Apple Care & ATT insurance doesn't cover shattered phone - why?

Purchased iPhone 4S + Apple Care $99 + AT&T insurance but when I shattered the phone into 8 pieces after 11 months (hit concrete on top corner with Speck case on), Apple said this is not covered and AT&T wants $200 for a refurbished replacement.  What should I have done better to get a replacement for a more reasonable amount?  I'd like to know for the next phone I'll upgrade to in May 2013 when I am eligible.  In meantime, am using 3G with VERY limited ability.

AppleCare has never covered user caused damage - it provides
protection against manufacturing defects. I do not know the provisions
of AT&T insurance so I cannot comment, but you should have been
given documentation when you signed up for the AT&T insurance or
find it on the AT&T website.
Do you have AppleCare or AppleCare+? Provisions are different and
details are at Apple.com
$199 US is the price for out of warranty replacement on an iPhone 4S.
Check with Speck to see if they provide any assistance/coverage for
using their case.

    As a apple customer I feel that is very important that we are  offered some type of insurance plan to cover our devices if it is stolen besides the AppleCare which is offered that only cover software issues.     Well...   Usually the deductible is high enough to make it barely worth the claim.  Once you claim you may also find your insurance rates go up for several years.  The rule of thumb is don't claim unless you're talking at least $1000.  In that regard it could be useful if somebody offered separate insurance, though I suspect one might wonder why one was paying $50 or whatever per year for insurance on a single item versus just paying a bit more for a lower deductible on the homeowner's that would cover a host of items.
    Qbangz:  I have almost always had renter's insurance or the equivalent, even when I stayed in a dorm at college.  If somebody decides not to have it then that's a personal choice, but it is available.

    No. The AppleCare extended warranty only doubles to time to a total of two years and extends the time of free telephone support. It still excludes accidental damage like what cases a cracked screen.

Then they told me that my iphone problem considered in a "physical damage case" which they have a policy for claiming a new iphone and pay some fee. After I got my new iphone, they said that all the information and phone data which is registered into apple system about the phone and the user and bla bla bla will transfer to my new iphone. Today, I just check my phone serial number in "support & coverage " column and I found that all the phone data in apple system is already transfered from the old one to the new one EXCEPT the data of apple care that I already registered before I claimed the new phone. Now, the question is " What can I do ? " and " will it be transferd to my new iphone or I have to buy a new apple care for my NEW PHONE That I got from their service "

    Placing your iPhone in the toilet was the correct thing to do but you should have purchased an android phone instead. That would have solved these issues. :-)

    Go to Settings>General>About.  Scroll down a bit then copy your serial number.
    Open Safari and navigate to selfsolve.apple.com.  Paste the serial number into that field and tap Return. 
    This should tell you your waranty status.

    Have you tried to power off, then take the battery and SIM out & wait a few seconds, then re-insert battery/sim and power on? Sometimes a complete turn-off might assist.

    I would think that at the very LEAST you would be eligible for a COMPLETE refund of ALL charges for insurance paid thru Verizon!  They were charging you for something you were ineligible for!  Verizon has a record of whether or not you purchased the phone from them - otherwise how could they deny the claim!
    It very sad to see how customer service and customer satisfaction has been lost here ... !

    I believe AppleCare+ is only available for the iPhone 4S. Give Apple
    a call to be sure I am not mistaken. AppleCare+ was released with
    iPhone 4S. Even if AppleCare+ is available for iPhone 4, you are
    past the 30 days from time of iPhone purchase to be eligible.

    No. Cosmetic damage is not covered.

    You now have to purchase Apple Care+ at the time you purchase the phone.  Does anyone know if this will be something you can just click on to buy when you go to preorder the Iphone 4S on Friday?

    I had the same problem, and there are many conflicting responses from various sources.
    I actually called verizon customer service to ask about this and I was told they did not offer it at this time.
    The person I spoke with had no idea what applecare+ was, and I had to explain to her the difference between it and the standard applecare (which does not cover the 4S).
    If you search for the issue on apple's community boards, there are all sorts of responses. The most common response is that we are supposed to bring our boxed, unactivated phone in to a brick and mortar apple store and they will allow us to purchase applecare+ then.
    However, I have been to two brick and mortar apple stores (Honolulu, HI), and spoke with the managers for each store, and they did not even know that we are not able to purchase applecare+ through carrier website pre-orders. One of them told me to cancel my order and preorder through them (which actually will reset your data package to one of their limited plans, even if you are supposed to be able to "grandfather in" your unlimited data plan). Another told me to bring my phone in on the release date and return and re-buy the phone.
    Luckily, one of the malls I went to has an apple store and a verizon kiosk. I went over to verizon, and again, the salesperson had no idea that applecare+ was not available on the website. Another salesperson jumped in and said that on October 12th a toll-free number would be released for all pre-ordered 4S phones purchased through carrier websites, which would allow us to buy applecare+.
    I guess we'll just have to wait and see...
