I am currently living in Malaysia.
I was originally using the Singaporean iTunes Store, and I have many apps purchased from that store.
My question is: can I use my Malaysian Visa with the Singaporean iTunes Store?
I previously used a Singaporean Visa credit card for purchases.
is it possible to use a Malaysian Visa card for purchases as well?

After you set up your family, any time a family member initiates a new purchase it will be billed directly to your account unless that family member has gift or store credit. As the family organizer, any receipts generated by the transaction will be sent to you. Learn more about how iTunes Store purchases are billed.
Once paid for, a purchased item is added to the account of the family member who initiated the purchase. In other words, the content belongs to them, much as it would if you were buying them a book or a DVD. If Family Sharing is ever disabled, each member will keep the purchases they initiated, even though they were paid for by the family organizer.
These are the only current options for payment within Family Sharing.

How can I use a credit card and NOT my iTunes Gift card balance?

I have an iTunes gift card balance of $25 already on my account.
American Express has offered me a $5 iTunes credit with a $5 purchase if I use my Amex Card.
When I make an iTunes purchase will I have the option to choose to use the credit card instead of the existing gift card balance?

It worked, I gifted myself a $10 iTunes Gift and it came form the credit card so it only cost $5.

Part of the validation for cards is that they are registered to the same name and address as on your iTunes account, prepaid cards tend not to be so won't be accepted.
What you owe isn't shown on the payment details screen of your account ? If iTunes gift cards are available in your country then you should be able to redeem them onto you account so that you can your debt, after which you should get the 'none' option so that you can remove the prepaid card.

On your computer's iTunes you should be able to edit your payment info by going into the Store > View Account menu option and logging into your account, and on your account's details page there should be a payment link. If you are doing it on your phone then tap on your id in Settings > iTunes & App Store and tap 'View Apple ID' on the popup - that should also give you a payments link on your account's page.

Get an iTunes gift card & redeem that in iTunes using your existing Apple ID.
Then the option for none, regarding a credit card should appear.
