Disk Utility Error- Filesystem verify or repair failed.

I ran Disk Utility and before it finished I received Filesystem verify or repair failed error message.
It says to run repair but I can't
HELP!!! What do I do? What is wrong?

Could be many things, we should start with this...
"Try Disk Utility
1. Insert the Mac OS X Install disc, then restart the computer while holding the C key.
2. When your computer finishes starting up from the disc, choose Disk Utility from the Installer menu at top of the screen. (In Mac OS X 10.4 or later, you must select your language first.)
*Important: Do not click Continue in the first screen of the Installer. If you do, you must restart from the disc again to access Disk Utility.*
3. Click the First Aid tab.
4. Select your Mac OS X volume.
5. Click Repair Disk, (not Repair Permissions). Disk Utility checks and repairs the disk."

Then try a Safe Boot, (holding Shift key down at bootup), run Disk Utility in Applications>Utilities, then highlight your drive, click on Repair Permissions, reboot when it completes.
(Safe boot may stay on the gray radian for a long time, let it go, it's trying to repair the Hard Drive.)
If perchance you can't find your install Disc, at least try it from the Safe Boot part onward.

I can't seem to mount my WD external hard disk. Run Disk Utility and these are the error messages:

Does anyone know what is wrong and how do I fix this problem? Is it a hard disk failure? Will I be able to recover my data? Thanks!

You will need Disk Warrior to fix the problem if Disk Utility cannot. However, to fix with DU you need to do this:
Repairing the Hard Drive and Permissions
Boot from your OS X Installer disc. After the installer loads select your language and click on the Continue button. When the menu bar appears select Disk Utility from the Installer menu (Utilities menu for Tiger, Leopard or Snow Leopard.) After DU loads select your hard drive entry (mfgr.'s ID and drive size) from the the left side list. In the DU status area you will see an entry for the S.M.A.R.T. status of the hard drive. If it does not say "Verified" then the hard drive is failing or failed. (SMART status is not reported on external Firewire or USB drives.) If the drive is "Verified" then select your OS X volume from the list on the left (sub-entry below the drive entry,) click on the First Aid tab, then click on the Repair Disk button. If DU reports any errors that have been fixed, then re-run Repair Disk until no errors are reported. If no errors are reported click on the Repair Permissions button. Wait until the operation completes, then quit DU and return to the installer. Now restart normally.
If DU reports errors it cannot fix, then you will need Disk Warrior and/or Tech Tool Pro to repair the drive. If you don't have either of them or if neither of them can fix the drive, then you will need to reformat the drive and reinstall OS X.

WD External disk Error: Filesystem verify or repair failed

Hi all
My Western Design external hard drive has started to run really slowly when accessing data as well as transferring. After plugging the usb to the mac it takes about 30 seconds to pick up the drive with a DISK ERROR message. I have been into disk utilities and tried verifying the disk and repairing with the following message coming up...

It sounds like your WD drive is failing. Download Smart Utility from http://www.volitans-software.com/smart_utility.php and see what it says about the health of the drive.
If you scan various discussion groups, you'll see issues with WD external drives, as the electronics can be problematic.

Unmounted External Hard Drive, Error: Filesystem verify or repair failed

Earlier today, I disconnected the external hard drive while it was working.
Now, the External Hard Drive won't even show up on my computer. However, on the Disk Utility it does. I have tried to verify & repair the disk. Plus click the mount button nothing work.
Whenever, I ran the system verify or the repair. I got this error: Error: Filesystem verify or repair failed
Ok. So I have searched all over the internet and forums. Tried everything. Nothing works.
I even tried Disk Warrior. It would work for a while, then when it came to step 8. It would make my computer go black and hold down the power button to restart.
Any suggestions? Please let me know, I would like to recover the files if I can. Thanks.
P.S.Yes I have tried to restart multiple time. I have tried with my USB 2.0, firewire 400, & 800 too.. Nothing.Everyone except the USB shows up in the disk utility but doesn't mount.

I have had the exact same situation occur, but was able to follow these steps to successfully save everything!!
The Unix program "dd" will instruct a drive to stream out all of it's data from beginning to end without regard to file structure, therefore creating an exact block-for-block duplicate. By doing this, it will allow you to copy everything off without having to mount the drive. It also ignores any corruption or physically damaged blocks on the drive which would otherwise crash or freeze the Finder. It saves the data to a disk image file which you can then open and use without fear of imminent failure (the format of the disk image will be the same as the original drive). Now that the data is preserved, you can safely go through and recover your files. Keep in mind that if a file was damaged or corrupted on the drive, it will be unusable, but at least you'll be able to get to the good ones safely. In my case, using "dd", I had no problems and got every file off. If it doesn't work for you, at least it's impossible for just trying it to further damage your files. I can recommend a few other remedies if this doesn't work for you.
Note: The computer must be able to at least recognize the drive. That means either 1) It mounts 2) When you connect the drive the Finder asks if you want to erase or ignore, click ignore and you should be able to do this, or 3) Nothing happens in the Finder, but the volume in question does appear in Disk Utility. If you connect the drive and absolutely nothing happens and it never shows up in Disk Utility, it won't work.
Note: As always, it is suggested that you backup important data on your startup disk before proceeding with stuff like this. It is possible to wipe a drive by mistyping a terminal command, so double check before you hit enter.
_Now, on with the show!_
Prep: Restart your machine so that it goes straight to the login window. Enter your user name and password. As soon as you press enter after your password, immediately hold down the shift key until the Finder finishes loading (this prevents any background startup items from running that might interfere).
1. Attach the drive to the computer and make sure you have another drive with enough free space to hold the entire capacity of the bad drive, not just it's used space, but the WHOLE size of the bad drive (for exception, see very bottom of this post).
2. In Terminal, type:
+diskutil list+
Look for the drive you want to recover in the "name" column, then write down it's "identifier" (ex. disk1s2)
3. In Terminal, type:
+dd bs=512 if=/dev/disk#s# of=/my_directory/image.dmg conv=noerror,sync+
!! Replace "disk#s#" with the identifier you wrote down before !!
!! Replace "/my_directory/" with the path to where you want to save !!
!! Replace "image.dmg" with the name you want to give the disk image (must end with .dmg) !!
4. To confirm that something is happening, open Activity Monitor (in the Utilities folder) and click "Disk Activity". Once you determine an average transfer rate, divide the size of the drive by that and then divide by 60 to see how many minutes it will take to finish. *+! ! FOR LARGE DRIVES THIS WILL TAKE A VERY LONG TIME ! !+* To be safe, do not use your computer for anything until the process finishes, you can leave Activity Monitor open so you can see when it's finished.
If you're only really concerned about a certain files and you know their exact path on the bad drive, let me know and I can give you modified instructions to copy just those files, saving the time and space of doing the whole drive.

Western Digital 1TB Hard D "Error: Filesystem verify or repair failed."

I've had this external Western Digital hard drive for a year, it stopped working, tried to repair in disc utility, repair/verify failed

If you have the data backed up, try using Disk Utility's Erase tab to reformat it. Then restore the data from backup.
If you have no backups, you might have good luck with DiskWarrior. It costs about $100.
If neither of those help, it sounds like the drive is dead.

Help with Error: Filesystem verify or repair failed

I already made a search on this problem already and there aren't many clues as to what could be the problem and solution. Like one of the solutions suggested is to start in safe mode and then restart normally (?) as if that could solve it.
I tried repairing the HD with disk utility and all I get after some time into the checking process is this error:

YES, that did it, it seems there are files that Disk Utility can't check while Leopard is running, and interprets as errors. The repair from the Installation CD said the disk seemed OK. No errors reported, just the green OK text.

I discovered my optical disc drive was having a problem when I tried to burn a dvd with dvd studio pro and it told me "the recording device reported the media error: unkown device error" so when I googled that, I was told to try to repair the disk. It didn't let me repair the disk so I tried to verify the disk. This is what it said:

I have no idea what any of this means! I don't have the original leopard installer disc because my computer just came with leopard. How else can I repair the disk?? Please help!!

But now when I push "partition" in Disc Utility is says "The disc cannot be partitioned because some files cannot be moved. Back up the disc and format it". But i don't have any external hd to back up. Anything else can be done? Thanx again

I have an old PM G5 Tiger hard drive that would not mount nor can be repaired by disk utilities (Leopard Disk Utility or Disk Warrior). I bought a new HD and installed Leopard on it. The old Tiger HD showed the following on Leopard Disk Utility (First Aid failed):

The Tiger HD cannot be booted at all.
I tried your second suggestion. I plugged the Tiger HD and booted the Leopard HD.
The Tiger HD can be seen by Disk Utility, still unmounted and greyed out.
Again, the old Tiger HD showed the following on Leopard Disk Utility (First Aid failed):

What do these "comments" mean?
Do you reckon my Tiger hard drive is corrupted and can not be repaired with any Mac Disk Utilities?

In the process of installing OS X Maverick on mac mini; got error "Filesystem verify or Repair failed"

I am in the process of installing Maverick on my Mac Mini. I got an error: "Filesystem verify or Repair failed".
Most threads suggest that I either:
1. restart process,
2. install disk and try again,
3. recover disk
Since I am in the middle of installing, I cannot seem to go back to my working OS to restart the process. Since Maverick is available in the App Store, i cannot use a disk for it. That leaves me with recovering the disk. Can anyone suggest how I can do this without destroying all my files on the computer?

I do have a backup, but I can't access it right now. I tried the OS X Recovery, but when I try the "Command-R" option, it just goes right back into the install. I have used the "Command-R" option before, so I know when and how long to hold it for. I just couldn't get it to not enter the install. I did manage to at one point get a prompt where it asked me if I wanted to install from Macintosh HD (main partition) or Backup (secondary partition), but I chose Macintosh HD, and it went back into the install. I couldn't get that option again for whatever reason.
Is there a way I can just start from scratch? Both Snow Leopard and Mavericks are online only. Is there a way to pause the install, connect to the internet and install a brand new (wipe out all files - I can always use by backup to restore my important files later) copy of the OS?

Time machine error: Filesystem verify or repair failed

Hi all,
Been using a Maxtor One Touch Mini for my TM back ups. All of a sudden I get failures.
- I get the message: Filesystem verify or repair failed.
- I once canceled a TM back up while it was in progress and my gut says this must have something to do with it.
- I've run disk utility and get the same error when verifying.
- I can read my files and copy them off the drive. I cannot transfer files to this drive. I can also enter TM and view the previous TM backups, etc.
Anyone have tips for me on how to handle this?? Thanks!

Reformat it anyway. You can probably continue using it, I just wouldn't trust it 100%.
I learned many years ago (the hard way, of course ) not to trust my backups to a single app or piece of hardware. No app is perfect, and certainly no hardware is infallible.
Many of us keep both full Time Machine backups and a bootable "clone," preferably on a separate HD, via CarbonCopyCloner (which I use) or the similar SuperDuper. There are variations, but typically a clone is an exact copy of your system, with none of the previous copies of things you've deleted or changed, like Time Machine keeps. Unlike Time Machine, you can boot and run your Mac normally (but a bit slower) from them.
This gives you the advantages of both types of backup, and protects you from a problem with one of them (external drives fail, too). Some folks do the "clone" to a portable external HD, and take it to their safe deposit box, workplace, relative's house, or other secure location periodically. Others have a pair that they swap weekly or so. Thus they're also protected against fire, flood, theft, direct lightning strike on their power lines, etc.
CCC is donationware, so you can use it free for a while. If you decide to keep it, send them some $$ so they can keep it up to date. SD has a free version, but to do updates (rather than a full replacement) or automatic scheduling, you need the paid version (about $30).
Either is available via Google, VersionTracker.com, or MacUpdate.com.

Disk Utility error msg: Filesystem verify or repair failed!!!

Ok so after erasing my partitioned HD I wanted to use the rest of my HD on my start up HD, So I pulled the expander in the partition window to resize the volume of the HD. Then I clicked apply to resize the HD and ended up with a Partition Failed message that stated
Partition failed with the error:
Filesystem verify or repair failed.
What do I do from here to get my current HD at full capacity?
The current size is 19.9 GB and it should be at 60 GB.

Ok, so I had a partitioned hard drive, and I wanted to expand the start up drive and delete the unnecessary partition. So in disk utility I clicked on partition and deleted the partition I didn't want.
Then I used the expander thing in the bottom corner of the start up hard drive box and clicked apply to enlarge it, but filesystem verify or repair failed message appeared.

So for some reason podcasts suddenly stopped downloading to my external music HD. Checked the permissions on the HD and it wasn't giving me access to the permissions even though I had permissions. Ejected the drive, mounted, ran Disk Utility and got the message above (subject of this post). The details box says the following:

So.... is my external drive dead? Can it be fixed? Is everyone going to recommend pro tools and disk warrior again? Anyone that has a bright idea, I am all ears. I have Time Machine running, but I am not sure how I would restore the drive from that and get my data back. Any help would be greatly appreciated!!

Well, oddly, it just fixed itself. "Bogus" you say? It is really odd. It just mounted itself, all my audio is working and podcasts writing to the drive again. I ran disk utility again and it ejected the drive and I got the same error message. I'm getting tons of conflicts on my system ever since I did my clean install of Leopard, so I imagine it has something to do with that.

My trusty MacBook Pro is starting to play up. i.e. crashing unexpectedly & at random, no particular program can cause the screen to dim and a box come up in different languages saying you must restart with a pic of the power button.

I have also run a hardware test after the disk verify didn't work from the install cd.

now I thought that sorted it. but after doing the updates. and then going to repair the disk permissions/repair the disk it came back with same error. and also the hardware test is still failing.
The system will never be stable until the HD directory is repaired.
- is the version of the hardware tester I have not compatible with the OS or the firmware i have upgraded to? or does the tester just check the integrity of the hardware regardless of firmware & os vers.
If this is the AHT disc that shipped with your MBP, it is fine, regardless of what OS you have installed.
I really would prefer to not start from scratch again. But could this be an user account problem?

I'll set up a new usr acct and use that for a while. just a pain to bring the files over from my current usr acct to the new one.
I believe that's a waste of your time. As has already been stated, at least one of the issues is the damaged HD directory. Until/unless that is repaired, what you install on the HD isn't going to work correctly, if at all. The big question is, is some other part of the hardware causing the directory damage, or did the directory take a freak "hit" and repairing it will resolve things? It's quite possible that bad RAM or the logic board could have caused the directory damage. You're either going to have to bring it in to an Apple Store "genius" bar and see how much they'll test for free, or do some things yourself. For things you can do yourself, apart from trying one RAM module at a time, and running Repair Disk repeatedly (I don't think that's going to do it), you're going to need to either get your hands on Disk Warrior (or comparable utility), or a different HD, or try the HD in an external enclosure, or get your hands on some different RAM to try.

hi this is my first time need some help. I ran disk utilty and got this first aid failed invalid directory item count, error filesystem verify or repair failed,it should read 35 instead of 34 mac hd needs to be repaired. I also have a 13 inch mac book and get the same error. Im not sharing the book with the i mac the only thing i did was apple updates to both i wonder if this caused this

Two ways of going about this:
Try a Safe Boot -- hold Shift Key down at startup -- it will take longer than usual to boot as it's checking and repairing the directory, if possible. If it finishes on a screen with Safe Boot in red, you should be OK, but go back and verify again from DU.
Sometimes, the errors reported from DU verify, as it is performing "Live Verification," may be spurious. Better to check with DU Repair Disk booted from the DVD.
Boot from the Install DVD by holding the C key down at startup.
