You will need to restore your device as New to remove a Restrictions passcode. Go through the normal process to restore your device, but when you see the options to restore as New or from a backup, be sure to choose New.
You can restore from a backup if you have one from BEFORE you set the restrictions passcode.

Don't know passcode for restrictions

Someone other than myself created a passcode for the restrictions & I'm unable to disable it

If you don't know the restrictions code then was it on the iPad when you last backed up ? If not then you can restore to that backup and it should be removed. If it was on the iPad when you last backed up then the only way to remove it is to reset the iPad back to factory defaults and you can then re-sync your content back to the iPad - you won't be able to restore to the backup as that will keep the code in place.

If the old ID is yours, and if it is an earlier version of your current ID, go to https://appleid.apple.com, click Manage my Apple ID and sign in with your current iCloud ID. Click edit next to the primary email account, change it back to your old email address and save the change. Then edit the name of the account to change it back to your old email address. You can now use your current password to turn off Find My iDevice, even though it prompts you for the password for your old account ID. Then save any photo stream photos that you wish to keep to your camera roll. When finished go to Settings>iCloud, tap Delete Account and choose Delete from My iDevice when prompted (your iCloud data will still be in iCloud). Next, go back to https://appleid.apple.com and change your primary email address and iCloud ID name back to the way it was. Now you can go to Settings>iCloud and sign in with your current iCloud ID and password.

Unfortunately, you can't. There's no way around it. You'll have to get the ID and password used to originally activate the phone or you will not be able to activate it again. If you can't contact them, try to get your money back because you won't be able to use it.

That Apple ID is stored inside each of those applications. If you can’t get back into it, you’ll need to delete and redownload all of them, which may require repurchasing paid ones.

That usually doesn't happen (unless you do not use automatic login) unless you boot into Safe Mode. When prompted do you enter your admin password? What happens when you do that?
