Excel Crashing with PERSONAL.XLSB enabled

Question

I have been trying to get a PERSONAL.XLSB file to work for some time now and it keeps crashing excel. I placed the file in the XLSTART folder under appdata/roaming...etc, and it works fine when I start EXCEL through an old file on the desktop.
However, once I try to generate an excel file through a third party program, excel crashes during the file generation. After which PERSONAL.XLSB is disabled (which I can reenable by going through the addin options). Once I reenable the file I
have the same issues.
Summary: It works on old files, but excel crashes when I am trying to generate an excel file through the third party program, and then it is disabled. 
I am not sure what the issue is, but here is the error log.

Could you tell me more detail information about which third application do you generate Excel file from?
