How do I change page size of an existing .pdf?

I have Adobe Acrobat  XI Pro on my Mac running 10.6.
How do I resize an existing .pdf document that is 20" x 40" down to 4" x 8"?

Thank you Gilad D, Steve and Bill for your answers. Unfortunately, they don't solve my problem.
Acrobat does not have a virtual printer option for a Mac operation 10.6. The only way you can "print" to a .pdf is by opening up any application and using File -> Print -> PDF -> Save as PDF.
Any application, that is, except Acrobat.
The Acrobat print dialog box gives the user a robust number of options to manipulate the printing of a document. Sadly, there is no "print to PDF" option. When you choose "Printer" (lower left corner), you get the printer's default dialog box that gives making a PDF an option. However if you choose this, another dialog box pops up stating "Saving a PDF file when printing is not supported. Instead, choose File > Save." When you go to File > Save, it is greyed out (since you are using an existing saved PDF file). If you try Save As, you are not given the option to resize the page.
Alternatively, when you open the existing PDF file in Macintosh's Preview or Skim applications, you are given the option in the print dialog boxes to scale the image. However when you print to a PDF, the file size does not change (which is one of the driving motivations for changing the page size).
I can't believe that Adobe doesn't have an option in Acrobat to address this issue. While I appreciate the suggestions regarding InDesign (or even PhotoShop for that matter), I'd rather not drop $400 +/- for a program to solve a problem that a reasonable person would expect Acrobat to do itself.
Please... are there any other options?

    Go to the "page" inspector and select "layout". Here you can change the dimensions by using the up & down arrows or by typing in a new dimension and hit the "enter" key.
    You can also lengthen a page quickly by dropping a text box onto it as a "footer". Enter your copyright info into it then click hold and drag it down the page until you get the length you require.
    eg "Copyright © Sam - 2007 - All Rights Reserved"

    If you use the wrench icon and choose document setup, a blue background screen opens that allows you to change margins, at the bottom is a change paper size button, but it only seems to allow between A4 and US letter. Not orientation or other custom sizes.

    We have a PDF of a CAD drawing from a consultant.  The problem is, the page size is letter, and we need it to be tabloid to accommodate a section of General Notes and a Cost Estimate table.  We would rather not go back to the consultant both from a cost standpoint and from a time standpoint.
    Is it possible to increase the page size within the PDF so that I have some blank space at top or bottom for the needed blocks?

    You're referring to the navbar.  You can remove any page from the navbar by going to the Inspector/Page/Page pane while viewing the page and unchecking the box to include that page in the navbar:

    I scanned a 50-page document with an HP 5610xi All-In-One using the software that comes with it: HP Scan Pro. Though the physical document has standard 8.5" x 11" pages, the PDF resulting from scanning has page size set to 35.42 × 48.67 inches (according to Preview's "Tools > Inspector". I want to resize the pages to 8.5" x 11". Shouldn't Preview be able to do that with "Tools > Adjust Size..."? When I try, the menu option is grey so I can't select it.
    I'm using Preview Version 4.2.

    Thanks for the insight; that explains why it's the wrong size. I selected 300dpi scanning (which is the default, actually), but HP Scan Pro evidently created a PDF with 72dpi indicated. I want it fixed because I want to distribute the file to other people and have them able to print or display it without problems.
    I still don't know why Preview won't let me change it, but your question about whether I wanted to print it did lead me to find a workaround: "File > Print" and "Save as PDF..." saved it with the correct page size.

    When I try this, I can do the first step and I'm left with pages about 5.5"x8". But the Change Page Size function does not allow me to resize the page.

    Your only option is to use crop. What you are trying to do is generally done in the original document, not in Acrobat. If you want to scale it you can print it to a new PDF with the size you want and scaling turned on.

    All fields in InDesign can be entered in any measurement system. So, if you want to make an 8"x10" document and go to File>New Document and the window looks like this:

If you do that to an open document, you will change the unit of measure for that document. If you do it while no documents are open, it will change the unit of measure for any new documents you create. To change the unit of measure for existing documents, you will have to open each and make the change.

    TextEdit is not a very sophisticated program. As far as I know, there's no way to resize an image on the fly in TextEdit, so you'll have to follow Shotgun00's advice.
    I would advise not using TextEdit for anything real. Buy yourself a copy of Pages from the App Store instead, you'll be much happier with its capabilities if you intend to create complex documents containing images and text.

    A lot depends on the form of the PDF. Is it graphics, a scan of a text file, pure text, or other? What version of Acrobat are you working with.
    You can do a save as to get a WORD file, but do not expect great results. The ability to get a decent WORD file depends on what the form of PDF you are working from. If it was created from WORD with tags and all, you might get good results. If not, you might get a lot of messed up results.
    Explain what you are starting with and your ultimate goal. Also check the audit of the file (should be under PDF Optimize) to see where the file information is concentrated (text, fonts, graphics, other).

    You can't in Safari.
    However you CAN with Firefox and the add-on called Theme Font & Size Changer, also include NoSquint and set a global web page zoom level a bit higher, say 150%.
    NoSquint has buttons on the tool bar to increase/decrease each websites size AND IT REMEMBERS IT!
    Under the Tools Menu > Customize you can add more toolbars as well, but they have to have something in it of course.
    (note: the green with yellow type is a person you can add from thousands of choices, it's not the default look of Firefox)
    So this setup, although it may appear large here compared to Safari, is fine on my 17" when sitting back about 3-4 feet, takes up about 1/6th of the screen, so I'm not hunched up close, and I have a lot of screen real estate to play with anyway.
