How do I create a fillable PDF?

I have a document in Word that I want to use for employee performance reviews and I would like to make it a fillable PDF instead of messing with the spacing in Word when I enter the data. Not sure where to start, any help?

    I have a PDF form all setup and have added fields.  All I want to do is save a copy on a webserver via HTTPS that another person can login and view via HTTPS.  The form contains some sensitive information.  I can save to a mySQL database if needed, but I would like to just save the entire PDF will fields filled it and name it with a field name for idtification like LASTNAMEDATE.pdf.  What is the best way to save the PDF docs?  Should we email them instead?  The pDF is an employment application so I need to secure it from spying eyes.

    Once you receive either FDF or XML data files, you simply open the form in Acrobat, go to the FORMS menu, select the Manage Form Data, and select Import Data. You would selecton the data type and select the file. The data then fills out the form and you have basically the same information in the form as the user had. You would have the folks submit the data file to a web script on your secure server. I am not an expert on secure servers, but assume that would do the job. You may be able to use the ASP or other aspect of your server to collect the data file (a simple file submission, if you know how to do that -- I am still having a problem there, but think it is rather simple once you know). After collecting the file you can then automate what you do with it, but you would probably name it to keep track of the various submissions. I tend to include the date and time in such file names.
    You can import each received file into the original form or process the data with such packages as the FDF toolkit. You will still have to write some code to do the manipulation if you use the toolkit, it only provides modules for processing the files to be used in various programming languages.
    If I am not making sense, maybe I need to go to bed rather than answer posts.

    I see. It's true that form fields do not support spot colors. I know you can flatten form fields, converting the field text to regular text, and embed the fonts using Preflight. What I don't know is if you can then convert the font color to spot (maybe using Advanced > Print Production > Convert Colors), but it's worth looking into.

    In Acrobat 11 you'd select: Tools > Forms > Edit
    to go into form editing mode. You'll see the tools that are available, thought it may prompt you to autodetect the fields. I usually don't do this, preferring to create them myself.

    I try to explain: with ID I'd create a file with text form, tables, images with text forms on them etc.... Now I have to export that in PDF and I want to transform the forms (sorry for the bad english, I'm italian) in fillable ones in PDF. How can I do it?

    Export the ID file as an Interactive PDF.
    Open the PDF form in Acrobat XI  and then:
    File > Save As Other > Reader Extended PDF > Enable More Tools (includes for fill-in & save). . . Your users will be able to fill in the form and save it (assuming they have the later versions of Acrobat Reader).
    Make sure you keep a copy of your original PDF with a different file name as once you've created a Reader Enabled PDF Form you can't change it.

    You're in luck. Create the first field and then right-click it (in Form Edit mode) and select Create Multiple Copies. You'll be able to specify how many copies of the fields to generate, and at what intervals. The nice thing is that although the properties of the new fields will be identical to the source one, they'll each have a unique name, so you'll get an instant "table" of fields.

    i used this calculation on excel but now we wanted to send a fillable pdf via email, but the recievers of the email always mix up the calculation, so i wanted to have it already in the pdf and calculating by it own.
    i found basic things like field1 + field2  but i just saw that i need javascript to get what i need.
    but well, i dont have any idea of JS.
    maybe u can give me some links where i can learn it or something else.

    "Automatic Form Recognition" looks for blank areas next to text and assumes the format.
    You probably will need to place each individual form and write each script.
    You can find articles at Planet PDF and Acrobat Users Community and there a number of books on how to create and program forms.

    We would like to basically do a 'mail merge' of our list of customers with an interactive PDF file (including videos, menus, etc - not just form fill out and web links) to create a single PDF file that contains multiple mail pieces ... one for each customer ... with each mail piece being customized for that customer.  Customizations would include different greetings (Dear Bob, Dear Dana, etc), as well as different charts based on data unique to the customer, different photographs, etc.
    I've seen that InDesign and Acrobat Professional can be used to create an interactive PDF (such as from http://tv.adobe.com/watch/ask-the-adobe-ones/14-calling-rufus-about-interactive-pdf-making).  However I don't understand how I can insert data from a database, csv file, excel file etc into the PDF file so that each page, or each set of pages, within the PDF can be customized.
    Can anyone point me to a tool to use for this?

    For that kind of volume and unattended operation, you want InDesign Server – which is the server/high volume edition of INDD.

    You can make a PDF file out of you notes and handouts but using the Adobe PDF printer.
    Open your PowerPoint Document then go to “File” “Print”
    Select ‘Adobe PDF” Printer
    Under slides, select which way you want to print. Note it will not print to a printer but to a PDF file.
    After then go into printer Properties and Setup PDF properties, do the following: (see second image below)
    Default: PDF/A 1-b
    Adobe Security: none (change as needed)
    Adobe Output folder: Prompt for Adobe file name
    Adobe Page size: Letter (change as needed)
    Check the following check boxes
    View Adobe PDf Results,
    Add Document Information,
    Rely on system fonts only,
    Delete Log files for successful jobs.
    Select OK
    A pop will ask you where you want to save the document. The file extension should be PDF. After giving the file name select okay and the file should popup as a PDF. Down side is if you want to speaker notes included then you will have to do another file, like wise with just screen shots then combine into one PDF document.
    I used Acrobat IX Pro. So to do this you need Acrobat IX or X Pro.

    You start with a source / authoring file for the "static" content (e.g., Name, Last:). Say this is an MS Word file.
    You master the "static" content..
    You keep this authoring file and have a backup of this file.
    You output a PDF and use Acrobat to build and fine tune the "form" (form = the form fields, not the "static" content).
    The authoring file  and output PDF (with form fields) gives you "rev 00".
    Now, you must change something in the "static" content.
    Open the authoring file and save as to a new version (e.g., "wordfile_v00"  becomes "wordfile_v01).
    Output this to "pdffile_v01".
    Use Acrobat to open the PDF form file "pdffile_v00". Save As to a new file name ("pdffile_v00update").
    Using Acrobat XI Pro the click-path Tools - Pages - Replace lets you replace a page or pages of "pdffile_v00update" with a page or pages from "pdffile_v01".
    This replaces the "static" PDF page content and does not affect the form fields on the page.
    (you may have to adjust position of form fields)
    Save "pdffile_v00update" to "pdffile_v01" to reflect the revised status of the PDF form file.
