    With the very limited information you have given, I would try a reset - hold the power button and the home button until you see the apple logo then release. Or, restore as a new device in iTunes. What does and does not your phone do?

    Disabled

    Need to know your Mac model. If it's a PowerPC, the max OS is Leopard 10.5.x. If it's an Intel procesor, you can upgrade to Snow Leopard (for $29) & later to Lion.

    I was having the same problem with a card that I received as a gift. Finally, I realized that it's an APPLE gift card and not an iTunes gift card. They are different and one will not work for another. The Apple card is used for hardware, software, cases, headphones, etc that you can buy in store or online. The iTunes card can only be used for music, apps, videos purchased through iTunes.
    But as long as it is in fact an iTunes card, the only other thing I've read about it not activating is to wait a few days and try it again. Sometimes it takes a while for the store where it was purchased from to send the activation info to Apple and read it as a valid code. I guess it doesn't happen instantaneously when scanned at the store.

    If you go to Settings do you see an entry for the Nike app? There is no way to delete or hide that app?
    Have you tired using the Spotlight search? Maybe you just can't find it.
    Next would be to restore a gain. First from backup then next to factory settings/new iPod.

    I posted about this yesterday -- it happened to me, and the solution was incredibly simple. An Apple tech guy had me delete my SC Info Folder, located on my XP in Application Data.

    When songs cannot be found, it's either because they have been moved from the original location that you told iTunes, renamed or deleted.
    So have you:
    renamed songs?
    moved them to a different loaction on your computer since you imported them into itunes?
    deleted them since you imported them?
    stored you iTunes library on an external drive? If so, that drive needs to be on and ready every time before you start iTunes.
    ... or is another programme renaming or even moving them, such as Windows Media Player? (It has been known to if you let it.) Did you previously use Windows Media Player?

    You can replace the back glass or get a case.  There are many iPhone repair centers... Google for one.  You can also try the Apple store... I've heard they will replace the back for about $30.
