Help please -SQL

Question

hey,
My cardType is entered into the Title Table in the database for some reason
I can't see how its doing this.

I have 2 groups of radio buttons and they're both different.
one group of radio buttons is Title which are Miss Mrs Ms Mr
and the other one is cardType which are Mastercard,American Express,Other etc
So when i click on say, Mr for Title
and later
click on American Express for cardType
American Express is entered into my Title column and cardType column, instead of Mr in Title column and American Express in cardType column.
