Directory Utility Won't Launch, Pinwheel of Death...
So, just today, two Leopard Macs (10.5.7) on my network lost connection to our Active Directory. A switch was rebooted, and the Macs lost DNS for only a couple minutes. When I went to reboot them to get the authentication back, as I've done many times in the past, neither of them could see the directory server. None of the AD accounts could log in. So I went to log in as a local admin, and went to launch Directory Utility to unbind and rebind, and I just get the pinwheel of death. This is happening on a G5 tower and an Intel iMac. Both were 10.5.7, and I was hoping an OS update might fix it, so I upgraded them to 10.5.8 and that didn't help.
Just out of curiosity, I tried to run DU on another Mac Laptop that has never been part of any directory, and it is doing the same thing. The G5 is a pretty fresh OS install with no third party software installed other than VLC. So I'm not really sure what's going on.
I tried using dsconfigad from the command line to Unbind the machines from the directory, and it just hangs after it asks for the network password... I'm confused, and unable to log into my macs...
Post to the Server Products forums where those mavens hang out.

This might be a coincidence but I set up a new 2Tb Time Capsule today and assumed all was working as planned. I setup Time Machine to refer to the Time Capsule drive and the backups have been performed in keeping with the schedule etc.
My issue is that for some reason the Disk Utility doesn't display. The icon will bob in the dock and then after that nothing happens. Like I said, coincidence?
Latest OS X Lion 10.7.4 with one External HD attached and working as expected. In fact I used the Disk Utility only two days ago to format said external drive.
Any ideas anyone?Try in safe boot mode ( start the machine with the left shift key held down) if it launches repair the disk permissions and the disk itself, reboot and re try.
If it doesnt work in safe boot mode, boot from the recovery partition in lion ( cmd + R while booting) if snow leopard or below boot from the install cd by holding c when your turn the machine on and try disk permissions from disk utility in that menu.
let me know that if doesnt help -
Safari 5.0.3 won't launch (dreaded pinwheel)
MBP 15" OS 10.5.8
Installed Safari 5.0.3; doubled clicked and app won't launch: pinwheel spins for 1/2 hr. until force quit. Used Appcleaner to uninstall Safari, ran Disk Utility-> repair permissions. Re installed Safari: same result. Tried going to Library->Input Manager folder; that folder does not exist. Using Firefox for now but would like to resume Safari. Any help (in basic terms) appreciated.I assume that it is not a plugin problem?
seems not, certainly.
It's a pain it not showing in AM - I can only suggest double-checking the other folders mentioned in http://support.apple.com/kb/ts3230
/Library/Input Methods/
/Library/InputManagers/
/Library/ScriptingAdditions
and also LaunchAgents & LaunchDaemons & StartupItems from the same HD/Library/
Let's try Safe mode too - restart the mac holding down the Shift key. Try safari, then reboot normally to return to normal mode.
At this rate it'll be quicker to get you back on 5.0.2 (was it?) & then at least it'll show in AM so we can see what's what. -
My apps won't launch & menu bar missing all top left icons!!!
A strange concurrence of multiple problems:
I am a pretty advanced user and have an iBook G4/1.33 GHz running Tiger 10.4.8, but I am having trouble with my PowerMac G4/500 MHz running 10.3.9 right now. I keep my PowerMac in Panther since it is almost 7 years old and I only have Tiger on my iBook disc.
Suddenly a few things started to happen at once yesterday:
1) NONE of the top right menu bar icons (OS 9 status, modem status, iChat, monitors, AirPort, Volume, clock, etc. show up upon log-in or restart.
2) Virtually NONE of my apps will launch from the Dock or the Finder. They bounce a few times then give up, with no error message.
I peformed successful repairs booting from both the OS X Panther disc and TechTool Deluxe. Although recommended, I can't create a TEST User because System Prefs won't launch, can't repair permissions from the problematic drive because Disk Utility won't launch, and I can't clear out caches because Cocktail won't launch.
I made sure the Application Support Folder in my Home Library had read/write permissions for me and applied to all enclosing folders as suggested. I tried trashing the com.apple.ActivityMonitor.plist preference in my Home Library also as suggested. However, it wouldn't allow me to remove the Launch Services file in the Caches folder of the main Library Folder as I also read here in the discussions.
Aside from successfully running disc repairs from the startup disc and TechTool, I tried repairing permissions from the startup CD as well (since couldn't launch any apps like Disk Utility from regular startup drive). Every time I try to repair in this manner though, after a minute it tells me "Disk Utility lost connection with Disk Managment" and I must quit and relaunch Disk Utility. However, same thing happens when I do, for both my drives running copies of OS X 10.3.9.
Only thing I can think of is to do an Archive/Reinstall of one of the two OS X systems on two different drives. But does anyone else have a clue what is causing this or what I need to try??? I read similar stories on here but nothing will all these symptoms at once.
I would REALLY appreciate some input, thanks!!!

Check the following link for OS X troubleshooting steps. When doing so, following all steps in the order provided:
http://www.thexlab.com/faqs/repairprocess.html
1 If resetting permissions doesn't help, you may have a corrupt preference file... Delete....
Delete ~/Library/Caches (folder)
~/Library/Preferences/com.apple.systemuiserver.plist
~/Library/Preferences/ByHost/com.apple.systemuiserver.xxx.plist
where 'xxx' is a 12 digit (hexadecimal) numeric string
• Then log out and back in again. You may need to go to System Preferences to put back the menu items you need to display.
• Removing systemuiserver.plist will cause the right half of the menubar back
its original pristine default state.
2. Check Preferences Thoroughly;
- Navigate to ~/Library and drag the "Preferences" folder to the desktop.
- Restart and test your applications.
• If this does not work, move the "Preferece" folder back to ~/Library
• If this works, delete the old preferences folder. (You'll have to go through some of your System Preferences and apps to set the preferences back to how you like them).
3. Navigate to ~/Library and drag the "Fonts" folder to the desktop.
- Restart and test your applications.
- If they work start adding the fonts back few at a time.
( A likely suspect is Time RO, also if you do not use Classic you can trash the classic fonts.)
4. Check Permissions Inside Home Folder
- Navigate to ~/Library.
- Get Info (Command + i) on the "Applications Support" folder.
- Open the "Ownership & permissions" flippy triangle.
- Make sure you are the owner, with "read and write" access.
- Click on "apply to all"
- If this is correct, open the "Applications Support" folder and do the same procedure (Command+I) for the folders with the names of the applications you are having trouble with.
Note...The reason to check this is because repairing permissions with Disk Utility doesn't touch permissions inside your home folder.
• Create a new User.... go to System Preferences/ Accounts and click on "+" at bottom left of window..... and test the apps in this new account if they work the problem is isolated to your User
(~ means your home directory)

After using Norton Utilities 8.0 my iTunes crashed and won't launch. Already ran Apple disk utility and repaired disk permissions. The following was in the iTunes.crash.log:

Why have my notes and contacts been deleted and my applications won't launch
I tried using my iPad mini today but both my notes and contacts have been deleted and my applications either won't launch at all or launch for a few minutes and then shutdown. I also tried to download another app which said had been downloaded but I couldn't find it anywhere, the same thing happened with another app I downloaded. It was working fine yesterday and I haven't updated it or done anything out of the ordinary since then. What's wrong?
There are some simple troubleshooting steps you can take.
Be sure you drive is not overly full. Rule of thumb would be over 85%.
Create a new User go to System Preferences >> Accounts >> "+" (make it an admin acct) and test the apps in this new account, if they work the problem is isolated to your User and not systemwide.
If the issue is limited to your user account try starting up Safe Mode (It will take more time to startup in Safe Mode because it runs a directory check.)
If your apps functions correctly that way, go to System Preferences >> Accounts >> Login Items, and remove them. Boot normally and test. If not go to ~(yourHome)/Library/Contextual Menu Items and move whatever is there to the desktop. Then do the same with /Library/Contextual Menu Items. Lastly, try moving ~(yourHome)/Library/Fonts to your desktop and restarting.
Log out/in or restart, if that sorts it start putting items back one at a time until you find the culprit.
If the issue is systemwide then, you may be able to repair this with the The 10.6.2 Combo Update This is a fuller install, as opposed to an incremental "delta" update so it should overwrite any files that are damaged or missing. It does not matter if you have applied it before.
Remember to Verify Disk before update and repair permissions after update from /Applications/Utilities/Disk Utility.

App won't launch--deadlines looming--help
Is there anything else I can try? Quark won't launch while logged in as the main user but will as a different user. I tried dragging the .plist to the desktop. I tried replacing the .plist with the one created when I logged in as a different user. I've turned off fonts and most startup items that are active only for the main user. I don't think I've changed/added much since this started happening today. Is there a log or report I can post? Should I reinstall Quark? Any suggestions would be appreciated.
Can I give you my full spiel?... Fantastic!
Hopefully it will explain why some problems...
"Try Disk Utility
1. Insert the Mac OS X Install disc that came with your computer, then restart the computer while holding the C key.
2. When your computer finishes starting up from the disc, choose Disk Utility from the Installer menu. (In Mac OS X 10.4 or later, you must select your language first.)
Important: Do not click Continue in the first screen of the Installer. If you do, you must restart from the disc again to access Disk Utility.
3. Click the First Aid tab.

The usual reason why updates fail or mess things up, or things don't load/run, is if Permissions are not fixed before & after every update, with a reboot... you may get a partial update when the installer finds it doesn't have Permissions to change one obscure little part of the OS, leaving you with a mix of OS versions.
Some people get away without Repairing Permissions for years, some for only days.
If Permissions are wrong before applying an update, you could get mixed OS versions, if Directory is the slightest messed up, who knows!
If many Permission are repaired, or any Directory errors are found, you may need to re-apply some the latest/biggest updates again, or even do an A&I if you have enough free disk space.

Do you think I should reinstall Quark?
I do think Carolyn's suggestion is worth a try, but you may need to uninstall it first, or get rid of every remnant anyway before installing, not certain, only used Quark 4 a couple of times & decided that I didn't need to spend a couple of years doing what I could already do on my Ataris of all things! -

I can't get FCE to launch. I've had the program for months and it was working fine. Recently stopped working (maybe after a couple of programs were updated?) Won't launch at all. I've tried all the stuff in related posts:
* Updating Quicktime
* Trashing prefs
* Repairing prefs
* Uninstalling and re-installing FCE
I've also disconnected various devices (Sony DCR-HC26, firewire splitter device, M-Audio Solo firewire audio input interface) and restarted the computer, re-installed drivers for the audio interface, etc. All to no avail.
Incidently, iMovie won't launch either, so the problem would seem to be related.
Thoughts? These are the exact instructions as described in the Disk Utility Help Manual:-
Testing and repairing your startup disk.
If you have problems with your startup disk, you can use Disk Utility to test it for errors and repair it. To repair your disk, you must start up from another disk, such as your Install Mac OS X disc.
You may be able to test your startup disk without starting up from another disk. Open Disk Utility, select your startup disk, and click First Aid. If the Verify Disk button is available, click it to test your disk. You need to start up from another disk only if Disk Utility finds errors or if the Verify Disk button is dimmed. When testing your startup disk, Disk Utility may report errors when there are none. Starting up from another disk and then running Disk Utility gives more accurate results.
You can always test and repair disk permissions on your startup disk without starting up from another disk.
Start up your computer using another disk.
To use the Install Mac OS X disc, insert the disc and restart your computer holding down the Option key, then select the Install Mac OS X disc and click the arrow.
Open Disk Utility.
If you're using the Mac OS X Install disk, follow the onscreen instructions until the menu bar appears with the Utilities menu in it., and then choose Utilities > Open Disk Utility.
Select the startup disk in the list of disks and volumes, then click First Aid.
Check the S.M.A.R.T. Status at the bottom of the window. If you can't see it, be sure you selected the hard disk your volume is on, and not the volume itself.
If the S.M.A.R.T. Status is "About to Fail," back up your files on the disk as soon as possible and replace the disk.
If the S.M.A.R.T. Status is "Verified" or "Not supported," click Repair Disk to repair the disk.
If Disk Utility tells you to look for links to corrupt files in the DamagedFiles directory, two or more files occupy the same space on your hard disk and at least one of them is likely to be corrupt. Examine each affected file in the DamagedFiles folder, which at the top-level of the affected disk. If you can replace it or recreate the file, delete it. If it contains necessary information, open it and examine its data to make sure it has not been corrupted.
If Disk Utility cannot repair your disk or reports "The underlying task reported failure," try to repair the disk again. If that doesn't work, back up as much of your data as possible, reformat your disk, reinstall Mac OS X, and restore your backed up data. If you continue to have problems with your disk, it may be physically damaged and need to be replaced. See an authorized Apple dealer for more information. -
PSE9 on iMac shows pinwheel of death for 10 to 15 seconds when mousing!
Help! My PSE9 on my iMac (10.8.3) has become nearly completely unresponsive. When I mouse over any menu or tool, I get a 10 to 15 second pinwheel of death. I can't select tools from the toolbar. I think this all started when I attempted to update the Inspiration Browser (that ended in failure). I have tried removing the program and reinstalling, but that hasn't helped. I'm relatively new to the Mac world, so I'm not sure whether I removed the program properly.
I trusted that this Adobe software would be a good program for my photo editing needs, but now it's useless. How can I resolve this issue?
I don't know if it makes a difference, but I bought Photoshop Elements & Premier Elements as a boxed set.
Any suggestions?I would try repairing and optimizing your catalog while you're in there (if you use the organizer), but that won't affect the editor's performance.
To repair permissions, go to applications>utilites>disk utility and click your computer in the list on the left side of the window, then click Repair Permissions. This can take quite a while, so don't fret, and don't get upset about all the messages you'll see passing by in the window while it's working (SUID has been changed and will not be repaired, for example). Those don't matter. The only thing that matters is if you see Repair Failed in red at the end of the process. -
Xcelsius exe won't launch - Entry Point Not Found message given
-Operating System: Windows XP
-OS Patch level: SP3
-Office Version: 2007
-Flash Player version: Adobe Flash Player 10
-Xcelsius Version and Patch Level: Xcelsius 2008 5.3.0.0
-Xcelsius Build number (Help>About Xcelsius): <not accessible>
-Business Objects Enterprise version/patch level (if applicable): Crystal Reports 12.1.0.892 - SP1
-Issue Description: Xcelsius exe won't launch - Entry Point Not Found message given
-Steps to Duplicate: When I try to open Xcelsius.exe, I receive a message box that says "Xcelsius.exe - Entry Point Not Found" "The procedure entry point ?LevelDiffToFirst@CTreeBtnCtrl@@QAEHPAU_TREEITEM@@@Z could not be located in the dynamic link library TreeBtnExtDll.dll."
I have uninstalled and re-installed Xcelsius with the same results.
Also, when I open Crystal Reports 2008, the "Crystal Reports 2008 SP1 - Software Manager" opens and shows an update available - SP2. When I try to install the update, it downloads and then displays a message the this update is only available for Crystal Reports 2008 SP1. I believe that is my current version.
After some research, I believe my CR Runtime app may be a different version than what Xcelsius.exe is expecting, but I don't know how to check or fix that. Any help is appreciated.Hi
I think I've managed to fix this error. I compared the TreeBtnExtDll.dll in my Xcelsius directory on the machine that wasn't working and one that was and found that the broken machine was using an older and smaller version of TreeBtnExtDll.dll. 89k and a date in 2008 I think. So I copied my newer version which was 109 k and dated 11/2009 into the C:\Program Files\Business Objects\Xcelsius directory of the broken machines and now xcelsius appears to be working fine.
I had lots of trouble installing the SP3 release because of the media issues so maybe the install wasn't complete.
Anyway, I hope this helps you

Disk Utility won't start up in 10.5.3
I have been unable to open up my Disk Utility at all. I was able to do so earlier, perhaps with 10.5.0 while I was mounting my external hard drive. Now, I cannot mount the hard drive, and therefore cannot use Time Machine all because DIsk Utility won't open. I've diagnosed my hardware with "Disk 1" and found no problem. And, I've reinstalled the operating system. Any ideas?
Chris:
I just had the same issue on my son's MacBook. As long as you have a wired keyboard, the following will work.
1. Shut down the computer if it is on.
2. Press the power button to start the computer.
3. Immediately press and hold the Command (Apple) key and the "s" key for single-user mode.
4. Type mount -uw / (then press the Return Key) wait for next line to appear with # root/
(there is a space between t and - and another space between w and /)
5. Then Type "passwd" (without quotation marks then press the Return Key)
6. Enter new password (this will be for the root user account) (watch your fingers when you type because it doesn't show what you are typing, then hit the Return Key after you type it.
It will prompt you to retype the new password again, so retype it again and then hit the Return Key) wait for next line to appear...
7. Then Type "reboot" (without the quotes and hit the Return Key)
after the computer restarts and comes up with the login window....
8. Login with any of your usernames just to get the desktop up.
9. After desktop comes up choose System Preferences, then choose accounts. Unlock if you have to and using the username: root ..... and the password you just setup in the single user mode You now should be able to make users including your existing user account an admin.
After you log back in under your real account, it is advisable to deactivate the root user account. To do this,
1. Click the Finder icon in the Dock.
2. From the Go menu, choose Utilities.
3. Open Directory Utility.
4. Click the lock in the Directory Utility window if you need to unlock the utility.
5. Enter an administrator account name and password, then click OK.
6. Choose Disable Root User from the Edit menu.
Hope this helps.

0x7ffFor the benefit of others, We found out the reason for this crash was due to permission issue of the user on the machine. Temporary solution was to run Illustrator with sudo permissions on the terminal
e.g.
$ sudo /Applications/Adobe\ Illustrator\ CS6/Adobe\ Illustrator.app/Contents/MacOS/Adobe\ Illustrator

Finder won't launch - tried everything - HELP!!!
Okay...
So following all of the sudden, the finder won't launch. Clicking on it on the dock doesn't work, and I can't use force quit to relaunch it (obvious, since it wasn't launched in the first place). Rebooted several times, to no avail. Further noticing, that only a few apps will open (Safari, Mail, TextEdit), but others won't, and even then, you can't interact with anything on the desktop (i.e., opening a text file with Safari). Things like iTunes or AppleWorks won't open at all, both are which essential to me.
I've tried booting up in Safe Mode. Nothing. I tried repairing the permissions. Nothing. I tried running from the Install Disc, and repairing via Disk Utility (several times). Nothing. In fact, it did note several errors, but was unable to repair the drive. Also tried archive and restore off the 10.3 Disc 1, but wouldn't let me, as it said 10.4.9 was newer and couldn't repair through the older disc.
So does anybody have any solutions on this? I upgraded to Tiger via a downloaded update, so I don't have the Tiger disc to repair through. And I certainly don't want to go through the whole debacle with a complete reinstall with a slave drive. Anything would be greatly, greatly appreciated.Well that sounds like it could work...and I really should get a backup...
BUT!
Saving that as the final straw, would there be any cheap (e.g. free) solutions left? I'd rather wait to buy another external drive, since I'm trying to save money now, so any immediate and cheap solutions would work best.
Hmm... I upgraded to Tiger via download, so I don't have the boot CD for it. But since I already have Tiger, would it be at all possible to get a free 10.4 boot CD to help with this problem? If I explain my plight to Support, do you think they could spare one, since it's very probable 10.4 Disk Utility would work, since the only reason 10.3 couldn't was because it was too low?
If that doesn't work, I suppose the only thing left to do would be to carry through and clone to an external. But first, do you think I could get a 10.4 disc for my problem, or would that be "forbidden" for some reason?
