Can't get inserts to work

Question

Im trying to insert values into a table and keep coming up with errors not sure how to fix the error. I wrote a procedure which complies correctly but does not execute properly.
any help would be great below is the code:

This code does not seem to work either.l What could be the problem with this revision.
