Creating a form in InDesign

Hi,
I have several documents set up in InDesign as templates.
My staff and volunteers will open the document, edit only a couple of items and print it out.
I would like to be able to have them use Acrobat Reader to edit the documents while (hopefully) maintaining the style (font style, etc).
See the items with the purple background in the example.
Most of the volunteers will have a PC.
Any ideas on how I should set this up and save it?

What you're asking for is totally doable, but will require that you have a full copy of Acrobat Pro. But once you set up the PDFs, your volunteers will be able to use Reader to edit the two form fields.
Set up your template in InDesign. Open the Buttons and Forms panel from Window > Interactive. You want to put those purple frame behind the form fields you're going to create below.
Draw a frame, select it, and choose Text Field.
Now, you can export this as a PDF, but you can't get that custom Font, point size, etc.
So, what you will have to do is open the PDF in Acrobat, Open Forms Editing, and edit that form to choose your own font, point size, etc.
It's a little complicated in Acrobat, and I'm in the middle of a project, but someone here might be able to talk you through it or in the Acrobat forum.

If you ave created the form and then filling it yourself and saving the form, the filled data should be there when you reopen the same form.

The only way to use Forms Central would be to export it the method that gives you the embed code (can't remember what the actual term is off hand) and then use Object--->Insert HTML and past your code in.

Is there any method for having text fields align the inputted text to the centre?

Trying to create fillable form from InDesign table

I am working on a very long document, over 100 rows and about 18 columns, which will exist only as a pdf. I need to make 6 of the columns interactive so that they can be filled in with one of three letters by the end user.
I created the design in InDesign CS 5.5 as a table. That worked well for design control, but Acrobat X 10.1.3 does not recognize any of the cells in the PDF document. Is there a different InD export option that will make a table convert to a fillable form? I used press quality, then opened the pdf in Acrobat.
I have worked around by creating a short drop down list with the three letters in it and copied that into the cells using the Place Multiple Fields command. Then I can adjust position easily enough.
*Except* my client would prefer that the end user just type one of the letters into the cell/field rather than doing a drop down selection on each of the several hundred cells. She'd also like to be able to copy and paste the final filled-in information into her original excel document.
Am I missing something in Acrobat? I don't do fillable forms often and have never had one this massive.
I am working on a Mac.

My client would like to fill in the PDF form using Reader, save under a different name locally on their device, and redistribute. The problem I have is that, sometimes, they want to redistribute the form as Read Only. I know how to flatten the form on a mobile device, but uncertain how they can do that with a PC or Mac using Adobe Reader. I looked at FormsCentral thinking there might be a solution going that route, but don't believe there is. Any advice?

FormsCentral won't help with flattening a form. You could set up the form in Acrobat so that the fields are set to read-only, using JavaScript. Not quite the same as flattening but it's the best Reader can do.
Note that when Reader is used to e-sign a document, it gets flattened, but this won't be possible if you need to Reader-enable the form and under certain other conditions.

Creating PDF forms: possible in InDesign?

I'd like to create PDF forms to be used for email questionnaires and surveys. The last time I looked into it I found that I needed to create the PDF first and then use another application (I can't remember the name) to add the fields for the form. I found it to be a little clumsy, and didn't have the layout control that I do with InDesign. Is there a way of creating forms entirely in InDesign?
Any help would be appreciated.

That would be Acrobat Pro.
Nothing ID can do for you here. You still create the PDF with a layout package, and add form fields (which are a PDF feature, not a layout thing) with Acrobat. Fortunately, Acrobat has a nearly magical Form Field wizard that'll help you through that process as painless as possible.

I need to create a PDF from InDesign that people can edit in Reader or Acrobat

I need to create a PDF from InDesign that people can edit (for example, change a name from "John" to "Johnny") from either Adobe Reader or Adobe Acrobat. I have already looked at forms, but they do not keep your font colors, faces, and sizes and just make everything that is selected as a text form Black, 12pt, Times New Roman.
Can anyone here help me?

If you use Acrobat pro, generate your .pdf file and then save it as (french version):
... so that anyone using Acrobat Reader (or Pro, of course) can modify the .pdf file (with limitations), add comments, ...

I read somewhere that one way is create a crossword with an external software and include it in the ID project but I don't know any crossword software and if I add in my ID project, I only need to export like a interactive pdf? I would like that the users can use the crossword in online version.

I'm not sure about what to you mean. But I create first the crossword with Excel, I mean to put the words in the right order and the questions and/or cue too. Then I create a table in InDesign and format some cells with black background and some borders. I upload both captures, from Excel and from InDesigne. Then with Adobe X Pro I create a form, so I can create singles inputs for the crossword.

Creating a form with radio buttons that users can fill, save, and e-mail back

I am new here, so please forgive me if this answer has already been answered. I have been looking around and still have not been able to find an answer.
I have created a pdf in InDesign and opened it in Acrobat to create a form out of that existing PDF. The form needs to contain radio buttons and text fields. When I save it to test it out on Adobe Reader (for the typical user), it says that I am only allowed to save it in its original form (without any of the changes) thereby making this form useless (I do not want users to print it out; I'd like them to email the entire PDF back to me).
I've looked online and found some answers saying that I would just need to enable commenting, but when I tried that, I could no longer select any of the radio buttons in Adobe Reader.
Any suggestions?

I'm on a Mac using Adobe Acrobat 7 Professional.

You won't see that in 7. It didn't exist until Acrobat 8. Though It would work with PDF's opened by Reader 7.
The only choice you have is connacting someone at Adob coporate and finding out how to sign up to a Service they have that will llow you to do that.
Caution: its very, very expensive and unless your fortune 1000 company you can't afford it.
OR you could upgrade to Acrobat 10 (if you have a Machine capable of running it - Intel Only) or Acrobat 9 (PPC and INTEL).
Note the feature they speak of only allows for 500 instances of any one PDF set up to do so.

Does anyone know the best way to create a form that you can do a mail merge with?

I have been asked to create a form that can be used to do a mail merge. At first I thought I could create it in indesign and then pdf the file and do form fields right in the pdf, but I'm not sure if you can do a mail merge with data in an excel file and a pdf file containing form fields.
The other option I know I could use is a Word file and excel, but I don't think the functionality is as good as in a pdf (like addition of percentages, or error messages, radio buttons, etc.). And I do need to be able to do a mail merge. Anybody have any suggestions?

There is not built-in feature for this, but it can be done with scripting. The problem is it can be a bit complicated, and it more complicated to decribe in much detail in a forum posting. It involves creating the base form and making the pages templates. You can export from Excel to a tab delimited text file. You can then either programmatically create a template-based FDF that when loaded into Acrobat populates the PDF form, creating as many individual forms as needed, or using JavaScript in Acrobat to read the data file and generate as many individual forms as needed. Both approaches involve a similar amount of programming, but they're quite different.

Creating a form with toggled sections

So I know how to use a button action to hide and display forms in Indesign. What if it's not a form element though? What if it's a text box or graphic?
I'm trying to create a PDF form with a tab menu where the user can click on one tab and the PDF will display one set of form fields while hiding the other set. Vise versa I want the opposite to happen with I click on another tab. I've seen that you can turn anything into a button and then it's available in the "Show/hide buttons and forms" button action but the problem I have is that I have form fields on top of other objects and when I convert those objects to buttons then I can't access the form field once I export the PDF. If anyone knows a way to make this work, that would be so awesome.

I sense your sarcasm, but of course Larry's answer, un-detailed as it may be, is fundamentally correct.
In Acrobat, you can use Javascript to show/hide buttons and fields, which is much more flexible than the basic functionality you get from the InDesign UI methods. If you're inexperienced with writing code, it might seem daunting at first, but once you find some examples (easy to find online), you really only need to copy/paste a few expressions, substituting your button/field labels for whatever pre-existing.
Assuming you have the full version of Acrobat, here's something to get you started:
With what you've already got...in InDesign...keep all your form fields as they are. Anything that's not a field, but that you will want to show/hide; make it a button...but don't set anything to show/hide...just export to PDF and get it all into Acrobat where you can manipulate it via Javascript. (You don't even need to have a final layout. Anything that's a field or button can be repositioned once you're in Acrobat.)
In Acrobat, enter form edit mode and see the list of fields and buttons in your form. Choose the first click-able (button) item to use as a show/hide toggle, and access its properties (double-click or rt-click). Go to the Actions tab, select a trigger, (Mouse Up is most typical), and from the Select Action menu, choose Run a Javascript. Here, you could avoid Javascript and choose Show/Hide a Field, which will allow you to choose from a list. This also works, but is ultimately still somewhat limited, not unlike the InDesign features. To explore the Javascript possibilities, click the Add button to open the Javascript editor.
Copy/paste this:
this.getField("fieldName01").display = display.visible;
this.getField("fieldName02").display = display.visible;
etc., for all the elements you want to make visible, and this:
this.getField("fieldName03").display = display.hidden;
this.getField("fieldName04").display = display.hidden;
etc., for all the elements you want to hide.
Of course, it can go much deeper than this, but that's all there is to basic show/hide function via Javascript. It solves your immediate problem, and once you get past that, there are many more equally simple possibilities.

I am trying to create a form in acrobat pro 8 and it can't be filled in in reader 9

When I open the form in reader, it says it is a pdf/a file.and the form fields do not show up unless I go to edit/preferences in reader and tell it to never view documents in pfd/a mode. I am putting this document on my website for users to fill out, and don't want them to have to do that. How do I create the form that is not a pdf/a document? I have been messing around with this for a day and have not been successfull. I have an old document from last year that apparently is not a pdf/a document and it works fine. When originally creating the pdf, you would need to choose another pdf conversion setting. In Word if you use the pdf menu, change your settings there: Adobe PDF > Change Conversion Settings. I would use High Quality Print instead. If you use the File > Print method, click the Properties button next to the Adobe PDF printer selection.
For your already created form, you can change the file so your users will not encounter issues. In Acrobat 9, which hopefully is similar in process to 8: Advanced > Preflight > Profile tab > PDF/A compliance > Remove PDF/A information.(You'll have to unsecure your form first).
