Autopopulate PDF Form Text boxes

I would appreciated any help on this matter as my IT department is unfortunately of little use.
I am trying to gather information from text boxes in a webpage dialogue inside of a EMR (electronic medical record) system to prefill out certain textboxes in fillable form I created.  I do not now how to reveal the source code from the page in order to get help creating the code/script.  I am a complete novice at javascript but have used the forum to create various arrays to autopopulate forms.  Here is a screenshot of what I am trying to accomplish.
I would like to take the patient account number and name from the "1. Choose a patient"" boxes and prefill in the form with the patient name and account # text boxes in the form.  We have to fill out a million forms a day and it would BE SO GREAT if we didn't have to type in the info that is literally 2 inches above it!
I do not know if this is even possible or I have to have some special privileges via the EMR company, but when I mentioned it to my IT department I got the deer in headlights look.

Same here. The boxes are shifted, to varying degrees, further down than in Acrobat Reader. This is a shame because I love Preview.

  • Preview: PDF Form Text Boxes Shift

    I am putting together a PDF form using Adobe Acrobat Pro and testing it in Preview.
    In Preview, the check boxes and radio buttons work fine, but when I click on or tab to a text box, the text box shifts downward and becomes somewhat wider than the box I drew. Multi-line text boxes become wider and shift upward.
    Why does this happen? Is there a way to keep the text boxes in place?

  • Font size in PDF form (text boxes)

    Hi!
    I have an application form as a PDF which I have to print and send back. But the text isn't fitting into the text box, so I get a scroll bar. Now I either have to change the font size or change the size of the text box. But I couldn't find out how to, neither in Reader 8 nor in 9.
    Another Option would be to export it as a jpg or something. Can I do that somehow?

    Yes, the form designer can change the font size. So can you if
    (a) you buy the same software - Acrobat Pro, and
    (b) the form is not protected - locked - by the form designer.

    Sounds like you have a stroke on it. Select the frame and set the stroke to none.

  • Can I shrink the size of a form text box based on the content?

    We are helping a customer migrate from some MS Word forms to use pdf forms.  We are screenscraping the data from an AS400 green screen system and populating these new pdf forms with data.  however some of the text boxes are in the middle of a sentence and the text box needs to be large enough to fit 50 characters but there are many times when the data we are entering is much less than 50.  The resulting document looks very strange with all of that extra white space in the middle of a sentence due to the size of the text box.
    So can we somehow shrink the size of the text box to fit the number of characters in the box?
    I considered making the box smaller and setting the font size to auto but I think that would look even more strange to have a word with smaller font in the middle of a sentence.

    Sorry, I should have mentioned I am using acrobat professional 9.  I see that I can change the size of the text field when I am designing the form but what I don't see is how to dynamically change the size of the text field based on what data is put inside it.

    I am a novice to Adobe PDF form creator and I need some assistance. I am creating the forms and I have all my boxes set, but the issue is that I can not find where to space the text boxes from the top line to the bottom. (As we would do with the enter/return key) I have the full letter page to fill but when I print the form it's on a 1/4 of a page and I need all the text to fill the whole page. I don't necessary want to enlarge the boxes to fill the space.
    Where can I create extra spacing between each top and bottom lines?

  • How to make form text box field display name or instructions?

    Using form tools, I have added text boxes in my PDF that I would like to have the field name display. So the user would understand what kind of info they need to fill in. For example, [Type organization name here] in one box and [type address] in another. This is not an actual form, so I don't want to put those labels outside the field as shown on the adobe sample form.

    You can add your text to the 'tool tip' for the field's properties, use the right mouse click.

    I want to give my clients some instructions to type in an account number that is taken from the pdf, submitted to my web server in php or asp, and generates the information into a text box field...

    I cannot use the typical regular HTML "a href",
    because I have to call a struts action.No... you said you have to submit the form with a link. A link is an "a href", it's exactly what <html:link> generates. But a link cannot be used for form submitting except by invoking Javascript to do it programatically.
    Using an action in <html:link>, presuming that's really refering to an action and not just the href value, which is what I'm not really sure of. It's been a while since I've used Struts. So maybe the action is the href part, and you can put "javascript:void(0);" there?
    But I'm absolutely positive about the HTML/Javascript aspect. Struts tags don't alter how HTML works. They just make generating some HTML content easier. But in this case, I'm not sure that's helpful because I never found a simple way to get the actual form name that the <html:form> creates.

    When I fill out a fill-able text box in a pdf form, the information for a number of boxes is longer than the space shown for the answer.  I am trying to find a way to adjust the text size with no luck, too. 
    How can I make sure the text provided in each text box will show when I save / print the fill-able form?

  • PDF form text fields' options change when opened in READER on a MAC.

    I created a PDF form using Adobe X PRO. All fields are UNCHECKED for Property/Options/ "scroll long text". I save and distribute the form as a "reader extended pdf/enable additional features" so it can be saved and returned using READER.
    When a Mac user opens the extended form in Adobe READER, ALL the text fields change to scroll. When he returns the completed form to me, every text field, even signature and date fields, are changed to scroll text.
    What is happening in the transition from PC to MAC?

    Preview is the default PDF viewer on the Mac. It should never be used with PDF forms. Just let them know that they should use Adobe Reader. Provide a link or URL where it can be downloaded and let them know that it's free.
    There are things you can do so that it doesn't work in Preview, but it involves some scripting. If interested in this approach, post again.

  • No text in form text boxes

    I have been sending out forms to be filled out in Adobe. So far, most of them come back with no issues. But recently, I have started to get them back where the text in the text boxes doesn't show up unless I click in the box. The text doesn't print when the form is printed, so that is a problem. I did some digging and it seems that the problem stems from people filling out the form in Preview mode as opposed to the regular Reader mode. So my question is two-fold.
    1. Is there a way to make it so that the form cannot be filled out in Preview mode, but will prompt the user to open it up in Reader?
    2. How can I fix the forms I already have? I read something about importing and exporting the form data, but I cannot see where to do that.
    I am using Adobe X Pro on a Mac, and I am NOT a power user, so please be very detailed in your answers about where I find commands.

    I figured that much out.
    My question was how do I prevent these problems? I saw another discussion (over a year old) that said something about importing and exporting the data, and another answer that mentioned turning off the ability to fill out the forms in Preview.
    So my questions were how to actually perform those "fixes" since I can't seem to find any sort of 'import/export/ command in Adobe Pro X nor can I find anything about locking the form from being filled out in Preview mode.

    I am attempting to write JavaScript for an Adobe Acrobat X Pro form. I would like to set an action script for a List Box titled "customer_name" so that when a specific customer name is selected, a corresponding text box titled "cutomer_number" will automatically fill with the correct number. I am not very familiar with JavaScript so any help would be greatly appreciated.

    The easiest way to do this is to set the export value of each list box item to the corresponding customer number. The script for the custom number field could then simply be:
    // Set this field's value to the export value of the selected item in the list box
    event.value = getField("customer_name").valueAsString;
    Post again if you don't want to set up the list box that way for some reason since there are other ways to deal with this.
