You will not find readymade document, you need to add this as per your requirement.

How can I get a refund for an in app purchase I did not want?
The app took over $35 from my account and there is no way this was authorized. Please advise on how I can recover these funds.

They did the same thing to me on 12/24/11. Apple overcharged me 3 times my purchase amount and have caused my bank account to be overdrawn!
I called my bank to make a statement of unauthorized charges AND filed a complaint with the FTC (Federal Trade Commission). Apple provided no way to contact a live rep and the email reps have been useless...so they left me no other option.
I think they do this to their customers very often. It is extremely dishonest.
I actually posted the above info on this discussion community earlier today...and Apple deleted it! If that isn't an admission of guilt, I don't know what is.

You can't. But if you backed up your computer prior to wiping it out, you should be able to restore your iTunes library, contacts, photos, etc. from that backup. Once you do, you can set the phone up again and just sync everything from your library back to it.
If you didn't back the computer up either, you can transfer items purchased on the phone into iTunes, and if you set up at least one contact in whatever application you sync your contacts to on the computer, it will offer to merge the address books on the first sync, saving those. Any media you'd acquired from sources other than the iTunes store will have to be re-downloaded from the source or re-ripped.
The iPhone is NOT a backup device.

Yes I have read of other reports from people saying theirs are fine. Go into your itunes store account and click on purchase history. Click on "Report a problem" at the bottom of the page. If it was your last purchase it should be at the top and have a link that you can click on that says "Report A problem". Follow the steps the screen gives from there. This is not a guarantee that you will receive a refund (or store credit which is what they would give) but one of the options is "Accidently clicked Buy" or something to that effect.
