apple is not a carrier so does not sell simcards or give them away
if you change carrier you change sim card
if you change phone and keep the carrier you move the sim card to the new phone

Pakistan is not listed for any device. So, I don't think it really matters where you buy iPhone because Pakistan is not officially supported country. However, I have read on several forums that iPhone warranty goes by the serial number regardless of where it was purchased. If you get the phone from a carrier it will come with a nano SIM. If you order direct from Apple you will need to get a sim from the carrier.

I never had a iPhone, but I have been researching that exact question. It seems that Verizon iPhone 5's automatically come sim unlocked. Which is not a full unlock where you are able to delete certain programs you were not able to delete when you first bought the phone. A sim unlock will allow you to use other sim cards for other companies, but you will not get LTE data. According to Verizon spokeswoman Brenda Raney: "she says there's no plans to lock Apple's handset at a later point. In other words, you should be free to use a Verizon-locked iPhone 5 on any compatible GSM and HSPA+ networks for as long as you'd like, including with AT&T and Canadian carriers. Of course, this still brings the caveats of having to both buy a Verizon model, either at full price or with a contract attached, and track down a nano-SIM for the carrier of choice. It could nonetheless settle the question of what carrier to pick if you regularly need a passport when you travel. I believe this article will tell you all you need to know. if not, you can go to verizon if the phone is locked, and have it sim-unlocked, as long as you have a account with them in good standing.

Both Apple and AT&T said that the phone was unlocked. The website imei.info also says that it is not locked. I will verfiy in the near future as I plan to try a different carrier.
It came with an AT&T nano sim card (but when I visited an AT&T store to use it on my curent plan, they exchanged that sim for a different one).

That's a great question Dvidtrrs. I am happy to inform you that yes, you may use your iPhone 5 while traveling out of the country. The iPhone 5 is already unlocked, so you may insert the SIM card from the international carrier and use your device while traveling.

I keep reading in articles online and in these forums that, unless you have unlimited data, Verizon will not charge for mobile hotspot on LTE because of an FCC ruling? However, when I preordered the iPhone 5it wanted me to buy a mobile hotspot plan. So which is it?

On the unlimited data plan, the mobile hotspot is a separate charge with the cost changing when you have a 3G only smartphone or a 4G LTE smartphone. On the Share Everything plans, the hotspot is included. On the individual tiered data plans, I'm not sure how the mobile hotspot is figured in anymore.
