Help with submitting form

Question

Can anyone please tell me why the 'Name' part of the form on this page doesnt show in the email that i recieve when a visitor submits the form.

oops, i see your issue... you are not using "."
Basically you are overwriting the body
change:
$email_body = "Request from: $Name.\n";
$email_body = "Email: $donerEmail.\n";
to
$email_body = "Request from: $Name.\n";
$email_body .= "Email: $donerEmail.\n";
