DW4: FLV Video Showing as White Box

Question

I am trying to read through all the past posts, but need to get this out to a client quickly and haven't yet found an answer to my issue.
I had an .mp4 file that I converted to .flv using Media Encoder. I inserted the .flv file per instructions. I now have the .flv file , along with the clear_skin_1.swf file and the flvplayer_progressive.swf file in one folder. The expressinstall.swf and swfobject_modified.js files are in the scripts folder. I have loaded them both to my server. Upon viewing in browser, all I get is just the white box.

Could really use the hold on this. Oh! and just ignore the video underneath...it is a placeholder and altogether different.

Here we go again!!!!!
White box! Happened when I switched out the video and I can't seem to catch what is causing the issue. Help! I feel so weak ;-(
