Anyone know if I buy a mobile phone in USA (Boston) if it works normally in Portugal? what should I care while purchasing? Should I do anything in particular?

You should also beware that unlike other Apple products, the iPhone warranty is NOT an international warranty, so you would have to return a USA phone to the USA for warranty service.

You must purchase an unlocked GSM iPhone 4 which Apple just began selling over the last week or two (a CDMA iPhone 4 is also sold in the U.S.). Not sure if the unlocked GSM iPhone 4 is sold at Apple retail stores or just via Apple's online store. If an exchange under warranty is required for such an iPhone, you may have to return to the U.S. for the exchange under warranty.

If you buy an Unlocked iPhone from the US, you can use that phone on any carrier that supports that particular phone (either GSM or CDMA). So yes, this is possible. But if anything happens to the phone that requires Apple's help, you will have to ship it back to US for that service.

It is advisable to purchase the phone in the country you intend to use it.
1) You will have Warranty...
2) your phone will work as expected.
3) Unless Purchased Directly from Apple as an UNLOCKED device, the iPhones will be Locked to a US Carrier.. usually for a 2 year contract period.

It is Prudent to purchase the Phone in the Country it is intended for use in... it will then work as expected.
If you are attracted by the seemingly Cheap prices in the US... you are failing to take into account that you are probably looking at the phones that are LOCKED and CONTRACTED to a US Carrier.
AND...
iPhone Warranty > The warranty is only valid in the country of original purchase... Except for EU Countries..

So the US Apple store appears to operate differently from the UK store (where I've just bought 2 new iPhones unlocked and taken SIM only contracts with 2 different service providers). I'm sorry if I've caused any confusion.
To be clear, you need to find somewhere you can get an unlocked phone to be able to bring it back to Europe and use it.

The problem is that: First, I don´t know if that is possible to be bought by Apple store and be sent directly to my country. Second, and most important thing, is that I have read that to activate the iPhone, it is needed the social security number and I am not a US resident. I would wait until the European model comes out, I know it is tough to wait. But it would be best, you live in Spain, the majority of your time would be there. If you buy the iPhone in USA version and set it up in America, you will pay roaming charges the whole time you are in Spain or thereabouts. Once it hits 50% of your total time on the phone, AT&T will drop your account. Please consider waiting, it is not that much longer, we had to wait a few months too.

Read what Peter said, he is from the UK. Also, if you send her money to buy one in the US, she would have one that would not support everything in the UK. Also the warranty is only good in the US, so she would have to bring it back to the US if she experienced a problem with the device. The best choice is to purchase the phone in the UK, but for it to work in the US with a US SIM, it would need to be unlocked.
