Form Failed To Submit Error

I have received the error "Task 6004 has failed to submit. (ALC-WKS-007-072)." The form rendered fine and the webservices worked fine. It's just when I clicked complete I got the error. I haven't changed anything and don't know why it's doing this. I have copied my log entry from the server log.

Thank you in advance!

Are you submitting from a user step within a workflow?  If so, is the process variable housing your form using a custom submit service?
Additionally, how is your form's submit button configured?
If you'd like, feel free to attach your form or email me a copy and I'll take a look at it!

    Hi,
    I think the problem is with the node that u have mapped to the data source property of the Interactive Form UI element. Create a element of the node(which u are mapping to the data source property) or set the cardinality to 1..1 or 1..n, for that node.
    Try this and let me know....

  • Some forms failing to submit

    We recently distributed a couple forms, most submissions were sucessful, however a 2  had issues. In one case the submit button triggers a Send Email dialog box, in the other case the button triggers no action.
    On the form where an email is triggered, under properties, the PDF producer is listed as Mac OS x 10.8.4 Quartz PDFContext
    PDF version 1.6 (Acrobat 7.x)
    On the form where nothing happens, the PDF producer is listed as Mac OS x 10.8.3 Quartz PDFContext
    PDF version 1.6 (Acrobat 7.x)
    In both cases the files had been renamed, but other sucessfully submitted files have been renamed.
    How can I determine what is actually causing the problem? I suspect it is because the files were opened in something other than Reader and then saved, stripping out functionality. Is that the case? How can I tell? And more importantly, how can I prevent it in the future? If that is not the case, what is the likely issue?

    Based on the PDF Producer info that you provided, it is most likely caused by Mac users using the Preview application instead of Adobe Reader to save the forms. Preview corrupts PDF forms in a number of ways and it should not be used. Apple doesn't seem interested in fixing it.

    I believe my it guys have php on their server. When I look on the internet
    to try and use the form to test it, I get the error:
    The website cannot display the page
    HTTP 500
    Most likely causes:
    The website is under maintenance.
    The website has a programming error.
    What you can try:
    Refresh the page.
    Go back to the previous page.
    Here's one php code I have linked to one of my documents:

    * QVCS Version: $Revision: 1.4 $
    * Read This First
    * ~~~~~~~~~~~~~~~
    *  This script is very well documented and quite large!  It looks daunting,
    *  but really isn't.
    *  If you have experience with PHP or other scripting languages,
    *  here's what you *need* to read:
    *      - Configuration (TARGET_EMAIL & DEF_ALERT)
    *      - Creating Forms
    *  That's it!  (Alternatively, just read the Quick Start and/or
    *  Quicker Start section below).
    *  Full configuration documentation is here:
    *      http://www.tectite.com/fmdoc/index.php
    * Purpose:
    * ~~~~~~~~
    *  To accept information from an HTML form via HTTP and mail it to recipients.
    * What does this PHP script do?
    * ~~~~~~~~~~~~~~~~~~~~~~~~~~~~~
    *  On your web site, you may have one or more HTML forms that accept
    *  information from people visiting your website.  Your aim is for your
    *  website to email that information to you and/or add it to a database.
    *  FormMail performs those functions.
    * Quick Start
    * ~~~~~~~~~~~
    *  1. Edit this file and set TARGET_EMAIL for your requirements (near
    *      line 235 in this file - replace "yourhost\.com" with your mail server's
    *      name).  We also strongly recommend you set DEF_ALERT (the next
    *      configuration below TARGET_EMAIL).
    *  2. Install this file as formmail.php (or other name ending in .php)
    *     on your web server.
    *  3. Create an HTML form and:
    *      - specify a hidden field called "recipients" with the email address
    *        of the person to receive the form's results.
    *      - in the your form tag set the action attribute to
    *        the formmail.php you uploaded to your web server
    *  Once you have FormMail working, you may be interested in some advanced
    *  usage and features.  We have HOW-TO guides at www.tectite.com which
    *  describe many of the advanced processing you can do with FormMail.
    *      http://www.tectite.com/fmhowto/guides.php
    * Quicker Start
    * ~~~~~~~~~~~~~
    *  Use the FormMail Configuration Wizard here:
    *      http://www.tectite.com/wizards/fmconf.php
    *  By answering a few questions you'll get a configured FormMail and
    *  a sample HTML form ready to upload and use on your server.
    * Features
    * ~~~~~~~~
    *  For a list of features go to: http://www.tectite.com/formmailpage.php
    * Security
    * ~~~~~~~~
    *  Security is the primary concern in accepting data from your website
    *  visitors.
    *  Tectite FormMail has several security features designed into it.  Note,
    *  however, it requires configuration for your particular web site.
    * Configuration
    * ~~~~~~~~~~~~~
    *  To configure this script, go to the section titled "CONFIGURATION"
    *  (after reading the legal stuff below).
    *  There is only one mandatory setting: TARGET_EMAIL
    *  and one strongly recommended setting: DEF_ALERT
    *  Full configuration information is available here:
    *      http://www.tectite.com/fmdoc/index.php
    * Creating Forms

    *         in writing with Root Software allowing you to do so; and,
    *      3. if you modify FormMail before distributing it, you clearly

    *              a) who you are
    *              b) how to contact you
    *              c) what changes you have made

    * Version History
    * ~~~~~~~~~~~~~~~
    *  Near the top of this file, you'll find its version. The version
    *  line looks like this:
    *       $FM_VERS = "N.MM";     /* script version ...
    *  The version history used to be located within this file.  However,
    *  starting with Version 8.00 we've moved it...

    Please help me, Thank in advance.
    Sravan.

    Hi Sravan, this means that one or more of the fields are required to be filled in or have validation set up on them so that the information entered matches a certain criteria. Make sure what's entered matches what's required. If the form still can't be
    submitted, check the rules set in InfoPath and the list/library to see what's causing the error(s).

    I haven't used  Matt Wright's scripts in this century, but I'm pretty sure that Perl still needs to go into your CGI bin and the permissions level (chmod) must be set to 755 or possibly 777 for the script to read, write & execute.

  • Forms fails to reload a form when run locally

    When I'm working on a form in Forms Builder (9.0.4.0.19) I can log on to the database and run the form locally with OC4J running. When I run the form the first time I'm prompted to log on to the database and then it loads the form correctly in Internet Explorer. But, when I shut down that form instance and try to reload it (with or without any change to the form) it fails to load and I get the following error:
    <html> <head> ORACLE FORMS.</head>
    <body onload="document.pform.submit();" >
    <form name="pform" action="http://markc.cis.cuny.org:8889/forms90/f90servlet" method="POST">
    <input type="hidden" name="form" value="U:\mvcbh\ACE_Enrollment_form\ace_enrollment2.fmx">
    <input type="hidden" name="userid" value="ABCDEF/12345@database">
    <input type="hidden" name="obr" value="yes">
    <input type="hidden" name="array" value="YES">
    </form> </body></html>
    The error loads in an Internet Explorer window and references D:\documents and settings\mvcbh\Local Settings\Temp\s35o.1htm. The Oracle Client and Oracle Developer are installed on the C drive on this machine but our IT folks moved the default "documents and settings" folder to the D drive, so.... FWIW, I wasn't getting this error before I was given a new computer so whatever is causing this must be a configuration error--I just can't find it.
    Can anyone tell me what I need to do to have Forms Developer load a form more than once? Having to shut down and restart OC4J and Forms Developer every time I want to load a form is a real pain....
    Thanks,
    Mark

    This is not necessarily an error. This is the result of Microsoft adding new security to its browser. What you are looking at is the html code used to launch your form.
    To correct the problem (assuming you are using IE6) do the following:
    1. In the Builder, verify that in the Preferences > Runtime tab you have both the Application Server URL and Web Browser Location filled. Do not leave either blank.
    2. Close ALL open browsers except one.
    3. Select Tools > Internet Options > Advanced
    4. In the Security section near the bottom, CHECK the box labeled "Allow active content to run in files from My Computer.
    5. Click on OK and retest.
    You can also disable this creation of the temp files which actually cause this problem in the first place, but this will expose the complete URL when you run a form. In other words the username and password will be in plain text in the URL. If this is not a problem then you can add the following to the registry in the area which contains the Developer Suite information:
    FORMS90_HIDE_OBR_PARAMS
    Set the value to 0
    It also looks like you are running your forms from a network share. This is not really recommended. Try copying the form to the local machine while testing and developing.

  • I am trying to skip logic a page, but my form fails to do so

    In my form, the deadline to submit an abstract has passed, so I would like to skip the pages which ask for contributed talk information.  Yet, in every iteration, the form fails to skip those pages.  I've tried everything, and nothing works.
    The only way I could hack a solution is to go to all of the different fields on those two pages, go "Show/Hide" on each field, and then hide if "any" condition was met, where the condition was a Yes/No question from before (and I hide if they said either of Yes or No).
    If I were to guess, it looks like since the form has already accepted information from those fields, it refuses to skip over those fields for future submissions.  But this is a problem, of course, because I don't want to write a brand new form that includes everything BUT those two pages. (And then later have to integrate the two separate databases.) The whole point of "skip logic" is for a user to "show and hide certain parts of a form", right?
    This smacks of a bug, as far as I can tell.

    Dear Brian,
    Thanks for responding, and I appreciate the suggestion.  I think I am
    following exactly what you are saying, and I agree with you.  The
    details: the pages with the "submit abstract" fields are on pages 6 and
    7.  I thought I could, at the end of page 5, say, "Skip directly to
    page 8".  I clicked the correct button at the bottom, I said "Skip
    directly to page 8", yet, the form fails to skip.  I tried it on the
    "Test your page", I tried it on the live form, I tried everything.
    I clicked every button on the Adobe Forms website associated with the
    form, and no matter what incantation I pick, I can't get pages 6 and 7
    to go away.
    I believe that I am correctly interpreting what Adobe advertises as
    "skip directly to page", but it simply isn't working with my form in
    this particular case, and for what reason I have no idea.  I wanted to
    submit a bug report but the only thing I could find on their website
    was "Start a discussion."
    Even though I can't make the entire pages go away, I managed to get the
    fields to go away by that "show/hide" trick, where I am hiding the field
    regardless of what the user enters previously.  Clearly, though, this
    isn't the right way to make pages 6 and 7 go away, and if I knew what
    peculiarity about my form was rendering 6+7 inviolate, I would change
    it.
    I was a bit surprised, frankly, because up until now everything about
    the form (and FormsCentral in general) seemed pretty slick - I was
    impressed.  Now, in the moment that I need these pages to go away, here
    I am, hacking again.
    If you'd like to see the form in action, you can find it at
    http://ohiomaa.org/register/
    Thanks again - I don't know that there is a solution here, and even if
    there were, it's going to be too late by this Friday anyway.
    Regards,
    Jay

  • How to distribute a form with multiple submit buttons?

    I have a form that has multiple submit buttons depending on the information that is being entered and then who the information should go to.  When I try to disbribute the form, I get an error message stating the following, "Acrobat is unable to distribute this form because it contains multiple submit buttons with different formats or return URLs."  Is anyone able to let me know how I can distribute the form?

    Sounds as if you need your own web script to process submissions - written by an experienced web programmer (I very strongly recommend someone familiar with all the latest security weaknesses, and not a project for anyone else). Your needs are (it seems to me) too complex for Adobe's automatic system.

  • File Attachment Upload - submit error in Ipad

    Hi,
    I created a webform for my staff to log visits to physician's offices.  The form includes a file attach button which works great from a PC.
    I downloaded ICab browser app for ipad, and it actually lets me choose file from ipad photo stream/camera roll.  However, when I hit "SUBMIT" button on form, they get an error that reads:  "An error has occurred.  There was an error communicating with the server.   Please try again later."
    Any ideas as to why I am getting this error when submitting form with file attachment from Ipad?
    Thanks !

  • CMP bean : 'create' method fails - SQL Syntax error

    I'm using SilverStream 3.7.3 server on Win 2K.
    In a CMP entity bean, call to any create method ( default using primary key or with any other list of parameters ) fails , giving this error :
    javax.ejb.CreateException: [Microsoft][ODBC Microsoft Access Driver] Syntax error in INSERT INTO statement.
    <<no stack trace available>>
    I've doubly checked for mismatch between types of database columns & parameters passed, but there is no difference. ( Another BMP bean uses same types and it's working perfectly )
    Interestingly, the 'finder' & other business methods are working without any problem.
    Any idea about cause of this error / options to solve / get around the prob?
    TIA,
    Subodh

    Hi,
    See if you can get SilverStream to give you the JDBC statements it is sending to Access. If SilverStream can't do that, try using P6Spy to get hold of the JDBC. Once you have the SQL, you should be able to figure out where the prolbem is... and if you can do anything about it.
    Does access actually accept any form of Standard SQL?
    Daniel.
