inside the cache (all folders and files)
    Restart your machine
    Re-add your email account from within Apple Mail: Mail - Accounts... - Add Other Account
    Fill out your account info fully,  re-launch Mail

    I'm seeing the following error in the console system log every minute: "Unexpected EOF, returning last token as fallback"
    Anybody know of a simple way to find out which calendar is causing this and how to fix it?

    Try quitting iCal and deleting the calendar cache.
    Choose Home from the Finder's Go menu.
    Open the Library folder. (If you are using OS X Lion, you will need to hold the Option key while clicking the Go menu in order to access the Library folder.)
    Open the Calendars folder.
    In the Finder, delete the Calendar Cache files, including any that also display a -1 (or -2, -3, and so forth) directly after their filename.
    Open iCal and wait for the calendars to rebuild.
    Then test iCal again. Let us know how it goes.

    Hi. We've had errors logged every few minutes for several years and would like to know how to stop them please? The log path and a file name example is:

    We stopped the built-in Jrun web server after installation years ago as it's an uncessary component in production. I don't understand why something is still requesting data from the Jrun server every minute.
    Setup: CF 8.01 x64 installed as multi-server instance on Windows 2008 R2 with IIS7.5.

  • ASM Disk offline - State hung

    Hey in my ASM configuration I setup this
    Disk Path ASM Name Failure Group
    /dev/raw/raw4 Data1 FG1_Data [on SAN1]
    /dev/raw/raw5 Data2 FG2_Data [on SAN2]
    /dev/raw/raw6 Reco1 FG1_Reco [on SAN1]
    /dev/raw/raw7 Reco2 FG2_Reco [on SAN2]
    all good.
    My I switched off SAN1. A query on v$asm_disk shows disk DATA1 & RECO1 as offline - hung state.
    I did in the asm instance in sqlplus:
    ALTER DISKGROUP DATA DROP DISK data1; -> success
    ALTER DISKGROUP RECO DROP DISK reco1; -> success
    ALTER DISKGROUP DATA ADD failgroup DATA3 '/dev/raw/raw4'; -> success
    ALTER DISKGROUP RECO ADD failgroup RECO3 '/dev/raw/raw6'; -> success
    The query on v$asm_disk stills show the DATA1 and RECO1 - but DATA3 and RECO3 as well.
    Is it right, that the "old" entries will disappear, once the ASM instance is restarted ? I can´t do this at the moment.
    In generell - why went the ASM disks to offline ?
    SAN1 was switched off for about 5 minutes - and was then switched back on.
    Is this expected behaviour ?

    Hi Chris,
    as long as the information is still partially valid, the devices will be displayed in v$asm_disk. Main reason is: Some Oracle processes still have handles open on these devices, and as long as they exist the devices can still be seen.
    A Restart of ASM + database will definitely remove these entries (if they cannot be seen by the operating system).
    Regarding your other question: Oracle can only detect if a device is going offline (errors in the database and asm alter log). If a device goes online again, Oracle does not check this (this would be a major overhead to rescan lets say every minute to see if a disk is back online).
    Hence you have to reinclude the dropped/offlined disks yourself.
    Depending on the version (10g or 11g) and depending on the diskgroup compatibility level for rdbms a disk can simply be returned into the diskgroup (11g - alter diskgroup <dg> set disk <disk> online) and resilvered or have to be reincluded into the ASM instance (no resilvering, but a complete rebalance 10g: alter diskgroup <dg> add disk <disk> to failgroup <fg> [force])

    I have the following finding from the Event Viewer.
    Event 7022
    The Forefront Identity Manager Synchronization Service service hung on starting.
    Event 7024
    The Forefront Identity Manager Synchronization Service service terminated with service-specific error %%-2146234334.
    Event 7031
    The SharePoint 2010 Timer service terminated unexpectedly.  It has done this 4007 time(s).  The following corrective action will be taken in 30000 milliseconds: Restart the service.
    Event 11016
    The machine-default permission settings do not grant Local Activation permission for the COM Server application with CLSID 
    {000C101C-0000-0000-C000-000000000046}
     and APPID 
    {000C101C-0000-0000-C000-000000000046}
     to the user NT AUTHORITY\NETWORK SERVICE SID (S-1-5-20) from address LocalHost (Using LRPC). This security permission can be modified using the Component Services administrative tool.

  • UPSA Errors, SQL Errors, Timeouts, Event ID 3, Event ID 3355

    Greetings,
    My User Profile Service has been running without issue for the last week or so; once I was able to provision it.
    Yesterday morning, we noticed it was in a "User Profile Sync is not currently provisioned" state.
    I went through the normal steps to start the service and after 15 minutes it stopped again.  The certifications are there, I saw the 2 Forefront services start, then I saw a number of errors being logged.  Then, of course, the sync service stopped.
    I am investigating the errors logged I see many Event ID 3; SQL timeout.  I am familiar with how to increase the Forefront timeout configuration but am wondering if the issue is something deeper.
    This is a small 3 server farm; 01 - Application Server (Central Administration), 02 - Dedicated Search Server, 03 - WFE.
    The UPSA and sync is configured to run on 03.  However, when I look at failed timer jobs, I see the "User Profile Service - User Profile Incremental Synchronization" job failing on both servers 01 and 03.  These jobs are running every
    minute and have thousands of failures.
    Why would this job be running on 01?  I have never scheduled it to run on the 01 server?
    I'm not sure if this is related, but I also see intermittent Event ID 3355 in the logs.  This event indicates the following:
    "Cannot connect to SQL Server. {Our SQL Server} not found. Additional error information from SQL Server is included below.
    Timeout expired.  The timeout period elapsed prior to completion of the operation or the server is not responding.
    Table 'TimerJobHistory'. Scan count 1, logical reads 4, physical reads 0, read-ahead reads 0, lob logical reads 0, lob physical reads 0, lob read-ahead reads 0."

    I recommend to check if the User Profile Synchronization Service has been started in both 01 server and 03 server.
    If yes, then User Profile Service - User Profile Incremental Synchronization timer job will run on both 01 server and 03 server.
    For this issue, I recommend to reset the user profile synchronization and reboot the 01 and 03 sever after the reset, add the Farm account on Sync DB as dbo owner on SQL server and then start the profile synchronization server to check the results.

    As you said that the User Profile Service - User Profile Incremental Synchronization is failing in 01 and 03 server, what was the error message displayed when viewing the failing timer job in Job History?
    I also recommend to check ULS log to see if anything unexpected occurred.

    Hi Victoria,
    I did resolve the issue with the UPSA Synchronization Service failing to run; it was a SQL Server error.  SQL Server was actually running out of memory.  My client added an additional 12GB memory and these specific errors went away.
    What I'm still questioning is; why is a synchronization timer job even trying to run on the 01 server.  The UPSA synchronization job was provisioned on the 03 server only and it is running now.  However, I still see the UPSA synchronization timer
    job failing on the 01 server.
    This is a production environment.  Tearing the UPSA down and rebuilding it is not an option.  I just need to get the UPSA synchronization timer job to stop running on the 01 server.

  • SQL Server log generating minidump files every minute

    Good afternoon.
    Our SQL server recently started generating minidump files almost every minute.
    I'm waiting on the final dbcc checkdb of the final database but all others have returned without error.
    I've run the dump files through the debugging utility and have the following stack dump which I'm not sure how to interpret.
    At this point, I'm not quite sure how to proceed, pending on the results from dbcc.
    There are no other obvious errors in the event or system logs to cross-reference.

    I upgraded to Windows 8 on 11/18 and started having problems where explorer.exe is restarting every minute or so. Here is the error from the event log:

    I worked with Microsoft Tech Support for 4 days and ended up doing a clean install of Windows 8.  I had been told by Microsoft that they would escalate me to Tier 2, but they called back and said it was an HP/Hardware issue and I would have to
