Dynamic radio problem

Question

I am trying set up a group of radio buttons for an election.
I am able to generate the radio from my query with the exception of
displaying the label, but thats another thing. It will create
separate radio groups for each office title(president, vp,
secretary, etc), but when I submit the form to enter the selections
into the db, it tells me the name of the radio is undefined. Can I
get some info to get me going in the right direction? TIA
