Form template with tick-boxes

I'm an Indesign novice and would like to create a questionnaire form which is to have tick boxes next to some questions.
I intend to print out the document and have people tick with a pen if the question applies to them etc.
The document will also have some other text of course, but I think I can handle that part. It's just the boxes and alignment next to each line I don't know much about, so a template would be helpful.

I'm using InDesign CS4 on a Mac, but there's no font by the name of "Garamond premier pro". The closest I get is "Adobe Garamond pro" -is that what you meant?
Since I have a lot to learn I ordered the Real world Adobe InDesign CS4 book. From what I hear it should be good for newbies and more experienced users alike, but until I receive it I need to figure out how to do some other layout styles in my document. It would help immensely if I could download one or several templates with pre-defined paragraph styles suitable for a questionnaire. Any websites where I can get that sort of thing?

Form template with lines that shouldn't print are printing

I have Windows 8.1 and Adobe Reader 11.0.09. I have had a template from my employer that we use to create address labels. There are lines for name, address, etc that are supposed to be invisible when printed, but that has not been the case the past few times I have printed.

Ask the author of the file to double-check their work. There's nothing you can do about it yourself if you only have Adobe Reader.

Designing Form Template

Our company just purchased CS2 with the expressed intent of creating interactive order forms. Unfortunately, we do not have reference books or users guides for any of the software packages. I've been asked to create an order form template with fillable "quantity" fields. This template will be used to create forms that contain data from an ODBC data source. The end result should be customers who can view these files via Reader, fill in the form, and submit via email.
Unfortunately, I am not a designer and I have no experience in scripting. Does anyone have a suggestion of tutorials or instruction on how I can accomplish this? I have no idea where to start on this project.

A great place to start would be the samples that come with Form Designer. There's a sample called Purchse Order that has a lot in common with what you're going to create.
There is a tutorial that comes with Designer as well that has you creating a form like this from scratch. The window that appears when you launch Designer has a "QuickStart Tutorials" link that you can click to take you to the tutorials - Purchse Order is one of the tutorials.

Issue with 'text box' in rtf sub template for peoplesoft XMLP

Hi all,
i have a weired problem using 'text Box' in a sub template. I am calling a sub template from my template. In my Sub template i have a text box ( i am using text box bcoz i want to justify the text not to the page margin but starting from a specific column.not sure if there is any other way with which i can do the same !!) . It works fine when i remove the text box from my sub template. I tried to put the text box directly in the main template and it worked fine !! it is erroring out only when i use the text box in sub template !!

i got the below error when i tried to 'preview' the report from my report definition.

How can I add extra tick-box columns to the Evaluation report template?

Is there a simple way to add additional tick-box columns to the evaluation report template? I want to have a column for every day of the week.

Are you part of a class project? We've just had another query on this template.
+Menu > View > Show Ruler/Invisibles+
The "columns" are just boxes and text aligned with tabs.
Select all of them and slide the tabs on the ruler across to tighten up the spacing.
Add more tabs to the ruler by clicking on the ruler where you want them, then right clicking on the Tab markers to change them to the type you want.
Copy and paste extra copies of the +tabs plus box+ in each line.

Has anyone had any problem with the multi choice tick boxes on a windows computer?

I have just created a form with check boxes on my iMac, emailed it to a windows based account and found that the check boxes don't work in windows explorer. Has anyone else had this problem?

If your Mac is truly running MacOS 10.2.x, you are limited to using 128-bit WEP security - WPA is not supported.

Adding a tick box on checkout form to let people opt out of email listing

hey guys ive set up a shopping site and have added an extra tick box at the bottom of the check out page to let people opt out the emailing listings if they choose to.
Problem is I dont think the result of the tick shows up on my orders invoice inside the admin or email invoice sent to me when an Order is placed, meaning I dont know if I have permission to send out emails to old customers in the future.

You can add {tag_formsummary}to the workflow notification email under system emails. You will then get the complete webform summary in the notification email.

The soluti as an image into on to your problem can be to use the symbol icon as told , but this may show in preview not in print.
I too faced the same problem.
So what i had done is created a new window with the diemnsions of the chekc box with border thcikness 8 tw and inside that window place a checkmark symbol.
if this checkmark symbol also does not show up in print preview/ doesshow in print preview and not in print then you can scan a chekcmark image and upload it into business document server as a BMP/ TIFF image and use this image in your checkbox window.

Help getting a form with check box capabilities?

Hi all,
I built a new site using Muse, which I am very happy with so far, but I am finding the forms a bit limiting. The previous website, which I am replacing with the new muse site, was built using Dreamweaver. The old site had a form with check boxes and it pointed to a PHP directory. Is there a way to acheive the same results in Muse? Either a form widget update (without having to use BC), or a simple way to configure Muse to interact with my PHP directory? I'm not a coder, but I can manage some light work.

Thanks Morgan! Jotform is in the realm of possiblities; however, the company the site is for has a very strict privacy policy, which might be a concern when looking at free form builders. I should have mentioned that before.
I beleive the best way to acheive what I'm trying to do is by linking the existing PHP directory to the new Muse site. The existing form built in Dreamweaver works fine, I just need to get the same form to work in Muse. Is there a way to do that?

I'm unable to clear my search history after many attempts with the firefox instructions as I'm unable to check the "Form and Search History" box for some reason

I've tried several times to clear my search history without any success. I've been able to do so in the past, but this is the first time I've tried in a while and for some reason it will not let me check the box for "Form and Search History." Then sometimes it will let me check that box, but when I click "ok" to apply the clearing of the search history, nothing happens and it still remembers all my sites. At that point I try to clear it again and its back to not letting me check the "Form and Search history" box (I've gone through this cycle a few times). I tried looking through some of suggested articles that Firefox sent me too, but I don't know much about computers and just doing the things they seem talking about isn't working either. Please help, thanks!

Then I exported this template to the FIM Portal server, in the "Email Template" administration section on the Portal.
Where must I put the image to allow FIM to link it? Which attribute have I to modify with the correct path (src?)? And how?
Can you provide me some examples of the correct form of the path attribute?

The problem with using "cid" for embedded images is that the email message needs to be formatted differently. You don't have control over the header, mime type and other aspects that need to be defined to include and reference a "cid image" in the message
body.
First, the message header will need to have the following fields:

How to automatically populate text boxes from dropdown and tick box options and then sum the results

Firstly I need to say that I have no knowledge of how to use Javascript.
I am using the 30 day trial of Adobe Acrobat XP Pro to see if it can be used for a Membership Application Form for the 1st year of membership. This 1st year has a pro-rata membership fee per month left in the year of application along with a pro-rata postage fee for non-UK members. All of the input will be via basic text boxes with the exception of the application date and overseas postage for which I have created 3 dropdown boxes, one for Day, one for Month and one forYear and for the 2 tick boxes which are there for the applicant to say if they want 2 other options which have a cost attached to them. I want to populate 2 text boxes from the Month lookup. One of these is for the pro-rata membership fee and the other for the pro-rata overseas postage. The other 2 optional items will be text boxes populated from the tick boxes. Having populated these 4 text boxes I then want to have them automatically summed to give the total to be sent by the applicant.
I have searched many forums and tried many of the suggested ways of doing it and none of the 4 text boxes are populated.

I have an InfoPath form. It is a checklist with 20 tick boxes. I want to be able to update a status field once all those boxes have been ticked and the form submitted. What is the best approach to this?
