How do I access documents in iCloud.

I can't find an iCloud icon & iCloud.com doesn't work? It tells me to set up iCloud but I already did that.

You can't access iCloud.com from an iOS device. To access data, use the appropriate app, like contacts, calendar, etc.
There is no iCloud icon. Are you trying to access a general storage area to move files to, like Dropbox or the now terminated iDisk?  iCloud doesn't off such a feature.
What kinds of files are you trying to access on iCloud.

    To use iCloud Drive and to work with documents, you will need the corresponding Pages, Keynote and Numbers iOS apps or third party iOS apps that support iCloud Drive to interact with those documents.     Sorry, I do not think rereading is any good. The fact is: I do not see what I "should" see.
    I have checked that both on IOS and Mac I use the same icloud account, both have iCloud Drive enabled. On the Mac I have e.g. a custom folder "History"
    On IOS, in Documents 5 by Readdle, after tapping icloud in the sidebar and then the iCloud Drive text, I see this: (in list view, alphabetically arranged):
    So, my "History" folder does not show up at the "H" and not at the end.
    I thinks there is still a fair bit of iCloud related "should's" that do not materialize in practice.
    For example, this same History folder is 1.11 GB in size, and when I added it a few months ago, not all files were synchronized (I checked after a week!).
    You never get a positive signal that all files have been uploaded. Thus far, iCloud leaves me with that same shaky iDisk feeling of 2007!

    Set up the new phone as a new iPhone (without using your backup), then update it to the latest iOS (7.1.2).  Then go to Settings>General>Reset, tap Erase All Content and Settings, go through the setup screens again and when given the option, choose Restore from iCloud Backup and follow the prompts.  This process is explained in more detail here: iCloud: Restore your iOS device from iCloud.

    Where are you expecting to see "iCloud document?" Pages stores copies of your documents in iCloud but the documents appear in the Pages app. When you edit the document changes are automatically saved to iCloud. You do not go to iCloud to look for documents to open or copy.

    Try logging out of iCloud and back in.
    You could also go to icloud.com, sign in and see if you can get to the docs through the web version of your app (Pages or whatever).

    I personally just use a free Dropbox account to access Office documents.  You can upload them to your Dropbox account, then view them from within the free Dropbox app on your iPad.

    You can use iCloud to access documents created by apps such as Pages and Numbers. There is no way to store documents not created by apps specifically designed for iCloud.

    iwork on iOS uses different file extensions and formats (*.tef )  then iwork for Mac (*.number,*.key*.page) . It needs to be converted all the times so that the different apps can deal with the content of the documents.

    In fact, document can only be seen from within apps.  On your Mac, it will be from within Pages, Numbers, etc. and from your iOS device, it will be again from the related apps.  These updated apps for Mountain Lions have a feature to select where to open/save document; on iCloud or on Mac.
    From iCloud.com, you can go on the iWork icon to see all your documents.
    Again, this only work with Mountain Lion.
