What is the difference(s) between the FormsCentral Plus (11.99 mo) and FormsCentral w/Acrobat (19.99 mo) packages? What additional features would I gain from the 19.99 version?

FormsCentral Plus w/Acrobat has the same level of service for FormsCentral as the FormsCentral Plus option and also includes two additional services - Create PDF (or Adobe Create PDF) https://www.acrobat.com/createpdf/en/home.html to create PDF files from many file types, and Export PDF https://www.acrobat.com/exportpdf/en/home.html to export to other file types from a PDF.
The features in FormsCentral in either plan are the same, the additional $8 gets you two additional services.

Acrobat lets you created PDF forms that can be used with FormsCentral, and Acrobat 11 comes with the desktop version of the FormsCentral designer application, which can create PDF forms. Acrobat does not include a subscription to the FormsCentral service, so that is separate, but it does come with Acrobat Plus: https://www.acrobat.com/acrobatplus/en/home.html
Acrobat is not included with a FormsCentral subscription and you do not need it for the full capabilities of FormsCentral.

FormsCentral Plus Activation

Hello! I purchased FormsCentral Plus (in addition to a full download of Acrobat 11) but still see the same Forms Central interface when I open up FormsCentral on my desktop. Am I missing a step in terms of activation?

When you launch the FormsCentral app. be sure to log in with you Adobe ID by clicking the "Sign in" button in the upper-right. You can then click the "Create New Form" tab in the upper-left to create a new web form or import a PDF form that you've created.

Unfortunately, no. But Adobe does offer a free 30-day trial of Acrobat (absolutely no obligation). So, you could download the trial to fix up the form. For more information, see www.adobe.com/products/acrobat.html. Acrobat is also offered on a subscription basis.

If a form is set up on FormsCentral Plus for a client by an external consultant, does the client need to also purchase an individual license?

Not necessarily. You can add them as collaborators, they just need to create/use their own free Adobe ID.

Difference between FormsCentral Plus and Adobe Acrobat Plus

I am new to creating PDF forms, and have a business need to create a form to send to 10,000 customers. Under the benefits of Adobe Acrobat Plus, it says "Publish forms and view results in real time." Is the AAP the only way to get a URL to provide to my customers? I want to make it as easy as possible to access and complete the form electronically.

Acrobat Plus includes a subscription to the FormsCentral service. Acrobat lets you create PDF forms that are not based on a form you create in FormsCentral, so you have much greater flexibility. The big advantage of using a PDF form with FormsCentral is the secure and reliable means it provides for collecting form data (but not the completed PDF) from an unlimited number of submissions. The down side is some features available with PDF forms are not supported by FormsCentral. If you create a PDF form, you are free to make it available on a web site or by email, they are not hosted on the FormsCentral servers as web forms are.
Note that Acrobat is able to Reader-enable PDF forms so that Reader users can save a filled-in form. The problem is if you distribute an enabled form to more than 500 recipients (as yo do when you post it on a publicly accessible web site, for example), you are limited by the Acrobat license to using data from no more than 500 instances of the form that has been returned to you, including hardcopies. When a PDF form is enabled with the FormsCentral service, there are no such limitations.

Our company subscribes to Adobe Creative Cloud. We are thinking about getting a FormsCentral account. Can we subscribe to the "Plus" version ($11.99/month) instead of the "with Acrobat" version ($19.99/month) and receive the same features as the "with Acrobat" version since we already have Acrobat?

If you already have Acrobat then you just need the Plus $11.99 version.

Why does my Adobe FormsCentral tell me to upgrade?

I am currently experiencing some frustration with Adobe FormsCentral. When I am creating my form, many of the options say 'upgrade subscription' in order for me to use them. When I click the option, it tells me to either upgrade to the 'Plus' option for $12/mo or to purchase Acrobat Pro which will include FormsCentral. However, I already own Adobe Acrobat XI Pro. In fact, if FormsCentral had not been included in my Acrobat installation, I would never have learned of it.
Can someone shed some light on how I can activate FormsCentral or why it isn't working for me?

EDIT: To clarify, I seem to be confused between the following two web pages:
Subscription Options - Free, Basic, and Plus | Adobe FormsCentral - This one indicates that Acrobat Pro is the ultimate version with unlimited everything. This is the up to date one that is linked everywhere

Here's my confusion.
On the Acrobat 11 subscription page I see this copy: "A FormsCentral Plus account and an Acrobat.com account are included with subscriptions to Acrobat Pro only when purchased in select countries."
I placed an order for the full download at $449 and was under the assumption that I would get FormsCentral Plus as well.
Does the download NOT get FormsCentral Plus? If so, seems a bit harsh as we paid $449 up front (I know you're not personally responsible for this policy, just thought I would pass this on).
Would you advise that I talk to customer service to see if I could get FormsCentral Plus added at no extra charge?

it clearly says

But doesn't list the countries and it sure as hell isn't part of ACROBAT PRO subscription anywhere I can see.

If you already have Acrobat XI then you could go with either the FormsCentral Basic or FormsCentral Plus plan. You do not need the Adobe Acrobat Plus plan since you already have Acrobat XI. FormsCentral could not import the PDF document because it does not contain interactive form fields. You need to add fields to your PDF using Adobe Acrobat and import again

Adobe FormsCentral can't import "flat form" (one that isn't fillable) and convert it to a fillable form. You would need to use Acrobat to add fillable fields to the PDF. If you don't have Acrobat, please feel free to give it a try. You can download a free 30-day trial from http://www.adobe.com/products/acrobat.html.
