Help to create LOV

dear all
I create a simple List of value (LOV) but whem I'm try to execute it I get this error plz
plz help me out
error message:
Column Mapping
FRM-30049: Unable to build column mapping.
LOV LOV10
Form: MODULE53
FRM-30085: Unable to adjust form for output.

I create this LOV manualy
1- I create a new Recored Group with name (RECORD_GROUP9) .
    the query in this Recored Group is ( SELECT ENAME FROM EMP)
2- I create a new LOV manualy
    with properties :
    - Recored group : RECORD_GROUP9
    - Column Mapping Properties
       column name (ename)
       return item ( BLOCK3.TEXT_ITEM4)
       display width ( 1 )
       column title (Emp Name)
       Automatic Position (Yes)
3- In the Data Block ->  Block3 -> Items -> TEXT_ITEM4
    I create a new trigger ( whem mouse click )
    this code :

     v_empno number;
     v_job varchar2(20);
     v_sal number;
     v_deptno number;
begin
     flage:=show_lov('lov10');
     select empno, job, sal, deptno
     into v_empno, v_job, v_sal, v_deptno
     from emp
     where ename=:text_item4;
     :text_item5:=v_empno;
     :text_item6:=v_job;
     :text_item7:=v_sal;
     :text_item8:=v_deptno;
end;
I also change the property in the TEXT_ITEM4  i change the LIST OF VALUE (LOV) property to
LOV10am I forget something after that ???
