Try doing a restore in recovery mode or what is known as DFU.

Once in DFU mode, plug into iTunes and click Restore.
I know you said you tried all reset procedures, but I want to be sure.
Note: This will delete all your data on the Phone, hopefully you have a backup.

I just want to repeat what was said up thre thread: make sure you report this phone as lost or stolen to your carrier; this will prevent anyone from wiping it out and registering it as their own phone with service.
With ios 7 and beyond ,it's not possible to wipe it out and have it set up as a new phone number unless you know the iCLoud ID and password. THat's great, but for iOS 6 you should report it stolen or lost so that it's worthless if someone stole it or found it.
Your IMEI number is on the box or with the carrier you bought it from. IT won't help you find your phone but you can list it as lost or stolen.

If they are on that list and you have the original receipt from them showing the serial number, they may be able to correct the error in the Apple database.
If they're not on that list, you were sold a black market phone.

You have a password, so good move on your part. Whether you erase it or the thief does, they still get to use the iPhone as theirs. Which is more important (pick one), protecting your data or punishing the thief?
Ringing occurs at the carrier. Chances are you're going to voicemail after only two or three rings. If you have an iCloud backup also a good move. Re-read my question in the first paragraph.

How do I get proof of purchase which has the IMEI number for my phone?

For my insurance I will need to supply a proof of purchase that includes the IMEI number on it.
I ordered my iphone online from the UK Apple store and the invoice does not have the IMEI number on it.
Any ideas on how I can get some proof of purchase with the IMEI number on it?

It has nothing to do with them.
If the phone has been lost/stolen then sure your mobile operator needs to know so they can block it but not your insurance company.
By the way its recorded with in your computer Itunes backup if you want to find it.
For security reasons Apple would never show an iMEi number on a receipt thats likely disgarded.
It is however on the box on a sticker underneath.

It sounds like you are listening to iTunes Radio and it stops shortly after you start listening. I recommend starting by signing out of your iTunes account in Settings > iTunes & App Stores. Then close all the running apps on the device:

Double-click the Home button.
Swipe left or right until you have located the app you wish to close.
Swipe the app up to close it.
When you have done that and restart the phone and test the issue by signing back in to your account, and testing again:

I am going to give my old iPhone to my daughter, and unlocked it officially in the shop. Now when I connect it to my own mac and iTunes, the phone wants to get activated. But does this mean it can only be used with my own computer if I let it? I want to give the phone ready to use to my daughter, or should she do the activation on her own computer?

You can activate it on any computer. For example, if you took it to an Apple store for repair they could activate it on their computer. However, if you sync it on your computer and she then syncs it on hers it will erase any content from yours.
There are still reasons she should activate it, as activation associates it with a billing account and phone number, and also credit information.

Now I am not able to restore my contacts because I do not have a "restore" soft button when I open the backup assistant app. How do I restore my contacts?

Have you updated the dreaded Backup Assistant program? I think you need to do this first before you can restore.

My iphone 5 has a 2 year contract with UK's carrier EE. And recently I want to unlock my phone via IMEI number. But I replace my phone last year in Apple Store due to broken power buttom. And the IMEI of my old phone is missing. I need to use the old phone's IMEI number to unlock my phone from EE.
I just want to know how I can get my old phone's IMEI number?

if you synced it with your computer it should be listed in the preferences of itunes on the computer
otherwise it's on the box of the phone

When i was trying to update my iPhone 5s to the latest version of ios 8.1 , it required more space so i had to delete data manually, meanwhile i was doing so the phone got stuck and went off & when it came on again i found it on the recovery mode ??

here is what happened with me exactly :-
I plugged in my phone into the charger & connected my phone to a wireless connection to start the operation of updating it, I immediately noticed that I dont have the required space to proceed the download so i had to delete data manually, & meanwhile i was doing so my phone became slow and it got stuck then it went off after that it came on again but it was on the recovery mode ??
