Do pay monthly phones come with a new sim card?

i want to order a new phone on a pay monthly contract. My old phone is a pay as you go on orange. Will my new pay monthly phone come with a brand new nano sim included?

if you join a new contract then yes you will receive a new sim card.
normally the sim cards are multi sims with 2 or 3 different sizes

Like you mentioned, she bought it from the Vodafone store and most likely the phone is locked to Vodafone. But, in your case since you are using the same carrier's SIM card (Vodafone), "unlocking the phone" is not an issue. As you have already tried restoring the phone (which technically wasn't required) and SIM change, your best option is to take the phone to Vodafone store and ask them to activate it for you.

No. Vodaphone Egypt and Vodaphone Qatar are two different carriers. They have the same parent company, but you can not use a phone locked to Vodafone Egypt with a Vodafone Qatar SIM.

You cannot. The iPhone is locked to Softbank, and only they can unlock it. Problem is that Softbank will NOT unlock the phone, as they do not offer iPhone unlocking.
Buy an iPhone in the Phillipines.
PS Before you go on a rant at Apple, be aware that Apple does NOT unlock phones and they do not set unlocking policies. The carriers (Softbank in this case) set the unlocking policy.

How is iTunes telling you it is unlocked? Did you go through an unlock procedure with your cell company and then Restore it in iTunes?

However a Sim swap is only possible if they are the same size.
If the new Sim is a nano Sim and the other device was a micro Sim or larger size, using the new phone with the new Sim is what you should do.

There was then a notification that the phone would therefor include a sim card. Later in the process when choosing various options, I was advised that I had to pick a "global data plan", the least expensive of which was $30 per month. I was under the impression that the phone just came with the card automatically.

If you buy an iPhone that is locked to one of the carriers, then yes, it will come with the appropriate SIM. If you buy an unlocked iPhone, you will have to get a SIM from the carrier.
