You need either a credit card or iTunes gift card to create an iTunes account. An App store account can be created without a credit card by following the directions here:

Create a NEW account using these instructions. Make sure you follow the instructions. Many do not and if you do not you will not get the None option. You must use an email address that you have not used with Apple before.

I want to make an in-app purchase, but it fails and tells me to contact the Itunes support. I did that and they pointed me to
update my billing / payment info. However, I have NO credit card available and payment info is set to NONE. I want to use my gift card for the in-app purchase.
Are in-app purchases only possible with a credit card? That would be weird, because i can usually buy and download apps from the appstore.

You can't change back to the U.S. Store without a credit card or at least a gift card, or any valid payment method.
The none option would only appear when you create accounts. The iTunes Store requires that you provide a payment method and a billing address to change stores in order to validate your residency in the country you're changing to.
