How do a get a record of the apps I have downloaded

You can view the apps that are still available in your country's app store via the Purchased tab in the App Store app on your phone.
If you want to view your account's purchase history then log into your account on your computer's iTunes via the Store > View Account menu option, you should then see a Purchase History section with a 'see all' link to the right of it. Click on that and you should then see a list of your purchases. That should show free as well as paid-for items. On your phone you can view the last 90 days purchases via http://reportaproblem.apple.com.
You can't view a list of actual downloads (e.g. you can't view how many times, and when, you've downloaded a particular app), only what you've purchased (included free items).

    You don't. It's how things are done now, likely to accommodate those that have auto update turned on.  IT's been said that the updates will just drop off after a period of time but  time will tell on that.

    She must have signed into her own Apple ID on your iPad in order to sync her content from iTunes so try this and see if it works.
    Settings>Store>Apple ID. See if your ID is showing in there and if it isn't - tap on her ID, sign out and then sign in with yours.
    You should not be syncing to to different iTunes libraries. The iPad can only sync with one iTunes library without erasing the the content on it if you decide to sync to another iTunes library.

    Along with what Skydiver119 has already told you, my 64GB iPad 3 only had 57GB of available storage out of the box, so if you keep doubling the amount of already used storage on the device as you go from 16 to 32 to 64 and then to 128GB - I assume that the figure that you see is correct. The iOS and preinstalled apps account for the storage that seems to be "missing" to you on your iPad.
    The amount that you see in Settings>General>About>Capacity is in fact, the available storage capacity for your iPad. You can't change it. It is whatever it says it is in that setting.

    I had to reinstall all my apps when I upgraded to the new iOS - while some were listed in my 'purchased' list a lot weren't and I had to go through all my iPhone apps to see what was missing. And my folders all vanished as well!  Any hints or tips before I upgrade the iPhone ?

    Take a photo of each page of your Phone's Home screen with your iPad's camera. This will not only give you a record of the apps but what page of the screen you had them on!
    I do not have an iPhone but if it gives the facility of storing multlple apps in a folder then make sure that they are ungrouped before taking the photographs.

    I bought GarageBand from the app store and have tried loading it on my two mac computers one at home one at work. The home one will not download it at all - it keeps getting so far and then (helpfully) saying 'an error has occurred'.
