Disk0: ioctl??? Disk: i/o ERROR?? Help

Yesterday, I downloaded Tech Tool Pro and I just made a Full test to my mac. I dont know what happened, but when I was checking bad blocks it found a few and it just froze!!, I had to do a Force Restart and then I tried again to check the computer and the same happened. Today, I just turned on my Mac (snow leopard) and checked the console messages and there were some disk0: i/o errors, I supposed it was because of the 2 force restarts I did and there is appearing sometimes this message: disk0: ioctl ( ' . I dont have idea what is this. my mac is behaving normal, fast, no freezes, etc.. as usual. But Im just worried, as 2 months ago, my hard drive was failing ( my mac was freezing all the time, there were heard some ticks, etc..) and I had to go to Apple to get a HD replacement, I remember that the apple tech checked on the console application if there were errors, he just typed disk0 and there appeared disk0: i/o errors, so he said that definetily the hard drive was failing.
Im not sure if this is happening again! my mac is really working good, Im just thinking to do a complete reinstall of the OS just to see what happens.
What do you people think I should do??

Try running Disk Utility first and Repair Disk. You will probably have to do this from the SL Install Disc. Here are some instructions:
1. Insert your install disk and Restart, holding down the "C" key until grey Apple appears.
2. Go to Installer menu and launch Disk Utility.
3. Select your HDD (manufacturer ID) in the left panel.
4. Select First Aid in the Main panel.
(Check S.M.A.R.T Status of HDD at the bottom of right panel. It should say: Verified)
5. Click Repair Disk on the bottom right.
6. If DU reports disk does not need repairs quit DU and restart.
If DU reports errors Repair again and again until DU reports disk is repaired.
When you are finished with DU, from the Menu Bar, select Utilities/Startup Manager.
Select your start up disk and click Restart

    First have you got a backup of your data? If not try to get one now while you have some access to the drive. The drive is probably faulty and will need to be replaced, without the backup you risk losing the data.
    Have you a bootable external USB drive with Mavericks on it, if so boot from that and try and repair or erase the drive and reinstall Mavericks.
    If you don't have Mavericks on an external drive,  boot from you Leopard DVD ( I am presumimg your profile is correct) and run Disk Utility from there, you may be able to erase the drive and reload Leopard. You can then install Mavericks. Even if all that works you will most likely need a new drive in the near future.

  • Disk errors help

    I have an external drive that is failing to mount, currently this is my only backup of some information i need. I usually have two but due to apple sending me faulty computers one backup turned into the boot drive and some information has been lost. So i plug in the back up and it will not mount.
    In disk utility in comes up with:

    Checking HFS Plus volume.
    Checking Extents Overflow file.
    Checking Catalog file.
    Invalid index key

    The volume Tabitha could not be repaired.

    when trying to mount via terminal it comes up with:

       That doesn't make sense. The disk need only be attached for dd to access it. That doesn't imply that the output will be usable but it should be accessible. I apologize for this if I give you incorrect information but I'm going to tell some things posted here. I've been fortunate to never have the need to test any of it though, so I hope that someone provides updates for out-of-date information. The thread is from 2003.
       The first thing mentioned in that thread is that the hfs.util command has a "-M" option that force mounts the disk. That might help you but I doubt it, as it appears that vital data holding the location of files has been lost. The hfs.util command isn't in your path but is part of the system. It can be found at:
    /System/Library/Filesystems/hfs.fs/hfs.util
    You generally have to invoke it with the full path. There is also a man page for this utility.
       Naturally to use dd in "raw" mode, you need to copy to an empty disk. If you have enough space, I think you can create a disk image to hold the contents of the troubled drive. "Attached" means that you can see the path to the device file that represents the disk. You said that you can see the listing of the disk in the output of diskutil. There should also be a disk with exactly the same name but with "disk" preceded by an 'r'. That device file gives dd access to the raw data of the disk. In the thread about which I told you, the following command was suggested:
    dd if=/dev/rdisk1 of=/dev/rdisk2 bs=512 conv=noerror,sync
    According to Mac OS Extended Format (HFS+): Default Allocation Block Size, the HFS+ default block size actually varies with disk size but 512 bytes is a divisor of all of the possibilities and it doesn't seem like the block size used by dd should affect the output anyway. The input device file and output device file will have to be altered according to your device tree at the time the command is executed. (it can vary) As I said, I haven't tested any of these commands and Apple has never put a lot of effort into the device tree.
       Again, it's also likely that the output disk will suffer the same problems as the source. There are a number of files that hold the structural data of the filesystem, like the Volume Header, Allocation File, Catalog File, Extents Overflow File and Attributes File. If these are corrupted, the system can "lose" the locations of files on the disk. In such cases, the operating system won't stand a chance of reading files. The app that can fix the greatest variety of such types of corruption is Alsoft's DiskWarrior. If the corruption can't be fixed, your only hope is an application that can identify files by their content. The only one of these of which I'm aware (at a reasonable price) is Data Rescue II. Reports about this app are pretty good. Both apps cost a little but if your time is worth anything, the lost files can easily be worth it.

  • DVD Tiger Disk Permissions on Main HD gets Error HELP

    I have ran TechTool 4.0.4 trying to get my main WD 120gb HD to be able to have the Disk Permissions done by booting up off my Tiger DVD and I get this error:
    Disk Utility Internal Error
    Disk Utility has lost it's connection with the disk management tool and
    cannot continue
    Please quit and Relaunch Disk Utility
    I've tried to do this a dozen times and I still get the same error, I've booted up with a back-up copy of my Tiger DVD and also I've booted from a external FireWire HD with OS 10.4.6 on it and ran Disk Utility and ran into the same message
    I read on the disscussions to:
    1) Move iTunes onto the Desktop.
    2) Run Disk Utility.
    3) Go to HD>Library>Receipts and trash all iTunes .pkg files EXCEPT for iTunesX.pkg and iTunesPhoneDriver.pkg.
    4) Empty Trash.
    5) Move iTunes from Desktop back to 'Applications'.
    Hey presto, you should now be able to repair permissions again."
    That didn't work please help as I've tried CockTail, Onyx many others and when I try to repair disk permissions all the app's just spin
    As I've stated I ran DiskWarrior 4.0.4 and had a couple of minor file problems and also I tried the Command S upon restart with some minor issues

    What I've done recently: Added a ATI 9800 Mac Edition AGP Video card and after I installed it I ran Disk Permissions and it worked fine, this has just started without notice!

    Your post is incredibly confusing.
    First, you should repair Disk Permissions from the copy of Disk Utility that is installed on your boot drive, not the CD?DVD.
    Try that and see if it is able to repair the permission or not.
    If it does then boot from the DVD and run the disk utility to repair the file structure of your disk. You need to run it until you get a message that the disk appears to be O.K. or else a message that it cannot repair the disk.
    If that is the case then boot from your Tech Tool or Disk Warrior CD, whichever you have and use it to repair the disk. Remember that you have to boot from the CD, not just insert the CD and try and run it from ther. You boot from a CD?DVD by installing it in the drive and then restarting while holding down the "C" key until you see the splash screen. With Disk Warrior that can take 10 minutes or so. From the Apple CD/DVD it should only take a couple of minutes.
    Start here and let us know how it goes.
    Also, for now get rid of all your third party utilities such as Cocktail or Onyx until wer get this mess straightened out, just in case the issue is with one of them. If you have any third party haxies like Fruit Menu or others go ahead and get rid of them for now. They can be reinstalled later from your backup copy.

  • Contacts on client machine suffer from disk I/O errors

    Contacts are running slow if not at all on my client machines.  I check the logs and it is having trouble with accessing the files and or writing the files.  all other programs seem to work fine.  Mail hangs but I think it is because of the contacts issue.  It seems to have trouble reading the contacts list and dumps the list and attempts to rebuild from iCloud and the server.  there are many .unknown files in the library.  The logs state that there is a disk I/O error while accessing /Network/myserver/Volumes/MyBook/Users/username/Library/Address Book (this is from memory but it is close).  The contacts work perfectly (as expected) if the user is logged into the server.
    I am running 10.8 server and clients with shared home folders.  the user folders are on an external drive.  the drive does have permissions turned on.  I have wiped out the contacts directory in the  Library folder and it proceeds to rebuild but the problem is still there.
    would rebuilding the clients from scratch help?

    At first I thought it may be related to upgraded 10.8 clients and server i.e. went throught the normal install option without re-installing OSX.
    So I completely rebuild (i.e. erased and then installed Mountain Lion from USB stick) the server and the client machines...but I still get the same issue, so clearly this is NOT the solution.
    I have also observed (upon reading others' issues with Contacts/mail) apple forums - so I followed the the same action (trashed the Address Book folder in ~Library/Application Support/ ) after unchecking everything in iCloud in system prefs/restarting etc.) but this doesn't so solve the problem either
    I also observed teh following behaviours:
    Login into iCloud for the first time - appears to be ok
    Run Contacts - appears to be ok
    Wait the addresses to sync from iCloud - appears to be ok
    Quit
    Run Contacts - still appears to be ok
    (i.e. the errors above are not displayed in log)
    Run Mail (which takes a long time)
    Contacts sudden looses all address and effectively both Mail and Contacts freeeze. It is at this point I start seeing the above errors
    I get a further odd error in Contacts (in a triange with '!' warning)
    Basically Contacts and mail don't seem to want to work together - they appear ok in isolation - obviously this isn't acceptable.
    I've noted that if I create a local logon on any one of the client macs, the issue with contacts/mail does not occur and runs perfectly smoothly. So in my eyes it appears to be a problem in Mountain Lion Server/Client Combo Only (I did not have this issue under Lion Server/Client Combo)

  • Internal Hard Drive Died - Now replaced - Disk Utility Internal Error

    When using the original system disk to install OSX the new hard drive isn't recognised even though I have formatted it for an intel based mac (GUID Partition, Extended Journaled etc) using my imac . When launching Disk Utility from the system disk and trying to erase/partion the drive, I get the following error message "Disk Utility Internal Error - Disk Utility has lost its connection with the Disk Management Tool...". When connected to my imac using a converter it appears on my desktop. Any help would be greatly appreciated. The macbook in question is a 2006 (white). PLEASE HELP.....

    Hi - Thanks for your reply. I have tried that but it still doesn't work. I have had a look around online for the past couple of hours and I found a post that said that newer hard drives can't be recognised by an original 10.4 osx system disk. I would need to find a copy of 10.5 to install OSX on a new hard drive. I dont know if that's true because I dont have a copy of 10.5. Any thoughts would be appreciated!

    Several things can cause the bios to fail recognize the hard dirive.
    0. Try loading bios defaults, save changes on exit of bios, see iff the error persists.
    1. Bad hard drive (try another hard drive)
    2. Bad hard drive data cable (replace cable)
    3. Failed hard drive controller on the motherboard (replace motherboard)

    I am receiving an error message every time I try to repair permissions on my MB running OS X 10.4:
    error:
    Disk Utility Internal Error
    Disk Utility has lost its connection with the Disk Management Tool and cannot continue. Please quit and re launch Disk Utility.
    Tried that several times to no avail. After quittting Disk Utility, system becomes very sluggish and Finder stops responding.
    I then booted from the installation CD and ran a Hard Disk integrity check. It checked Ok. Then I tried a permission repair and got the exact same error message quoted above.
    Any help?

    It seems unlikely that would surface now, but the suggested fix is worth trying. The article also mentions looking at Console log. Console is in Applications/Utilities.

  • Disk Utility: Burn error: 0x80020022 The device failed to respond...

    I've been working on archiving a ton of video to DVD. I'm using Dual-Layer discs to minimize the amount of physical storage space needed for the finished project. I burned several discs in iDVD in July and regularly received errors. Then the drive suddenly stopped loading any type of media. Anything I inserted would spin briefly and then eject. Repaired permissions and followed steps outlined here and here. Took the Mac back to the store where I bought it (sadly, not an Apple Store - nearest one is 12 hrs away) where naturally it performed flawlessly for the store rep.
    So I took it home and resumed burning, now saving as a disc image (ignoring iDVD's warning about the performance of DVDs burned this way) and using Disk Utility, since iDVD didn't seem to be reliable. Burned about 25 discs, error-free, in this fashion, then three days ago, got the error: "Unable to burn “Disc 19.img”. (The device failed to respond properly, unable to recover or retry.)"
    Retried the same image, and it worked. Then burned two more in the same fashion with no errors. Today I got the same error again. Logs are identical for each failed burn, except for the time span between certain lines.
    Price of DL media being what it is, I'd hate to keep ruining discs if there's something I can do to avoid it. I suppose it could be bad media, but it seems odd that I'd get two bum discs that close together after burning so many without issue. Checked the site and the forums, and there doesn't seem to be any answers. Does anyone have any ideas? (Other than "try a different brand of media." I've got a buttload of these discs and it's too late to return them now.) Even if it just helps determine where the issue lies. Warranty will be up soon, so if it's hardware related, I need to get it taken care of quick.

    I have encountered the same problem. I have a Mac Book Pro from 2007, and it has gone through 3 DVD writers so far, all of which have ruined DVD+R DL disks. I have occasionally been able to burn one, but usually during verify, I get this error message:
    Communication to the disk drive failed: 0x8002022
    And disk utility fails with a "Invalid B-tree node size" error
    Oddly, when I try to burn a disk near capacity (6.5Gb or more) it is more likely to fail than if I burn a smaller disk.
    I've been using Memorex DVD+R DL disks, but I have used other brands in the past but they have failed, too. Out of about 50 disks so far, I have had success with about 5.

  • Disk Management Tool error in Disk Utility

    Update: (first, reviewing problem that I and others have been having, including on 10.4.x systems):
    When I attempt to repair permissions on my HD with Disk Utility, I get this specific error message: "Disk Utility internal error -- Disk Utility has lost its connection with the Disk Management Tool and cannot continue. Please quit and relaunch Disk Utility" and the repair-permissions will not proceed. It fails to get the proper "correct" permissions info for that drive to allow the repair. The error log points to a file in the Library called "DiskManagementToll" The path is ~/System/Library/PrivateFrameworks/DiskManagement.framework/Resources/DiskManag ementTool)
    This problem is appearing in many forums here and elsewhere, including 10.4.x systems (like mine) and persists in a few after deleting suggested files. Is there a sophisticated solution that addresses the problem through the DiskManagementTool file by replacing some values or replacing the file from a backup vis SUDO or Root?
    Previous posts have focused on Macs running 10.3.x and with iTunes 6.0x installed. Not here, so the problem is more general. None of the "delete iTunes / plists / Chess.app" suggested fixes have worked for me. Also tried running DU from my Tiger install disk; no luck! -- still fails when it tries to determine the correct permissions.
    The "DiskManagementTool" file apparently either stores the correct file permissions or handles reading them from the receipts in the Library. If I understand things right, Disk Utility is actually a (user)-permissioned graphic front end, while DiskManagementTool is a Root-user-level tool that actually does the heavy lifting. Can I replace -- and any benefit to that -- or rewrite it with proper access?
    There is a complete crash log report in Crash Reporter, by the way. Anyone want the text, and would it help figure out what's happening?
    I've tried the suggested deletion of iTunes 6.0.2 or Chess.app and their plists (these and other steps were suggested on MacFixIt at http://www.macfixit.com/article.php?story=20060111090035797 the advice is, basically, to delete iTunes 6.0.2 and/or Chess.app and related plist files.) As I said, no luck!
    Everyone that I talk to says it MUST be the fault of some other 3rd-party disk utility software.
    For what it's worth, I HAVE run Disk Warrior -- and, the drive that's having the problem is a recent reinstall from a backup using Carbon Copy Cloner. The original disk is NOT having such a problem, only the hard drive on my iBook after the reinstall. Don't know if that gives any clues (or to how the link gets somehow disengaged between Disk Utility and the Disk Management Tool file / software).
    Any thoughts on that? What about replacing the Disk Management Tool file in the Library or forcing some kind of re-link to it? some Unix guru MUST know a fix!
    Obviously, the big worry for me is that I can't update my System to 10.4.5 or anything else without repairing permissions first. This IS a big deal. And others are having the same problem. Help?!?

    Time machine fails backing up with a "backup disk not found" error after upgrade to Lion 10.7.2. I've erased and reformatted the drive; reconnected Time Machine(it saw it) and began a new backup. The backup starts again but fails before completing with backup drive not found error. I used the 10.7.2 combo update. Any help on how to fix this problem would be appreciated.

    After waiting since October for a fix, I have now upgraded the firmware on the Time Capsule and installed the new Airport Utility, released yesterday, and the situation is even worse.
    Up until now, the Airport Utility had an option to disconnect all drives manually and the Time Capsule would then work until the next reboot – a temporary (?) work-around.
    Now that option does not exist in the new-look Airport Utility. And the Airport Utility installation can’t be rolled back and the old utility won’t restore.
    The sparesbundle is still not accessible. Anyone worked out a work-around in the new environment yet?
    I have another Time Machine backup working fine to a trusty old Lacie Drive so erased the one on my Time Capsule to see if that works. I have renamed the Capsule and the Time Capsule Drive.  But to build another full backup will take at least two days and I shall be away from tomorrow and am reluctant to leave the Capsule and computer up and running for a week. And I’ll bet the sparsbundle will still be nowhere to be found.
    How can Apple screw up so badly when they are to become the richest company in the entire world and – soon – will have more cash in the bank than the entire United States? Can’t they afford someone who really can sort this out? Especially when a Firewire connected hard drive – my trusty and simple LaCie – works fine.
    Words, almost, fail me.

  • Logic Freezes During Playback w/ Disk Too Slow Error

    I'm trying to mix a song in Logic 9.1.7, but it has gotten to the point where Logic immediately freezes as soon as I hit the playback button in order to actually listen to my project. Before this I was occasionally getting the Sudden Motion Sensor/ Disk Too Slow error. I have increased the buffer size to 1024 and set the buffer range to large. I haven even disabled the sudden motion sensor, just incase this was the problem. I am currently using a Macbook Pro which is not connected to the interface, so this shouldn't be a problem. However, I use a Focusrite Liquid Saffire 56 interface. The only third party plug-in that I am currently running on the project is the Focusrite Plug-in Suite - which I think might be the issue, considering that my other laptop is runing the same plug-in and having the exact same error. I already tried downloading every relevant software patch and update from the Focusrite website, all to no avail. I have been running Logic along with this plug-in, for over 2 years now and have never once had this problem. I haven't had any problem running Pro Tools, so obviously this problem is specific to Logic. This is incredibly frustrating, and I need this resolved soon. I have a deadline that I must meet by the end of the week.
    Update: I just tried running Logic in 64-bit mode. It played for maybe a second and then gave me an error indicating that the plug-in suite was indeed keeping it from running properly. Guess I was right. It will play properly now, but contantly pops up with errors, not to mention that the HD and CPU meters are both constantly riding the red. I will put the most recent 64-bit error report log before the first one I got. I recieved the first one while still running 32-bit.

  • No Reason For Disk Too Slow Error, any ideas?

    Ok,
    So I've used Garageband for years from iLife 06 and up and I've never had an issue with any Garageband until now. Here's the deal, I still get the disk too slow error, and there is no explanation. Below is my computer config:
    COMP I'M USING:
    Macbook Pro 13 inch
    OS X Mavericks
    Intel Core 2 Duo (1st unibody)
    4 GB of RAM MAX
    BRAND NEW Hybrid 7200 RPM Drive Installed 500GB total, 300 GB FREE
    NO Other programs are open.
    I use Garageband for recording songs but mostly now for recording podcasts. So the track I am using is normally 1 hour in duration. I can't seem to skip around, from minute 1 to minute 54 and edit at my whim without beach ball and timeing out getting the disk to slow error I never had this issue before, I either get the disk to slow error when trying to navigate large gaps of time in the podcast as I said earlier or even during RECORDING a podcast it'll simply say disk to slow, stop recording and occassionally say there are too many tracks in use, when I only record using 1 track of audio.
    How can this be possible? Even my old G4 allowed me to jump all over the timeline with no issues. It seems there is some kind of flaw in the software. And yes, I just restored OSX Mavericks to my brand new Hybrid Hard drive, so there's no explanation as there are no disk errors or permissions errors of any kind.
    I'd rather not hookup an external drive to record too as that seems silly when I've never had too before. If this "modern" computer can't do something that my old G4 ibook can do, don't you think we're going in the wrong direction with this program?
    I've heard of so many people having this issue, and I thought upgrading my OS and getting a new HD would help but it hasn't. I'm at wits end with this issue.

    There are a few other reports of problems with 10.6.7
    Some audio device(s) inputs have ceased working, a few disk to slow messages...etc. I don't see the wisdom of upgrading a working DAW system if that's it main function. OSX has to support a lot of different hardware now, many, many Intel motherboard and/or processor revisions... it's not going to be as smooth sailing as it used to be.
