Error code after crashing and reboot

I just went to the genius bar yesterday to have a full system diagnostic (Running MRI and a Storage Diagnostics).  To make a long story short,
i've spent two weeks with system shutdowns, safari crashes, and permissions issues with ITunes.  I've reinstalled a new 1tb hard drive, and
fresh Yosemite install.  I thought everything was fine, then it crashed twice in a row this morning with this code. This is my first Mac, so please take it easy on a
mid forties guy behind the times!

  • Fresh Install (10.9.1) Unable to create OD accounts after crash and reboot

    I have a real big issue and I was hoping if someone could help me out or point me in the right direction.
    I have installed a new Server (10.9.1) for a client and everything has been working fine for over a month now, DNS resolving, users being able to login etc, no issues at all.
    But today (last night) the server crashed and they rebooted it, and today when i try to add a user I get an error:
    existing connection is not authenticated: password change denied
    I OK it then Cancel the adding, but find the user being added (bizarre!)
    When I try to setup the user email on the server for testing, I get an error with password being wrong, and I can't change the user password, with reset password the drop down just stays there and I have to cancel it.
    As a note, all current users within the OD are working fine. And as a test I tried to change a password for a current user and getting the same issue above. They are not mobile accounts, the server is being used for Email, Cal, Card, VPN and AFP only.
    I have had this issue before when installing a fresh server and the way I fixed it was to delete the OD and start again, but this time I have many users with a lot of emails/data so I need to be able to fix it rather than starting again.
    I have checked the DNS, flushed it, rebooted to no avail. Also I have cheked and tried fixes that have been posed on this form but most are talking about migration erros, even so they don't seem to work.
    Has anyone else had this issue? and fixed?

    1. Install:

    2. Change to password:

    3. Run apex_epg_config.sql
    On windows:
    @apex_epg_config.sql (page 30, the guide of intallation)
    Important:Replace SYSTEM_DRIVE:\TEMP by C:
    E.g.: @apex_epg_config C:
    After this, follow the next steps
    4. ALTER USER ANONYMOUS ACCOUNT UNLOCK;
    Finish! Just execute apxldimg.sql script if you is upgrading from a preview release.
    Now try the connect on the browser IE6 o later:
    http://localhost:8080/apex/apex_admin
    Then create your workspace.

  • Random iPhone crashes and reboot loop after iOS 7 upgrade

    Hi all
    Since I upgraded my iPhone to iOS 7, I suffer from random crashes/resets, where my iPhone reboots and enters a reboot loop (Apple logo, black screen, reboot, Apple logo, black screen, reboot, etc) that I can only exit by restoring the device (resetting doesn't help).
    Some more information
    a) The device: iPhone 4, 32 GB, has never been jailbroken, and has never shown any similar issues before.
    b) The crashes/reboots: I haven't been able yet to find a pattern when and why they happen. I had reboots when searching for an app, when pluging in the wall charger, when entering the access code, and even when doing nothing at all (iPhone in my pocket). The only constant is the reboot loop after the crash.
    c) What I tried so far: I restored the device several times, using normal restore (with the iTunes logo) and DFU mode (black screen). After that, I recovered from a recent backup (takes some hours), and everything is ok again - for some hours, until the next crash and reboot loop. I also set up the device as a new phone, and there it seems stable, so I exclude a hardware issue. I tried quite a few of the advices that I found when googling for reboot loops: reset all settings (this led to an another immediate crash and reboot loop), disable app background refresh, etc.
    I've been using this device for my daily business. With the current instability (and the couple of hours it takes to bring it back to life again), it became more or less useless. I wish I could downgrade again (I know that I can't). My assumption is that either iOS 7, or some incompatible app or some corrupt setting is causing this situation. But I don't know how to proceed to isolate and eliminate the issue. I really would like to avoid setting up everything from scratch again.
    Any advice is greatly appreciated!

    First of all, sorry for my half-finished post above. While typing, I was interrupted, and by reflex I hit CMD-S. Unfortunately, the stupid interface of Apple's support website interpreted that as a submit command. I tried to complete my text with the update function, but when finally hitting the submit button, I got an error message, and all my updated text was gone... No way to bring it back, and also no way to delete the half-baked posting. Quite frustrating.
    Now, to complete what I started above:
    In my case, the problem definitely had something to do with the springboard process (an iOS system 'app' that manages the iOS homescreens) and the watchdog process (another iOS system 'app' that monitors certain things on your phone and that can initiate a restart when it thinks the phone is frozen). In the folder '/Users/<your account>/Library/Logs/CrashReporter/MobileDevice/<your phone>' I could see what caused my crashes; they were all 'stack+SpringBoard' and the reason in these crash log files was 'Watchdog timeout'.

    I'm quite sure that my issues had to do with the 'application map' that is mentioned in the above posts. So far, Ive been thinking that it was just too large (as in the case of Andi1996 [second link above]), but jmbladen's observation seems not to support this. So, the absolute number might not the source of the problem, but I still assume that something went wrong with my application map during the upgrade from iOS6 to iOS7. I remember having stability problems before, as soon as all my 10 home screens were full of apps and newly downloaded apps would generate additional pages (only visible via iTunes). Considering that there are some fundamental changes to how we can organize apps within folders in iOS 7, it can well be that the springboard process just wasn't able to cope with my application map correctly within the 120 seconds before the watchdog process restarts the phone. My massive cleanup action must have fixed this somehow.
    @jmbladen: I'm only using iCloud for the 'find my phone' functionality, so all the backup & recovery was done via iTunes from my local harddisk. And there was no temporal pattern for my crashes before. You might suffer from the same symptoms, but probably not because of the same cause.
    My advice for anyone affected by random crashes and a reboot loop is to check out the directory with the CrashReporter logs mentioned above to find out what app or process causes the crash. If it's the springboard and watchdog problem, then you should try to clean up your home screens (from iTunes). Maybe the absolute number of apps is not that important, but make sure that you reduce the number of screens. And then sync and backup before you start using the phone again. In my case it helped.
    Hope this helps someone else too.
    P.S. Once you know the right keywords to search for, you find forum postings back to 2008 / iOS 2 describing exactly the same problem. This reboot loop is a very nasty issue, and it is hard to understand that Apple still hasn't been able to address this, or at least provide some better help on how to get out of it again.

    error 1015 usually means you have a jailbroken  devices, which per the term of use of the forum, you can't get any help here.

    After installing the latest updates for Mac OS X yesterday, my iMac constantly crashes and reboots. It will only boot properly in safe mode. Even when starting with command+R for recovery the kernel crashes. I have had this iMac for about 3 years and never had an issue until last night, when this problem started and it seems to have gotten worse over night. I already tried the <option><command><R><P>, but it seems that that made it worse as well.

    Trying running this program and then copy and paste the output in a reply. Please use copy and paste as screen shots can be hard to read.

    Note the date and time the problem occurred, then go to Applications/Utilities/Console and copy the log starting at the time of the crash. Paste the log in a reply. Please do not use a screen shot as they can be hard to read. If there are repeated runs of the same messages, please post only one run. Be selective about what you post and don’t  post huge amounts.     I have an issue where my iMac crashes and reboots 9/10 times when I log my wife's account off. I'd like to get it resolved as it is very annoying. We switch between accounts all the time and everytime it crashes I lose a bunch of info from my account. Can anyone help me understand how to fix this issue?

  • OSX 10.8.5 crashing and rebooting unexpectedly

    Hi everyone,
    Recently I've experienced some random crashes and reboots. I first noticed it when playing netflix and thought that was the cause; I tested it a few times and everytime I opened netflix the crash occured. However, I stopped playing netflix for a while and the crash occured again. My second reaction was to asume it was chrome so I did the exact same thing with safari- played netflix and random browing- and it crashed again.

    That panic was not caused by third-party software. If the problem is recurrent, the possibilities are:
    A stale or corrupt kernel cache
    A damaged OS X installation
    A fault in a peripheral device, if any
    Corrupt non-volatile memory (NVRAM)
    An internal hardware fault (including incompatible memory)
    An obscure bug in OS X
    You may already have ruled out some of these.
    Rule out #1 by booting in safe mode and then rebooting as usual. Note: If FileVault is enabled on some models, or if a firmware password is set, or if the boot volume is a software RAID, you can’t do this. Post for further instructions.
    You can rule out #2 and #3 by reinstalling the OS and testing with non-essential peripherals disconnected and aftermarket expansion cards removed, if applicable. Sometimes a clean reinstallation (after erasing the startup volume) may solve a problem that isn't solved by reinstalling in place, without erasing.
    Corrupt NVRAM, which rarely causes panics, can be ruled out by resetting it.
    If your model has user-replaceable memory, and you've upgraded the memory modules, reinstall the original memory and see whether there's any improvement. Be careful not to touch the gold contacts. Clean them with a mild solvent such as rubbing alcohol. Aftermarket memory must exactly match the technical specifications for your model.
    The Apple Hardware Test or Apple Diagnostics, though generally unreliable, will sometimes detect a fault. A negative test can't be depended on. Run the extended version of the test.
    In the category of obscure bugs, reports suggest that FileVault may trigger kernel traps under some unknown conditions. Most, though not all, of these reports seem to involve booting from an aftermarket SSD. If those conditions apply to you, try deactivating FileVault.
    Connecting more than one display is another reported trigger for OS X bugs.
    If your system is not fully up to date, running Software Update might get you a bug fix.
    In rare cases, a malformed network packet from a defective router or other network device can cause panics. Such packets could also be sent deliberately by a skillful attacker. This possibility is something to consider if you run a public server that might be the target of such an attack.
    If none of the above applies, make a "Genius" appointment at an Apple Store to have the machine tested. You may have to leave it there for several days. There isn't much point in doing this unless you can reproduce the panic, or if you can't, it happens often enough that it's likely to be repeated at the store. Otherwise you may be told that nothing is wrong.
    Print the first page of the panic report and bring it with you.
    Back up all data on the internal drive(s) before you hand over your computer to anyone. If privacy is a concern, erase the data partition(s) with the option to write zeros* (do this only if you know how to restore, and you have at least  two independent backups.) Don’t erase the recovery partition, if present.
    Keeping your confidential data secure during hardware repair
    *An SSD doesn't need to be zeroed.

  • System crash and reboot since Mavericks install

    Hi all
    I have recently upgraded my OS to Mavericks, on my early-2008 Macbook Pro, and have experienced system crashes and reboots since then.
    The crashes occur when using either of my web browsers, Safari and Chrome, and definitely seems to occur more frequently when using YouTube.  I haven't experienced a crash in any other applications.  I have tried updating Flash, then deleting and re-installing Flash when the upgrade was unsuccessful; not sure if this was relevant or related but I thought I'd give it a try anyway.

  • IPod causing my laptop to crash and reboot when connecting to USB !!

    Hello all,
    Here is what I am working with, 30 GB , 5th gen Video iPod with Itunes 7.6.2.9
    Problem:
    I have had my Sony Vaio laptop with Vista for a couple of months and it had been humming along just fine with my 30GB Video iPod. After loading some new pics via Itunes recently I noticed that the capacity used information in the About section of the iPod was incorrect, my iPod was showing 1gig left, and I only have 500 songs, 600 photos, 3 videos and some podcasts loaded. Itunes showed I had plenty of space left so that contradicted what the Ipod reflected. So after some googling I found that the recommended solution was to restore your iPod and reload all your music back via Itunes. Little did I know that trying to solve that little problem would create a far greater one.
    First using an external hard drive (Western Digital) I backed up all of my music from Itunes. I then started the restore process via Itunes, during the restore my computer hung and froze. Then my computer crashed and rebooted. When it came back up I plugged the iPod back into the USB to complete the restore, this time however my laptop did not recognize my iPod (note, it is empty, with no music since I did the restore). Instead my system assigned it a drive letter G: in the my computer section. A message box pops up asking if I want to scan and check or skip this new drive, it also says scan is recommended. I selected the scan and it checked the Ipod with no problems. If I elect not to scan the iPod then the computer will crash within 30 seconds. After it scans it appear s as if it is going to synch up, iPod says ‘do not disconnect’, then bam, computer crashes and reboots with the black screen. I tried various troubleshooting steps and each time when plugging the iPod in Itunes it starts up and it takes maybe 1 minute trying to recognize it when the computer crashes and reboots automatically going to the black screen and doing a file dump. It seems that no matter what I do Itunes does not recognize the iPod, the system sees it as a mass storage device, hence I am unable to complete the restore.
    To troubleshoot I have done the following without any luck:
    - Removed Itunes (3x, using different versions) and reinstalled it, tried again
    to restore nothing, still crashing
    - Removed and reinstalled Quicktime as stand alone, still same thing. Note: for some reason on Vista you have to have Quicktime installed separate as stand alone and not select the automatic bundle version that comes with Itunes otherwise you will get a separate error that won’t allow you to open Itunes and it will say it’s corrupt. I spent hours on just this problem.
    - Downgraded my Itunes to an older version. Same problem.
    - Upgraded ITunes to the latest and greatest, 7.6.2.9, nada.
    - Reformatted my ipod at the command line, so as to bypass my computer from doing it, plugged it back in, this time it went a little longer before it crashed when Itunes came up.
    - Downloaded and installed all the latest Vista patches and fixes.
    - Downloaded and installed the latest Sony patches and fixes.
    - Re-assigned the drive letter on my Ipod, changed it to “Ipod”, “P”, and lastly “Q”, still shows up on my system as that drive letter, then Ipod. Another time it showed up just as drive letter, and “mass storage device”. Still Itunes is not recognizing it.
    - Plugged the iPod into different USB ports, 3 different ones, still same.
    - Checked each USB port, a sound does come on for each port when either plugging in the Ipod or another device, all ports recognize when a device is plugged in.
    - Made sure no other devices are plugged into the USB ports.
    - Disabled every single USB port via Device Manager, then plugged in Ipod, at that point it didn’t even recognize it.
    - Disabled every single USB port, except for 1, then plugged in Ipod, recognized in computer but not Itunes, then Itunes tries to recognize it, during that time my computer crashes and reboots
    - Disabled and reenabled every single USB port, then tried. Ipod is recognized in device manager but not Itunes. Ipod shows message “do not disconnect” for awhile, seems like its working… then bam, my laptop crashes. By the way, Itunes does come up and all my music is there.
    I also did this:
    Reset the iPod using these steps:
    - Disconnect iPod.
    - Make sure the 'hold' switch is not on.
    - Press and hold the 'menu' and 'select' (circle) buttons simultaneously.
    - Keep holding them both down for at least five seconds.
    - The iPod displays the apple logo and properly resets.
    In summary, I have spent over 30 hours working on this problem. Bottom line is Vista is assigning a lettered drive to the iPod thinking it is a mass storage device, hence it is not being recognized in Itunes enabling it to complete the restore. To complicate matters before you can do much troubleshooting the *laptop crashes and reboots within 3 minutes of plugging the iPod into any of the USB ports.* !!!
    This is just utterly insane, don't know what else to do. For starters stopping my laptop from crashing everytime it is plugged in would be nice !

    I'm curious about this point. Is there any flash of a blue screen at any point here? (That's a more familiar sort of thing to be seeing prior to the system doing a restart after a serious error.)

    Anyone ever get a 2106f Error Code after Internet Recovery Attempt? What next?
    Background: Macbook Pro (late 2011) shows startup progress bar, goes ~ 1/10th of distance, freezes, then goes black after ~2min. Did this:
    1. Clean startup attempt (shift)
    2. Zapped PRAM (option command P R)
    3. Reset SMC, Startup
    4. Attempted Internet Recovery process (Cmd R) over ethernet connection, but after several hours, got the -2106F error.

    How long were you able to watch this movie before it crashed?
    Now is it everytime you boot up, you get the BSOD  0xc0000001 ?

    Do you get an error number? What is the exact message?
    Two things to try.
    - Sign out of the App Store (in the Store menu)
    - Quit the App Store
    - In Finder - Go>Go to folder...  (or Apple-shift-G)
    - Paste in the following "~/Library/Caches/ "
    - It will take you to a folder.  Delete com.apple.appstore
    - Launch the App Store.
    If that doesn't work, try an App Store reset and then try again.

  • Safari crashes and reboots phone

    Hi, I have an issue with an iPhone 3GS 16 GB.
    When opening Safari with Javascript turned ON, it instantly crashes, goes black and reboots phone. This happened after 4.3.3 upgrade.
    Tried
    Rebooting phone
    Empty cache, history and cookies
    Reset to factory settings
    Restore phone again
    Changing language
    When DISABLING Javascript it works fine. Any javascript-enabled page will crash and reboot the phone.

    I use 10.3.9 and I have no idea why yahoo mail stopped working today. I have firefox installed on my machine. BTW - firefox is able to serf the web without issues on all of my sites. I often wonder why apple wanted to build a browser, why not just use firefox and contribute to it's development for the mac environment?

    I just viewed the website on my computer, and it appears to be working okay on my end. It's possible that there's a setting in your web browser responsible for what you're seeing. I would recommend trying the following steps:
    1. Try to view the site in a different web browser to see if that's the issue. Normally, Internet Explorer, Google Chrome, Firefox, and Safari all display the website properly. Try one or two of those (other than the one you've been using) and it should do the trick.
    2. If that doesn't work, try resetting Internet Explorer's default settings by clicking "Tools" (on the top menu bar), "Internet Options" (usually at the bottom of that list), "Advanced" (far-right tab), and "Reset" (towards the bottom-right of that box). After resetting the defaults, try viewing the ePrint Settings area again.

     Hi, I just recently purchased a 785GT-E63 mobo with a Phenom II x4 965. I swapped my old mobo and cpu (nividia mobo +Intel Core 2 duo) with those, and now I can't get into Windows. It posts ok, and then I see the WinXP logo, but it crashes and reboots just as it's about to show the desktop.
    I don't think it's a hardware issue because I can boot up into Safe Mode just fine. It just won't finish booting into normal Windows mode. I've already tried a barebones boot (nothing but the hard drive connected + onboard video), and different sticks of RAM with no luck.
    Any ideas?
