Acrobat 8.1.3/4 crash opening form

Question

I have a form Acrobat created using a scanned image with some fields added. This was done using Acrobat 8 (.?) under Win XP SP3. When I tried to open it with Acrobat 8.1.3, I had a "sorry for the inconvienince". I found Acrobat wouldn't update to 8.1.4 and that other documents opened fine. At this point, I removed Acrobat 8.1.3 and installed Acrobat 8 from the CD.
I could open the pdf fine. I checked for updates and was updated to 8.1.3 with no problems. At this point Acrobat crashed when I opened the problematic PDF. It updated to 8.1.4 this time but continues to crash on this one PDF.
I extracted the following text from the MS error reporting dialog.
AppName: acrobat.exe     AppVer: 8.1.0.137     ModName: unknown
ModVer: 0.0.0.0     Offset: 001800ba

It's almost 3 months later and I still have the same problem (with Acrobat 8.1.6 Pro).
I've removed, reinstalled Acrobat Pro and upgraded it countless times. In each case, the initial 8.0 installation from the CD works fine. Subsequently, the first upgrade offered is 8.1.3 which dies when opening the PDF with the form. The subsequent upgrades all do the same.
I've tried disabling programs started at boot (except ones needed for hardware on the system) and I've stopped services too with no change. I've disabled Norton Internet Security which had no affect on the problem.
This is pretty frustrating.
Any ideas?
