Creating a fillable PDF form, check box entries do not print

Hello.
I am creating a PDF in Acrobat Pro for Adobe Reader users to enter information, with an option to print after the form is completed. I've added check box fields for users to indicate various options. However, when checked, these fields do not print.
In the properties dialog, the field is visible - so theoretically it should print. All other form fields added to the document print, except for check boxes.
Could this be a font issue? I notice that Adobe Pi font is used for check boxes, however, Zapf Dingbats is being substituted (I can see this in the Document Properties/Fonts dialog).

I have used adobe products quite a bit, but this is my first time creating a PDF Fillable Form that will be distributed for use by several people so I am a little lost.
Also, I seem to be running into one more problem that I am hoping you can help me figure out. On the form there is a question for the recipient  to answer by checking one of two boxes, yes or no. Depending on the answer an action is initiated. This feature works perfectly, however the issue is that when the recipient choses "yes" he/she is directed to fill out four additional fields that I currently have set as "required", but when the recipient choses "No" the four additional fields are hidden yet still required. This becomes an issue when the recipient has completed the form and tries to submit the form. An error message pops up notifying the user there are required fields that are empty and must be completed before submission.
My thought is (in a perfect world) one of these two options would be ideal:
1. When the "No" option is chosen the four additional fields are changed from required to not required, or
2. When the "No" option is chose the four additional required fields are autopopulated with NA.
Either remedy would work just fine, however I am open to suggestions... Again thank you so much for your help with my previous question and thank you in advance for any information/help/advice you can provide in regards to my second question.

    Re: Help Icon - The PDF does not display the "?" like the web form.  In the PDF when you mouse over the field and hover for 1-2 seconds the help text is shown in a tooltip.
    Re: Form Reset button (Clear Form) - FormsCentral does not provide a clear form button but you can add it in Acrobat.  You will want to open the PDF in Acrobat and then follow these two tutorials:

  • When creating a fillable PDF form can I add a password to only selected fields?

    I am creating a fillable form which will require data entry from several different people before being returned to my office. There are certain fields on the form that need to be restricted to personnel in my office only. I have tried to accomplish this restriction many different ways but have been unsuccessful.
    The closest I have come to accomplishing this restriction is by adding a "For HRO Only" button which I can use to hide certain fields from personnel outside of my office and unhide the text field when the button is clicked. This would be perfect if I could add a password to the button. Unfortunately this is either not possible or I haven't figured out how to do it which means that personnel outside of my office can still access the fields with a click of a button.
    The only other way I have thought of to accomplish this restriction is to mark those fields as "read only" but it seems as though it would be very time consuming for personnel in my office to complete the form due to the fact that they would have to open "edit form",  open the properties for each field requiring their input, and uncheck "read only" for each one.
    Any help/options/ideas you can provide will be greatly appreciate. Thank you!

  • Need help creating a fillable PDF form from a Word Doc coverted to PDF

    Hello, I recently switched from the downloaded program to the monthly "subscription type" of adobe "cloud".  Previously, I was able to select in "Tools" the option to insert blue text boxes in my PDF where customers could fill in  info and return to me.  Now, I can no longer find this option on the Adobe Reader XI. Am I missing something?

    You're using Reader instead of Acrobat... Only Acrobat can edit PDF files.

  • How to save fillable pdf form *without* the blue highlight?

    Created a fillable pdf form.
    Clicked "remove highlight" and it went away.
    Saved file as extended other so I can fill in and save when opened.
    Open it and it still has all fields highlighted in the blue. How to I change this to be transparent?

    Thanks for the respons... if that is the case how come when I open fillable pdf's in AA from other people there is no highlight. But when I open the one I just created, there is highlight. That's all on the same computer with the same preferences?

    Yes, double-clicking a document that has been inserted as an OLE object does not invoke the application associated with it in InfoView.
    When you insert a Word file as an OLE Object in the CR designer, you can open the doc in Word and edit it without having to re-import the doc. So, this is almost like editing the CR report!
    Well, in InfoView as we know, a CR report cannot be editied so I feel the fact that it does not open the pdf in Adobe on double-clicking could be because the report cannot be edited per se.
    Now, of couse, a pdf doc cannot be edited in Adobe reader too, so the pdf file opening in Adobe on double-clcking in CR designer could be just 'By design'.

    I have created a fillable PDF form in Acrobat 9, which opens fine as a fillable PDF from my desktop.  I also uploaded it to our website but when i open it up from the web; it is no longer fillable.  Can you tell me how to fix this?

    That really depends on how a user (client) machine is set up, as opposed to something you can do on the web server. For example, the user can configure Acrobat/Reader so that it does not open in their browser, but rather directly in Acrobat/Reader. Or they can set things up so that PDFs are always downloaded and not automatically opened in Acrobat/Reader. You could provide instructions for downloading the PDF on the page that has the link to your form.

    There are suggestions in the Help documentation and on this site that indicate it is possible to attach files to a fillable PDF form that is being submitted. But I do not see any instructions for adding this functionality to a form. I am using Acrobat 9 Pro to create my form. I have a Submit button with an Action/Menu Item/Submit a form option, and the URL link for the submission is a local coldfusion file with a #FDF suffix. The export format is HTML. The form also contains a hidden EMAILTO field, with an Options/DefaultValue option identifying the recipient's email address. (I'm guessing the local cfm file uses this field.) There is also a hidden URL field in the form, and its Options/DefaultValue specifies the URL of the fillable form itself. (Perhaps the local cfm file uses this field too.)
    How can I add functionality to this form, using Adobe/PDF, to allow the user who submits the form to attach a file to the submission?

    You have to first configure a text field so it can be used for file selection. You do this on the Options tab of the field properties dialog. In order to select the corresponding check box, make sure only the "Scroll long text" option is selected.
    You then can set up a button that has the following JavaScript attached to the Mouse Up event:
    // Mouse Up script for button
    getField("upload_text_field").browseForFileToSubmit();
    Replace "upload_text_field" with the actual name of the file selection text field.

    PDFpen pro can be used on the Mac to generate fillable forms.  It is much less expensive than Acrobat.
    For IOS, I would check out
    PDF Expert by Readdle
    DocuSign Ink
    I know the allow you to sign your name on PDF forms.
    I would think they can extend that to fillable forms.

    I can create fillable PDF Forms using Adobe Acrobat X Pro. I can email them to someone using Adobe Reader (free) and they can fill them, save and email to me. But when I upload the form to Dreamweaver CS5, the form is not fillable. And yes, I saved it as a Reader Extended PDF-Enable Additional Features.

    I see no reason for this form to fail in Reader. Some questions for you.
    1. On the system with Adobe Reader, what is the system and the version of Adobe Reader?
    2. Are you viewing the PDF in a browser on the system with Reader, or in an Adobe Reader window?
    3. If it's a browser
    (a) what is the browser
    (b) if you download the file instead, and open directly in Reader, what happens?

    thankyou! - yes it seems though I was using the text tool when I had this problem: When I had created a few fields in the 'form edit' mode I then may later have added some normal 'comment' fields of text in the normal mode to highlight certain aspects on a form - I used the text tool thinking that was the right way in to add or edit text - I can now see I double click with the hand tool to still be able to access any text field I want to. Yay!
    Many thanks for your kind assistance - I shall try this method later on a new form I was trying to create and which was giving me grief.
    One last thing then: what is that default text field that can't be deleted actually for - why is it there? After all, a 'blank pdf' should surely be a truly blank pdf? Perhaps this is one for mighty Adobe....?

  • Check box results are not included in PDF download.

    I created a form that includes 300 plus checkboxes and imported the form into FC.
    I can see the form results in the excel spreadsheet and the table of results. But when I click the download PDF, none of the check box answers are downloaded. The text box results are visible on the pdf, but not the check-box results...
    I want to download a completed PDF that looks like the original document... Here's a link to the pdf form...

    Thanks for letting me know that there is a 'bug' and not a problem on my end.
    However, two weeks for resolution is a long time to wait when your working on a form project for a client. I just signed up for my first month on Forms Central and after a frustrating week of forum and one on one support, I learn that this online or "cloud" system couldn't process the PDF form.
    My concern is this... I found a possible solution to my need to obtain results from a pdf form, but is it reliable? I am currently a week behind on a project and will be even further behind if I use FC to complete the project.
    I'm questioning whether I need FC or not. Couldn't I email the fillable pdf form to the user and they complete it and email it back to me... I can save the completed form and export the data????
    I'm questioning the reliability and value of FC. If I could speak with support via telephone, then I wouldn't be behind on this project.. If you were in my shoes, how would you feel right now?
    Yes... I am a bit frustrated that my first FC form project doesn't work. I believe my concerns are reasonable. What can be done about the two week timeframe? This isn't what I was expecting.
