How can I allow users to submit a form without entering credit card info?

Hello all,
We are offering a free gift to our constituents. So, what we need is a form that will allow users to either claim the gift and make a donation, or claim the gift and not make a donation. Ideally this is one form. Is there a way for users to fill out everything but the credit card info?

Hi Mike,
You can use "Free" payment method to do so and hide the credit card fields using JS and CSS based on the payment method type selection. Please refer to the following article : http://kb.worldsecuresystems.com/893/bc_893.html

1. Sign out of current Apple ID if you are sign-in to one (important)
2. Go to App Store and select a free app
3. Tap INSTALL APP
4. Create New Apple ID
5. Confirm Your Country
6. Agree with Terms and Conditions
7. Fill in your Apple ID and Password (you must create a new Apple ID; don't use your old Apple ID)
8. Create and answer your secret question
9. Select NONE for Payment Method
10. Fill in Billing Address
11. Submit application for new Apple ID
12. Wait for verification email
13. When email arrive, verify your account
14. Start downloading your free apps

Read the steps carefully as the order in which you follow them is critical. Note that you can do this only when creating a new Apple ID. You cannot use an existing ID.
You will of course not be able to get anything other than the free apps without entering in some sort of payment method (credit card, prepaid iTunes card, gift certificate, etc.)

No, you don't need to use LiveCycle Designer. To create a form, first create the PDF and open it in Acrobat. You can then add the form fields by selecting: Forms > Add or Edit Fields
It will ask you if you want Acrobat to attempt to create fields automatically where it thinks they should be. The form will then be opened in form editing mode and you can add/edit the fields.

I know that you can use two credit cards to pay an item but the problem is that i can only put the billing address of just one of the credit cards, and since the two cards are from different people (my parents) i can't do it. They live in the same place so i don't know if i should just put one of the billing address.

I don't believe Apple is setup for tax exempt purchases. It's strictly comsumer oriented.
Are you making a profit on your book sale? If so just up the price to cover the tax and you purchase it and deliver to the school. If not then reimburse the school for the tax and write it off as a deduction.

I am in Australia at the moment and when I try to purchase an app, it won't accept my credit card.

what do you mean it doesn't help? did you follow the steps stated below
Creating an account on iPhone, iPad, and iPod touch
Open the App Store on your device.
Find a free application on the App Store.
Important: Before proceeding to the next step, you must download and install the free application by tapping Free followed by tapping Install App.
   Tap Create New Apple ID.
Choose your country, and then tap Done.
   You'll need to read and agree to the iTunes Store Terms & Conditions. Tap Agree located at the bottom of the page to continue.
Enter your email address, security questions, birthday, and create your password.
You also can enter an optional rescue email address. If you frequently change email addresses, it is recommended that you add a rescue email address. Learn more about the rescue email address.

my problem solve by me
first create apple id
fill credit card details
and complete your account creating process.
than go to app store or itune store
login your acount
click right side - account button
than again login for account setting
next go to payment information and click edit button
when u enter payment infomation
click none button in payment method and click done button.
than ur credit card has been removed.
but rs. 60 will deducted in your account when u doing this process.
