Folder with question mark when I start my computer... what does it mean?

When I turn on my computer, intead of the apple sing, appears a folder with a question mark.,... and it never changes... help!

It really means that your computer cannot find a valid startup drive. This can occur for several reasons, software corruption, hardware failure, or an incorrectly selected startup disk (although it will eventually find a valid drive in this case). You are going to need to boot from your install disk (or Lion Recovery if you are on 10.7) and use Disk Utility to repair your disk.

    That folder with the question mark icon means that the MacBook can't find the boot directory. That can either mean it can't find the hard drive or the Operating System data on the hard drive is somehow corrupted.
    With 10.4, 10.5 or 10.6 put your install DVD into the optical drive (CD/DVD drive) and reboot. Be sure to either use the disc that came with your Mac, or, if you installed a later Mac OS X version from disc, use the newer disc. As soon as you hear the boot chime, hold down the "c" key on your keyboard (or the Option Key until the Install Disk shows up) until the apple shows up. That will force your MacBook to boot from the install DVD in the optical drive.
    Or if you are running 10.7 Lion, 10.8 Mountain Lion or 10.9 Mavericks, boot from the recovery partition (Command +R on boot) and use Disk Utility to repair your OS 10.7, 10.8 or 10.9 partition.
    When it does start up, you'll see a panel asking you to choose your language. Choose your language and press the Return key on your keyboard once. It will then present you with an Installation window. Completely ignore this window and click on Utilities in the top menu and scroll down to Disk Utility and click it. When it comes up is your Hard Drive in the list on the left?
    If it is, then click on the Mac OS partition of your hard drive in the left hand list. Then select the First Aid Tab and run Repair Disk. The Repair Disk button won't be available until you've clicked on the Mac OS partition on your hard drive. If that repairs any problems run it again until the green OK appears and then run Repair Permissions. After repairing use Startup Disk from the same menu to choose your hard drive for restarting from your hard drive.
    If your hard drive isn’t recognized in Disk Utility then your hard drive is probably dead.

    The folder with the question mark, I'd say about 95% of the time, indicates a failed or failing HD. The exact meaning of the icon on startup is that no valid bootable OS (or startup volume) could be located. To further complicate matters, I've seen machines with failed HDs lock up the system bus and prevent the unit from properly booting to an optical drive when a disc is inserted.
    If you can't startup the computer (with the System Installation disc) inserted by holding "Option" to get to the startup manager, I'd then recommend removing the HD from the unit and trying to startup. You can always "insert" the drive back in on the fly to run diagnostics via Disk Utility. It's tricky to do because you'll have to have the battery removed but it might be worth a shot given where you're currently at in the process.
    Do you have a backup of the data? Is there any data of significant value on the machine, currently?

    When I power on my laptop the screen is just completely white and after a while a gray folder with a question mark in it pops up and starts blinking.

    There are four general causes of this issue:
    1. The computer's PRAM no longer contains a valid startup disk setting when there aren't any problems with the disk itself. This can be checked for by pressing the Option key and seeing if the drive appears.
    2. The internal drive's directory structure has become damaged. This requires usage of an alternate bootable system to perform the repair.
    3. Critical system files have been deleted. This requires usage of an alternate bootable system to reinstall them.
    4. The internal drive has died or become unplugged. This is the most likely case if the computer took a sharp impact or there are unusual sounds coming from the hard drive's location.

    My friend was running MacScan. All the sudden, it was blocked. He waited and nothing happened. He turned it off. Turned it back on, but nothing changed so he took the battery out. When he tried to turn it back on, there was nothing for about a minuted except the grey screen when it turns on until there was (and still is) a flashing folder with a question mark?
    What should he do?

    Hi Antoine;
    Generally the flashing question mark in a folder at boot indicate that the Mac is not able to find a disk with a system on it. It usually indicates problems with the system disk. I would suggest you boot from the install media that came with the Mac. Then run Disk Utility from there to check out the disk.

    Go to System Preferences -> Startup Disk, and check that your startup disk is set correctly. If it is, and you continue to see the flashing question mark folder when booting, try resetting the PRAM.
    It's also possible that your hard drive is failing. Make sure you have an up-to-date backup at all times. Does the S.M.A.R.T. status check out okay with Disk Utility?

    Sorry, didn't read the whole question.   Hold the Option or Alt key down and the boot manager will start giving you the choice of the DVD or disks.

    Long story short, disk utility is unable to repair the "Macintosh HD" disk. Re-installing mountain lion is not an option due to the computer's inability to detect the HDD disk. How do I differentiate between a corrupted (dead) hard drive or a broken SATA cable (in which acts as a medium)?
    Note: my intentions are to fix the issue instead of resorting to apple care.

    If you have a  known functioning boot drive, you can substitute that for the internal HDD.
    You can try to use your current drive externally.  Use Startup Manager and if the external HDD appears, select it.  If it boots the MBP, then that is an indication that the SATA cable is faulty.

    These are related but not identical issues. Their causes are outlined in Intel-based Mac- Startup sequence and error codes, symbols. Solutions may be found in:
    A flashing question mark appears when you start your Mac
    Mac OS X- Gray screen appears during startup
    In most cases the problems may be caused by one or more of these:
    Problem with the computer's PRAM - See Resetting your Mac's PRAM and NVRAM.
    Boot drive's directory has been corrupted - Repair with Disk Utility.
    Critical system files are damaged or deleted - Reinstall OS X.
    The disk drive is physically non-functional - Replace the hard drive.
    Note that the information I have provided is what Apple recommends, If other users suggest different solutions than found here, then be sure what they recommend does not impact on your warranty, if any, or ability to get continuing Apple service.
    Please don't start removing drives or changing cables unless you know what you are doing and have exhausted other non-invasive alternatives outlined here.     The main difference if you are using Lion or Mountain Lion is that you must first boot from the Recovery HD. Simply boot from the Recovery HD to perform the above.

    Download and install Mac OS X 10.6.8 Update Combo v1.1.
    Reinstalling Lion/Mountain Lion Without Erasing The Drive
    Boot to the Recovery HD: Restart the computer and after the chime press and hold down the COMMAND and R keys until the menu screen appears. Alternatively, restart the computer and after the chime press and hold down the OPTION key until the boot manager screen appears. Select the Recovery HD and click on the downward pointing arrow button.
    Repair the Hard Drive and Permissions: Upon startup select Disk Utility from the main menu. Repair the Hard Drive and Permissions as follows.
    When the recovery menu appears select Disk Utility. After DU loads select your hard drive entry (mfgr.'s ID and drive size) from the the left side list.  In the DU status area you will see an entry for the S.M.A.R.T. status of the hard drive.  If it does not say "Verified" then the hard drive is failing or failed. (SMART status is not reported on external Firewire or USB drives.) If the drive is "Verified" then select your OS X volume from the list on the left (sub-entry below the drive entry,) click on the First Aid tab, then click on the Repair Disk button. If DU reports any errors that have been fixed, then re-run Repair Disk until no errors are reported. If no errors are reported click on the Repair Permissions button.     So my imac from 2006 (i think...20 inch plastic intel) booted up today to the blinking folder with a question mark on it. I rebooted into the osx disc to set the hdd as the startup disk, but it didn't show up in the list. Then I tried to see if I needed to repair the hdd using disk utility but the hdd wasn't showing up. So, do I have to replace the hdd? Not looking forward to it if I do.

    I have had similar issues, you will have to restart it and press and hold the eject disk on the keyboard right away and it will force it out.
    Also to answer you question on way you are not seeing the Hard Drive and way you have the folder with question mark on the start up: is because your Hard Drive is dead.
    You will need to buy another Hard Drive install it and then fresh install the OS X.
    I have seen this issue too many times and every time is resulting with bad Hard Disk Drive...
    Look at the bright side, Hard Drives are cheep now and you have iFix it to assist you on step by step installation. (this is unless you have Apple Care).

    Try starting up your Mac while holding down the Option key. That should prompt the Startup Manager window where you can select the startup disk then click Restart.
    The question mark folder indicates your Mac cannot find a system folder to boot from.

    spinning, and then blinked to a black screen that looked like DOS that was
    blinking error messages (one message was "diskOs2 undefined Oxe0030005")

    Was there a storm or outage that could have brought it down? for now it sounds like a kernel panic screen is what you are seeing. If you don't have a UPS, I wouldn't be w/o. Never know when one of those high winds will knock out power.
    You could try to do an SMC Reset and see.
    I assume you usually sleep the system and it doen't have trouble with wake from sleep, and you haven't altered the system (new hardware etc) in any way that would change that.
