How do i deauthorize a dead computer after using the "deauthorize all" button withing 12 months?

Ask the iTunes Store staff to do it. What you can do on your own is limited to reduce the possibility of abusing the remote deauthorization functionality.

You can authorize a total of up to five systems, so unless you've hit that limit it's probably not really important that you deauthorize your dead unit now. Once you reach five total authorizations, a button will appear in your iTunes Store account that will allow you to deauthorize all your systems, so that would at that time recover the now-useless authorization.
    If for some reason you really feel you need to deauthorize a system and you're not at the five-system limit, you can try contacting  the iTunes Store customer support department through their Contact form (follow the on-screen instructions)  and explaining the problem to them. They might be willing to clear your authorizations for you.

    You can't deauthorise a 'dead' installation or computer on it's own. However once you reach the limit of 5 authorisations you have the option to deauthorise them all in one go from your iTunes account and start over.     There is no need to deauthorize unless you've reached the 5 computer max.  If you have reached that max, deauthorze all computers and then authorize the one's you actually have.

    iTunes > Store > "Deauthorize this Computer..."
    You can deauthorize individual computers, but only by using those computers. Otherwise you can "deauthorize all" from your iTunes account.
    Open iTunes on a computer
    From the Store menu, select "View my Account..."
    Sign in with your Apple ID and password.
    Under "Computer Authorizations" select "Deauthorize All".
    Authorize each computer you still have, as you may require.
    You may only do this once per year.
    After you "deauthorize all" your authorized computers, authorize each one as required.

    No you do not reload.  When you deauthorize, your music stays exactly where it is. 
    You can reauthorize at any time, provided the computer is in working condition and you are able to physically access it (and you still know your ID and password).

    On your daughter's computer you need to use Store > Deautorize Computer and enter your Apple ID and password.
    If you don't have access to her computer, but have 5 authorized in total, you can deauthorize all then reauthorize the ones that you want to. You can't repeat that process for a year.

    I have been working for months now trying to find a solution on how to deauthorize every computer I have used for my Ipod touch (every computer I have used has been mine, of course). But I've run into an issue: every computer I've authorized (5 total) have been either killed (hard drive died), or reformatted.     Open iTunes, select iTunes Store from the sidebar, click on the Support link under Quick Links. Post an email explaining the situation to seek assistance.
