I forgot my restrictions password.  What will change if I have
to restore the factory settings?

Restoring to factory settings, does just that... your iPhone will be like it was when new and not activated, i.e. everything will be wiped off the phone.

    I would say yes. I once had the same problem (iPhone 5, IOS 7) and first restored to factory settings and after that from iCloud backup.
    It worked very well.

    Sounds like then maybe a "friend" got hold of your phone and set a SIM card password.  Not much you can do if that is the case if you do not know the password except go to your carrier and request a new SIM card.

    If you can't remember the restrictions passcode, the ONLY way to remove it is to restore your phone as a "new" device in iTunes. That will erase all data on the phone, including the restrictions passcode. If you try to restore from backup, you'll merely continue to restore it, as the restrictions passcode is included in the iPhone backup.
