Create a Registration Form on Website?

Please help ...
How do I create a form that can be submitted to me through my website, rather than it needing to be emailed as an attachment?  As it stands now, once someone clicks the "Submit" button a pop up appears asking them if they want to use Outlook or another email.  I want it similar to a college application, where once "Submit" is clicked, that's it.  All the information is sent in.  Thanks!

    I shall make a suggestion to renew the registration for a education centre.
    Some time ago I heard of a possibility to create forms in Acrobat... that worked, so I have the form.
    But how is it possible to send the filled pdf-form to an website (myqsl for example) and create an automatic chart, which collects the values from the sent forms, so that I can print the whole chart (or export it to Excel)?

    You do not send the form, but the form data (FDF or XML). The FDF toolkit allows you to manipulate FDF files on the server end. You will need to parse and send the data to your database.

    I have created a registration form, where on the first page
    the user enters a user name and password, then they go to the next
    page where they enter their name. This then goes to the next page
    where they enter their address, then there contact details.
    But I want all these details to go on to a page where they
    can review their details, then press a submit button which inserts
    the data into a mysql db.
    But I need help with how I do this?

    should it be in the header? Or on another form? to insert their

    Your code is correct only for the first page. What you need
    to do after the
    initial record is entered is grab its ID and then on
    subsequent pages you
    would use and UPDATE sql statement

    Hi, i have two tables: pet_owner and pet...trying to make a pet registration form....owner might have more than one pet.
    ho do i create a form with the owner's details at the top, then a drop down, how many pet would you like to register? [1][2][3] and depending on the selection made, 1, 2 or 3 sets of input fields appear below?

  • Registration form in apex

    Hi,
    I have created a registration form for a sample application, and it's all working great, except for i dont know how to use the inputted data to add the new user into the
    security system of the application. I'm using the standard apex authentication scheme. But it's just not clear in which table i have to insert the username and login to let them be
    able to log in.
    Any help would be much appreciate.

Use the <tt>apex_util.create_user</tt> API and associated procedures.

    If you created it in Forms Central, you have to edit it there. I believe Forms Central is similar to LiveCycle Designer in that the form created is no longer able to be edited in Acrobat. I might be wrong, but that is my understanding. You add the submit button in Forms Central. Within Acrobat, you should be able to go to the forms menu and Manage Data to save the data to an Excel file. Others better with forms should be by to clarify things, but this should get you started. In the future you might find it better to post a forms question in the forms discussions.

    Hi, I would like some help on a way to create a family registration form for a church, where the user fills our basic information about the family, where some is required and then they fill out information per member of the family with some of those as required fields. How would I go about doing this with a max number of members as 6. How would I make it where they could say pick how many members they have and it would add a form for that many members.

  • Desperate for help: ADDT registration form

    New to ADTT and back again with another, more urgent problem.
    I have used ADDT to create a registration page, using UPDATE instead of INSERT (this is necessary because the users are already in our system as contacts).  To accomplish this I had to do some hand coding and I fear I may have gone awry.  Initially I kept getting a blank page with the message "error: internal server" message when the page loaded...currently I get "Fatal error: Call to a member function addField() on a non-object in /home/agciorg/public_html/events/2009/259/users_Registration.php on line 33."  Now I can no longer open the registration page in Dreamweaver -- freezes DW everytime.  So I'm trying to edit in Text Edit to get it back to a functioning page.  I've already rebuilt the page from scratch twice trying to get things to work and I'm on a major time crunch and just can't afford to do it again.  Below is the current code (I've also attached the php file as a .txt)...any help in resolving my issue would be much, MUCH appreciated!

    Actually if you do not need a public registration form, its much more practical to use the "Create Dynamic List" and "Create Dynamic Form" behaviors in your site administration back end. And it really only takes about two minutes to set up.
    This will give you the ability to add, delete and edit all your users.

  • Create online PDF forms with database backend

    I created a PDF form using LiveCycle. I want to put this form on my website so that when my users fill it out, the data goes into an online database. At the end of each month, I would like to be able to export this database as CSV for reporting purposes. How do I go about in setting this up. Please help, I have been searching this forum for over an hour now and I am not able to find the solution.

    You would need to first create the form ( or Widget in EchoSign) first to generate a url which you can paste it in the email being sent to customers. You would need to make sure you have a Document Cloud Enterprise Premium account in order to create a Widget. If you don't have the account, you can use the trial version to test it.   • Event registration form

    Is there a way that I can create an event registration form were users log in and post an event through a registration form then use a seperate page to search those events for results. They should be displayed in a table structure with submit and search buttons and multiple registration and search fields.

    That's a Swing tutorial. The idea is you learn Swing, and then use the things you learned about to create the application you want. There is nothing special about a feedback form, it just uses elements common to almost any GUI app.

  • Firefox w/ not open forms from websites. Internet Explorer had no problem. Re-installed FF with no resolution.

    Firefox will not open forms (i.e. registration forms, class schedule, flyer, etc.) on certain websites. Updated Adobe Reader and Re-installed and updated FF. Still will not open the forms. Tried Internet Explorer with no problem at all. Tried FF on a different computer w/ no problem. Help please?

    I also have this problem. I cannot open links from a discussion board I frequent nor can I open links from my email. Also after opening my email, I cannot go to any other sites except my home page. I can then go no where else.
    When I close Firefox and try to reopen it, it does nothing! I have to bring up the Task Manager and end the Firefox process before I can get back in.
    To the best of my knowledge, this problem started with the new update to Ver. 3.6.9. I am running Windows XP Media Center 2005

  • Self Registration form

    HI,
    I have created a self registration form based on stored procedure that executes packages owned by Portal30_sso.
    When I ran the form, logged as another user, the procedure does not terminates correctley.
    This user, owner of the form, has execute priviliges on all packages that are used in the procedure.
    Has anyone made a self registration form?
