Bank cookie not recognized

Question

Two days ago, out of the blue, when I tried to log into my bank website I got a message that my computer was not registered with the site and i needed to be send an access code.  Same computer for years--nothing has changed and I'm at home.  Using Safari 6.0.2 in OS 10.7.5.  Not new.  The cookies were configured correctly.   Got an access code and checked to activate my computer for later use and then when I tried again later I was asked to register again.  Since 12/11/12 I have been unable to log into my bank's website unless I get an access code EVERY TIME I LOG IN.  This happens on Safari and Firefox.  Talked to customer support at the bank and went through all the settings, added in the bank web address into custom settings in Firefox to always allow access and that did not work.  I went onto my husband's iMac here, which only has system 10.5.8, and was able to get in repeatedly so I was told by the bank internet customer support person that the issue is something on my computer.  The problem is that I can't find anything on my computer to change that will affect this issue and don't know what changed from 10/10/12 to 10/11/12 when this started.   Right now I have to either use my iPad or my husband's computer to access the bank.  If anyone knows of a setting that would affect this, please let me know.  I was sure this as on the bank's end but was told that nothing on their end changed.  I have not had problems logging into any other websites so far except my bank.  I don't understand this.

This is a long shot because I don't use it, but are your log-in credentials for the bank stored in Keychain Access? Your iPad and your husband's Mac wouldn't be using it and logging in from Chrome is new, so could what's in Keychain have gotten scrambled? Also, I wouldn't bet the farm on what the bank internet customer support person told you; I doubt he'd know what the programmers were doing under the hood and it's always easier to blame the customer's equipment. Finally, do you have any security or privacy related extension in both browsers which might have gotten silently updated together? I was initially thinking NoScript or AdBlock Plus because I find they often are blocking something that didn't matter yesterday but suddenly does today, but they're not available for Safari.
